[Hysteroscopic diagnosis of cervical involvement in the staging of endometrial carcinoma].

Hysteroscopy could be an interesting approach to evaluate the cervical invasion by endometrial cancer. From 2003 to 2004 we performed 30 hysteroscopy with endometrial biopsy with conclusive diagnosis of endometrial carcinoma and we calculated the sensitivity and specificity of hysteroscopy in assessing tumor invasion of the uterine cervix. Our data showed an high sensitivity and specificity of hysteroscopy that reported almost the same value of the histological examination. Our conclusions are that hysteroscopy is a suitable examination in endometrial cancer staging.

[AIDS and pregnancy: clinical and medicolegal reflections].

The authors discuss the various medicolegal aspects concerning the management of pregnancy in women with AIDS. They report the most recent data on the epidemiology and perinatal transmission of the disease. The effects of pregnancy on the evolution of disease as well as the effects of the disease on the evolution of pregnancy are discussed. A distinction is drawn between the progress of pregnancy in seropositive women and the progress of pregnancy in seronegative women. Lastly, the authors raise a number of questions of a medicolegal nature in the light of the legislation currently in force in Italy.

Menopausal hormonal status and vaginal microbiology.

The authors have examined endocrine function in women during postmenopausal age. They have also studied polymicrobic vaginal flora related to sexual hormone levels. Have been controllated correlations between vaginal microbic status and cytohormonal response at different hormonal situation present in the same woman. In postmenopausal women have been formed low oestrogen levels related to low presence of Doderlein. Perhaps in postmenopausal women other factors than oestrogen and progesterone levels, could influence vaginal polymicrobic flora.

Puberal hormonal status and vaginal microbiology.

The Autors have examinated endocrine function in girls during puberal age. Have been showed datums relative to cultures effected on vaginal secrets withdrawn from little girls in prepuberal age. They have also studied polimicrobic vaginal flora related to sexual hormone levels. Have been controllated correlations between vaginal microbic status and cytohormonal response at different hormonal situation. The Doderlein presence is as much greater so higher is the oestrogen and progesterone (E/P) ratio, while the Doderlein bacillus isn't present when E/P ratio is lower. Perhaps in puberal age other factors than oestrogen and progesterone levels, could influence vaginal polymicrobic flora.

Controlled ovarian hyperstimulation: comparison between u-hFSH conventional protocol and a personalized approach.

OBJECTIVE: To investigate the therapeutic effectiveness in infertile couples with anovulatory, mild endometriosis-related and unexplained infertility of a personalized approach to controlled ovarian hyperstimulation (COH) (group A) on the basis of body mass index (BMI) and waist/hip ratio (WHR) in comparison to a conventional protocol (group B). RESULTS: In group A, we obtained a significantly higher pregnancy rate and number of multifollicular cycles in comparison to group B. In addition, the pregnancy rate/ovulatory cycle and the number of ovulatory cycles were higher (but not significantly) in group A. In group B, we observed a not significantly higher number of monofollicular cycles and required length of stimulation in comparison to group A. CONCLUSION: A personalized therapeutic protocol, based on simple criteria such as BMI and WHR, leads to a higher number of single and twin pregnancies, without any significant increase of higher grade multiple pregnancies or severe ovarian hyperstimulation syndrome.

[Effect of hormone replacement therapy on postmenopausal ocular function].

OBJECTIVE: Determination of the effects of hormonal replacement therapy (HRT) on various ocular parameters and symptoms in postmenopausal women. DESIGN: Prospective study. SETTING: Department of Obstetrics and Gynecology, University "Federico II" of Naples. PATIENTS: 14 healthy women treated orally with equine conjugated estrogen in continuous (0,625 mg/daily) and acetate-medroxyprogesteron (10 mg/daily) from 17th to 28th day for three months. MEASURES: Ocular symptomatology, intraocular pressure (IOP), lacrimal secretion, reflected and basal and corneal thickness. RESULTS: After 3 months of HRT the IOP was reduced of 10.8% (p < 0.005), the lacrimal secretion, reflected and basal, increased of 19% and 48%, respectively and the corneal thickness increased of 16.6%. CONCLUSION: The HRT has a positive effect on ocular physiology.

[Underestimation of ovarian pathology. Clinical aspects and medico-legal considerations].

In this review are reported the data regarding medical and gynecological responsibility in case of underestimation of ovarian pathology compared to epidemiological data. It is important to recognize the characteristics of the symptomatology, in general very mild, and the diagnostic tests suitable to detect ovarian masses. Four steps in the management of ovarian masses which may generate errors in the diagnostic and therapeutical process are stressed: the clinical diagnosis phase, the surgical phase, the management of borderline tumors, and the follow-up. It is also highlighted the importance of the concept of the "delay" in the medical action, both diagnostic and therapeutical, and an involvement of the Public Health Service, both in the prevention and the treatment of ovarian lesions, is ipothesized.
