Significance of QT dispersion in the long QT syndrome.

The long QT syndrome (LQTS) has often been considered as a model to study the abnormalities of cardiac repolarization in humans because it represents a pure electrical disease with no evidence of cardiac structural abnormalities. The arrhythmogenic potential of prolonged ventricular repolarization has been extensively studied both in experimental models and at the clinical level in LQTS patients, and many studies pointed to the pathogenetic role of the dispersion of ventricular recovery times (i.e., dispersion of ventricular repolarization). In the last few years, a new critical knowledge has been achieved thanks to the molecular biology techniques that are unveiling the genetic bases of LQTS. Indeed, the understanding of the genes and mutations that may cause the LQTS opened the way to understanding the molecular determinants of the altered ventricular repolarization that can be found in LQTS patients. From the clinical standpoint, the traditional tools applied for the detection and quantification of the dispersion of ventricular repolarization (monophasic action potential, QT dispersion) showed their effectiveness but also their limitations. More recently, the availability of new algorithms and the development of powerful computerized supports allowed the evaluation of innovative techniques, which now represent possible attractive alternatives intended to quantify the degree of repolarization abnormalities in LQTS patients and possibly to noninvasively quantify the risk of cardiac events.

[A benign retroperitoneal schwannoma with vertebral lysis. A case report].

A case of a benign retroperitoneal schwannoma invading vertebral body is reported. Clinical manifestations are not specific. Imaging studies (U.S., NMR) provide detailed information with regard to its anatomical relations. The treatment of choice for this lesion, regarding its slow growth and exceptional malignancy, is a large removal of the tumour respecting major truncal nerves.

[Post-menopausal metrorrhagia: endometrial ultrasonic aspects].

Main causes of uterine post-menopausal bleeding are estrogenic substitutive therapy, ipo/atrophy, hyperplasia, carcinoma of endometrium and sarcoma. To evaluate the real usefulness of echotomography in these conditions, 128 patients in post-menopause (end of menstrual cycles from over an year) with at least one uterine bleeding were considered. The patients were submitted to pelvic ultrasonography in order to study uterine and ovarian morphology and, in particular, to examine endometrial echopattern. Endometrial reliefs were classified into 4 groups of echopattern and put in comparison with histopathology. From this comparison a well defined role of echotomography in the study of post-menopausal bleeding is evidenced: in cases of "abnormal echopattern" the probability of endometrial pathology is elevated (83.3%), with a good "sensibility" but without a so good "specificity" of ultrasounds: therefore its "subsidiary" but "necessary" role in the diagnosis of climateric uterine bleeding is emphasized.

[Ultrasonics in the diagnosis and staging of endometrial carcinoma].

In the study of endometrial pathology actually ultrasounds have a few utility in precocious diagnosis, but can be useful during the staging phase (echo-histopathologic correlations, tumor biometry, myometrial invasion). Present study is relative to 134 patients affected by endometrial carcinoma (histologically proved) and preliminarily submitted to echotomography. The "false negative" results of ultrasounds were 16/134. Echographic observations, classified according to Niwa e coll. and Obata, were put in comparison with anatomo-histopathology. The evaluations of tumor dimensions and myometrial invasion were correct respectively in 59.2% of cases and in 64.1% of cases. During the staging phase of endometrial neoplasia echotomography can allow a better pre-surgical definition, mainly with a more detailed informations about tumor biometry (T) and myometrial invasion (M).

[Ultrasonics in the follow-up of cases of non-surgically treated endometrial carcinoma].

Authors present an experimental study to evaluate the possibilities of ultrasounds in the follow-up of endometrial carcinoma inoperable cases. 18 women affected by endometrial adenocarcinoma (I or II stage F.I.G.O.) not submitted to surgery (due to increased surgical risk or refusal of operation) and treated by radio-hormonotherapy were considered. In these inoperable cases, echotomographic follow-up (in association to the usual oncologic specific follow-up) permitted to evaluate uterine modifications directly "in vivo": both for tumor dimensions and for myometrial invasion. Without reference to the effectiveness of radio-hormonal therapy, from our experience the utility of ultrasounds in the selection of "non-responders" cases is evidenced ("non-responders" cases were then treated by chemotherapy).

[Celioscopic digestive surgery: from the explosion to a critical reflexion].

A critical review of laparoscopic digestive surgery indications is proposed. Patient's benefit by laparoscopy is to undergo the same procedure as by open surgery with identical security conditions but with less postoperative pain and an earlier recovery. Today laparoscopy in digestive surgery is mainly indicated in the fields of functional and benign diseases. Oncology seems for our team to stay a relative contra-indication as yet. Taken into account the future technical progresses, this 1996 state of the art is obviously only a temporary view of the technique before expected mutations.

Cholecystectomy, Laparoscopic↗

[Duodenal lipoma causing intestinal subocclusion. Case report].

Duodenum is a rare position for gastrointestinal lipoma which sometimes may lead to severe complications. Authors report the case of a 60-year old woman treated with a transduodenotomic excision through a midline laparotomy. At admission the patient presented epigastric pain for 1 month. Physical examination was negative except for a modest epigastric sensitivity. CT scan showed image with regular features and fat density, and permitted the exact preoperative diagnosis. Final histopathological examination confirmed the diagnosis.

Hepatic resections through the laparoscopic approach.

Hepatic surgery has been undergoing progressive modifications in surgical approach to liver, passing through tohraco-phrenolaparotomy to bilateral subcostal incision and current Makuuchi's. Laparoscopic liver surgery should not be considered a new surgery, but simply a new surgical approach, with difficulties but advantages too. Laparoscopic hepatic resections are feasible with low morbidity and mortality; the short and medium term results are comparable to those obtained with open surgery provided that the surgeon has a significant experience in open hepatic surgery, advanced laparoscopic surgery and the availability of all and pertinent instrumentation. The aim of this paper is to show the rationales formal of hepatic resections through the laparoscopic approach, focusing on the necessary instrumentation, the surgical technique and results.
