ABR, an active BCR-related gene.

The human BCR gene on chromosome 22 is specifically involved in the Philadelphia translocation, t(9;22), a chromosomal rearrangement present in the leukemic cells of patients with chronic myeloid leukemia or acute lymphoblastic leukemia. In most cases, the breakpoints on chromosome 22 are found within a 5.8 kb region of DNA designated the major breakpoint cluster region (Mbcr) of the BCR gene. Hybridization experiments have indicated that the human genome contains BCR gene-related sequences. Here we report the molecular cloning of one of these loci, for which we propose the name ABR. In contrast with the other BCR-related genes studied to date, ABR represents a functionally active gene and contains exons very similar to those found within the Mbcr. Unlike the BCR gene, the ABR gene exhibits great genomic variability caused by two different variable tandem repeat regions located in two introns. All other BCR gene-related sequences isolated so far and the BCR gene itself are located on chromosome 22. In contrast, the ABR gene is located on chromosome 17p.

Internal amino acid sequence analysis of the 80 kDa protein kinase C substrate from rat brain: relationship to the 87 kDa substrate from bovine brain.

We have obtained sequence data from five proteolytic peptides totalling 102 amino acid residues of the 80 kDa protein kinase C substrate purified from rat brain. The amino acid sequences of these five peptides were compared with that deduced from a cDNA encoding the 87 kDa protein kinase C substrate from bovine brain. The overall amino acid sequence identity within the regions covered by our peptides is 54%. Two peptides aligned at the C- and N-termini of the bovine protein kinase C substrate with a very high degree of homology (more than 80% identity). Two other peptides exhibited 62% and 46% identity with two regions located in the C-terminal half of the bovine protein. The fifth peptide which contains the sequence PEQPEQPEQ did not reveal any similarity with the bovine protein. Based on the homologies of our experimentally determined sequences, which represent about 30% of the deduced sequence of the bovine protein, we suggest that although these protein kinase C substrates are not identical, they may belong to a family of related proteins.

Chronic aortic insufficiency: factors associated with progression to aortic valve replacement.

STUDY OBJECTIVE: To determine whether the initial measurement of clinical variables in patients with chronic stable aortic insufficiency is helpful in identifying patients at risk for earlier progression to aortic valve replacement. DESIGN: Prospective analysis of a cohort of patients for a median follow-up time of 44 months (range, 8 to 57). SETTING: Referral-based cardiology clinics at two university hospitals and their affiliated Veterans Administration medical centers. PATIENTS: Cohort of 50 asymptomatic or minimally symptomatic patients with chronic aortic insufficiency and left ventricular enlargement. Patients had preserved left ventricular ejection fraction at rest and no evidence of coronary artery disease or significant noncardiac illness. INTERVENTION: None. MEASUREMENTS AND MAIN RESULTS: Baseline evaluation included a history and physical examination, chest roentgenogram. M-mode echocardiogram, treadmill test, and radionuclide angiogram done at rest and during supine bicycle exercise. Ten patients progressed to surgery because of the onset of limiting symptoms or objective evidence of left ventricular dysfunction, or both; the overall rate was 4% +/- 3% per year. The Breslow and Mantel-Cox statistics were used to compute survival (surgery-free) dichotomized by prognostic variables. The progression to surgery was earlier in patients with left ventricular end-diastolic volume indices of 150 cc/m2 or more, end-systolic volume indices of 60 cc/m2 or more, a left ventricular ejection fraction at maximal exercise of less than 0.50, or an end-systolic wall stress of 86 dynes/cm2 or more. CONCLUSIONS: Patients at higher and lower risk for early progression to aortic valve replacement can be identified through the measurement of left ventricular size and function. This information can be used to decide the frequency and intensity of follow-up evaluation in these patients.

Life as a GDP--down under.

At the end of 1987 a colleague and friend from New Zealand offered me the opportunity to work in a general dental practice in his beautiful country for six months. It seemed an ideal way to combine some travelling 'down under' with living and working in Auckland thus affording the chance to get to know the country and its people better.

Transfer of radioactive caesium from soil to vegetation and comparison with potassium in upland grasslands.

The distribution and transfer of caesium and potassium between soils and vegetation has been investigated by field sampling and experimental studies on soils and vegetation typical of upland grassland in north west England. Total (137)Cs content to a depth of 0.05 m below root matt ranged from 13 000 to 18 000 Bq m(-2). This caesium content derives from three sources: the Windscale accident of 1957, weapons-testing fallout which peaked in the early 1960s, and the Chernobyl accident in May 1986. From 2200 to 6200 Bq m(-2) is attributed to the first two sources, and the remainder to Chernobyl. In December 1986, 23-78% of pre-Chernobyl (137)Cs was associated with soil underlying root matt and 0.5-5.5% was associated with vegetation. Plant/soil concentration ratios for pre-Chernobyl (137)Cs were in the range 0.5-6.5, the lowest values being associated with patches of Festuca/Agrostis turf. At the same time, 4-19% of (137)Cs deposited from Chernobyl was associated with vegetation, although higher values appeared in conjunction with the moss, Polytrichum commune. Corresponding total potassium contents were in the range 9.6-22 mg m(-2) to 0.05 m soil depth. Lower values were found at the wetter sites where, on average, 5.7% of the total potassium was present in vegetation. At drier sites the potassium content was higher and, on average, 8.9% was present in vegetation. Plant/soil concentration ratios ranged from 2.2 to 9.2. During accelerated growth of vegetation, on monoliths in glasshouse conditions over the winter of 1986/87, (137)Cs was transferred from soil and root matt to new growth, such that concentrations in fresh growth were similar to or higher than those observed in the field during December 1986. Removal of caesium by successive cuts resulted in up to 25% of the original estimated total being removed over a 240 day period. Increased concentrations coincided with the emergence of Carex sp. and Trichophorum caespitosum, as well as the development of Agrostis sp. and Festuca ovina. Observed Cs/K discrimination ratios, particularly for (137)Cs deposited from Chernobyl, were higher than previously reported in the literature. The experimental results were confirmed by field observations during spring 1987 and it is concluded that caesium deposited as a result of the Chernobyl accident will continue to be recycled in organic and low potassium soils.

Epicardial mapping: ventricular epicardial activation is unaffected by heart position.

Epicardial ventricular mapping was performed in five dogs during sinus rhythm with a sock array containing 41 bipolar electrodes. Maps were generated with a computer-assisted mapping system when the heart was in situ and when the heart was lifted by 44 degrees out of the chest. Times of earliest and latest epicardial activation in these two states did not differ. Despite a different frontal plane QRS axis, location of earliest activation was not affected by lifting the heart. In two of the five animals, the site of latest epicardial activation was minimally different from the heart in situ, but the general pattern of epicardial activation was unchanged. Therefore, the change in frontal plane QRS axis with lifting the heart was due to a change in heart position rather than a general change of heart activation.

The diacylglycerol kinase inhibitor R 59022 potentiates bombesin stimulation of protein kinase C activity and DNA synthesis in Swiss 3T3 cells.

Addition of the inhibitor of diacylglycerol kinase R 59022 to quiescent Swiss 3T3 cells potentiated protein kinase C activation by the neuropeptide bombesin, a potent mitogen for these cells. This was detected as a marked shift in the dose-response relationship for bombesin-stimulated phosphorylation of a Mr 80,000 protein, which is a major, specific substrate of protein kinase C. R 59022 also promoted the inhibition of [125I]epidermal growth factor binding induced by bombesin, an effect mediated by protein kinase C. A salient feature of our results is that R 59022, at concentrations that enhanced the activation of protein kinase C in intact cells (4-6 microM), potentiated long-term mitogenesis elicited by bombesin. Thus, R 59022 may be a valuable tool for elucidating the contribution of the protein kinase C pathway in mitogenesis.

Purification of a phosphoprotein from rat brain closely related to the 80 kDa substrate of protein kinase C identified in Swiss 3T3 fibroblasts.

A phosphoprotein expressed in rat brain is closely related to the 80 kDa substrate of protein kinase C present in 3T3 cells. The protein kinase C substrates from both sources migrate identically on two-dimensional gel electrophoresis and give similar phosphopeptide fragments when digested with protease. Using a series of chromatographic steps, including DEAE-cellulose chromatography, Sephadex G150 gel filtration and reverse phase fast protein liquid chromatography, this phosphoprotein was purified 3800-fold from rat brain. The preparation appears homogenous by one- and two-dimensional gel electrophoresis, is an effective substrate of protein kinase C and contains a high proportion of the acidic amino acids glutamate and aspartate, and of alanine.

Prenatal diagnosis of distal arthrogryposis.

We report the prenatal diagnosis of distal arthrogryposis type I by ultrasound at 18 wk gestation in a family with two other affected members (mother and sister of the fetus). The pregnancy was followed with serial ultrasounds, and the diagnosis was confirmed after birth. The clinical findings in all affected family members are described. A literature survey of prenatally diagnosed cases of multiple joint contractures is presented. These include cases with many different diagnoses. This is the first report of the prenatal diagnosis of distal arthrogryposis type I. It helps to illustrate the variability and prenatal natural history of the condition and the subtlety of the prenatal ultrasound findings.

Changes in dehydrodolichyl diphosphate synthase during spermatogenesis in the rat.

The levels of dolichyl phosphate and 2,3-dehydrodolichyl diphosphate synthase were determined in seminiferous tubules of prepuberal rats to assess any changes occurring during early stages of spermatogenesis. Dolichyl phosphate increased in concentration two- to threefold from Day 10 to Day 23 after birth. A method was optimized to measure dehydrodolichyl diphosphate synthesis from delta 3-[14C]isopentenyl diphosphate and t,t-farnesyl diphosphate in homogenates of seminiferous tubules. Both dehydrodolichyl mono- and diphosphates were observed as products of the in vitro assay. The specific activity of tubular synthase increased twofold between Day 7 and Day 23 and decreased similarly between Day 23 and Day 60. Since there was a parallel increase in the concentration of tubular dolichyl phosphate and dehydrodolichyl diphosphate synthase activity during early stages of spermatogenesis, it is proposed that the level of dolichyl phosphate may be controlled at least in part by the regulation of de novo dehydrodolichyl diphosphate biosynthesis. The synthase was also solubilized from tubular membranes with deoxycholate and partially purified by chromatography.

Dehydrodolichyl diphosphate synthetase from rat seminiferous tubules.

Homogenates of seminiferous tubules from rat testes catalyzed the incorporation of label from [14C]isopentenyl diphosphate into a variety of polyprenyl products. Long chain polyprenyl mono- and diphosphates were formed as major products when undesirable side reactions were minimized. The long chain polyprenyl diphosphate synthetase was measured as a sum of the mono- and diphosphate derivatives formed and was dependent on the addition of t,t-farnesyl diphosphate, isopentenyl diphosphate, and divalent cation. The highest activity was associated with the membranous fractions, whereas activity was negligible in the cytosolic fraction. The products of this prenyl transferase were labile to acid and yielded petroleum ether soluble products which indicated that the alpha-isoprene unit was unsaturated. Hydrolysis of either the polyprenyl mono-or diphosphates with a testicular phosphatase in the absence of NaF yielded C75, C80, C85, and C90 polyprenols. The chain lengths of the products of the synthetase suggest that this enzyme is responsible for the de novo biosynthesis of dehydrodolichyl diphosphates which are precursors of the dolichyl derivatives found in testes.

Static exercise with congestive heart failure and the response to vasodilating drugs.

The hemodynamic response to static exercise in 28 patients with congestive heart failure (CHF) was compared with that in 8 control subjects. Static handgrip exercise at 50% of the maximal voluntary contraction was performed to fatigue. In patients with CHF, pulmonary arterial wedge pressure increased from 20 +/- 18 to 31 +/- 10 mm Hg (p less than 0.001) (mean +/- standard deviation) and systemic vascular resistance increased from 1,730 +/- 454 to 2,151 +/- 724 dynes s cm-5 (p less than 0.001). Although cardiac index did not change significantly, stroke volume index and stroke work index decreased from 24 +/- 6 to 20 +/- 6 ml/m2 (p less than 0.001) and 28 +/- 11 to 25 +/- 12 g-m/s2 (p less than 0.05), respectively. In control subjects, pulmonary arterial wedge pressure did not change significantly; cardiac index increased from 3.6 +/- 0.3 to 4.0 +/- 0.4 liters/min/m2 (p less than 0.05) and systemic vascular resistance increased slightly, from 1,011 +/- 186 to 1,106 +/- 180 dynes s cm-5 (p less than 0.05). The effects of arterial dilation with hydralazine on the response to static exercise were assessed in 10 of the patients with CHF. Compared with predrug exercise, cardiac index increased 68% (p less than 0.01), stroke volume index increased 76% (p less than 0.01) and systemic vascular resistance decreased 47% (p less than 0.01) after administration of hydralazine. Thus, static exercise can have adverse effects on cardiac performance in patients with CHF.(ABSTRACT TRUNCATED AT 250 WORDS)

Selection criteria for surgery in patients with refractory epilepsy.

Between February 1983 and August 1986, 53 patients underwent inpatient evaluation at the Neuropsychiatric Institute, Prince Henry Hospital to determine their suitability for epilepsy surgery. The patients were first continuously monitored with a video camera during surface and sphenoidal electrode telemetry. During this phase, clinical, radiological, neuropsychological and psychosocial evaluations were performed and intracarotid sodium amylobarbitone injections were used to lateralise language and memory function. Following this evaluation, 30 patients were considered not suitable for further investigation. Six patients were able to have definitive surgery without further evaluation while 17 required depth electrode studies. These patients had flexible wires inserted into their frontal and temporal lobes under stereotactic guidance and video recording and depth electrode telemetry was continued until typical seizures were characterised. These studies led to a further 9 operations; one patient was suitable for operation but deferred surgery. When the original 53 patients were classified according to seizure type, the outcome was as follows. 1. Twelve patients had classical complex partial seizures (CPSs) only and 5 of these had temporal lobe surgery. 2. Five had CPSs with motor accompaniments and 3 of these had temporal lobe surgery. 3. Seven had CPSs and generalised seizures; one had temporal lobe and another frontal lobe surgery. 4. Four had partial simple seizures with secondary generalisation and 3 had cortical excisions (2 frontal, 1 occipital lobe) surgery. 5. Twenty-two patients had mixed seizure disorder; one had lesion excision and one had corpus callosum section in 2 stages. 6. Three patients were non-epileptic.
