The activity records of five groups of healthy or ill subjects have been measured for 4-26 days by an accelerometer placed on the nondominant wrist. These data, together with a record of times retiring to/rising from bed, have been used to produce a series of dichotomy indices for comparing the amounts of activity when in bed and out of bed. Reliable differences between individuals were found, with healthy subjects showing a greater degree of dichotomy than one subject with delayed sleep phase syndrome or three subjects with colorectal cancer. The method is convenient for extended data collection and offers the possibility of describing an individual's activity profile in a variety of circumstances.
The tetrapeptide AcSDKP (Seraspenide) has been described as an inhibitor of CFU-S entry into DNA synthesis; as a result, its administration can protect mice against lethal doses of cytosine arabinoside. We have studied the ability of AcSDKP to protect and promote the growth of early CD34+ human bone marrow (BM) stem cells in Dexter long-term cultures following exposure to a toxic concentration of mafosfamide. The protection assay was based on the preincubation of CD34+ BM cells with or without AcSDKP at 10(-10)M or 10(-8)M followed by exposure to a toxic dose of mafosfamide (100 micrograms/mL). The production of granulomonocytic progenitor cells (CFU-GM) was subsequently studied in long-term bone marrow cultures (LTBMC) of the samples exposed to mafosfamide and preincubated or not (control) with AcSDKP. After a lag of 2 to 3 weeks, the number of CFU-GM peaked at the 4th to 5th week in both the supernatant and the adherent layers. A greater production of granulomonocytic progenitor cells was observed in LTBMC from the samples preincubated with AcSDKP compared with control cells. Depending on the BM samples, enhanced production of CFU-GM in the AcSDKP-treated cell cultures was observed at either the 10(-10)M or 10(-8)M concentration. These results suggest that AcSDKP can protect in vitro human long-term culture-initiating cells (LTC-IC) from mafosfamide, resulting in an increased production of CFU-GM from this early stem cell compartment.
From observations made on the rhythms in different systems: changes in plasmatic concentrations of 5-fluorouracil administered at a constant rate, proliferative activity of human tissues (bone marrow and oral mucosa), actometry, variations of glutathione levels in the liver and kidney from ice, we show the existence of a 12 hours rhythmic component and we discuss the putative importance of this component.
Among 95 patients who have received a penile prosthesis between 1982 and 1993 (mean follow up of 46 months), 21 (22%) presented complications requiring one or more reintervention(s). Only for two patients the problem was mechanical, in the majority, they were non mechanical. Five of those subjects (5.5%) presented such complications that definitive explanation of the device was necessary. The problems presented by the 9 remaining patients were fully satisfactorily managed. Presently 94.5 per cent of the patients are using the device. Follow-up study of our interdisciplinary group demonstrates the efficacity of the prosthesis for revitalisation of the couples. The definitive parameters of implantation acceptance are essentially the partner influence and the treatment cost. This clinical study shows that the implantation of a penile prosthesis is an effective therapy in the treatment of impotence in a selected population. Presently, the mechanical reliability of different device has been good to date.
Human small cell lung cancer (SCLC) is a neuroendocrine tumour with a very poor prognostic. Receptors for somatostatin-14 and its synthetic analogue BIM23014 (Lanreotide) were characterized in 3 human SCLC xenografts (SCLC-6, SCLC-10 and SCLC-75) transplanted in nude mice. The binding activity of both iodinated ligands was tested by cross-linking assay. One major complex of 57kDa was identified by both ligands in all 3 tumours. Two other minor complexes were only detected by the natural ligand: 90kDa in all 3 tumours and 70kDa in 2 out of the 3 tumours (SCLC-6 and SCLC-75). Analysed by Northern hybridization, the expression of the gene encoding for the receptor subtype I was detected in all 3 tumours whereas the expression of the receptor subtype II was only detected in 2 out of the 3 tumours (SCLC-6 and SCLC-75). No receptor subtype III transcript was observed. The relative quantification of the detected messengers and of the cross-linked complexes determined by densitometry suggested that SCLC-6 contained a large amount of somatostatin receptors. SCLC-6 growing in nude mice was used to evaluate the antiproliferative effect of BIM23014. BIM23014 (250 micrograms, b.i.d. for 5 days) significantly inhibited tumor growth and had an additive effect with cis-platinum (1.5 mg/kg/day for 2 days) when given concomitantly. Values of the relative tumour volume as compared to control were: BIM23014 alone 57%, cis-platinum alone 57% and BIM23014 + cis-platinum: 78%. These experimental data suggest that BIM23014 given alone or in combination with cis-platinum could have a therapeutic potential in the treatment of somatostatin receptor positive SCLCs.
Several native peptides can regulate tumour cell proliferation. After binding to specific membrane receptors they have the ability to stimulate or inhibit directly cell growth. Peptides can also control the regulation of endocrine or paracrine growth factor secretion. Agonist and antagonist molecules have been synthesized for therapeutic purposes. Hypothalamic neuropeptides are used in oncology. GnRH agonists lead to biochemical castration which is useful in treatment of hormone-dependent tumours (breast and prostate). Somatostatin analogues are beneficial in the treatment of gut neuroendocrine tumours and have demonstrated an antitumoural effect in experimental studies. Cytostatic agents, such as Gastrin Releasing Peptide antagonists, may be of interest as an adjuvant to chemotherapy or surgery in small cell lung cancer and other malignancies. The role of peptides in antigenic presentation, cell proliferation control and the metastatic process suggests a new therapeutic potential for these compounds. Progress in biotechnology could provide specific tools to screen new molecules and increase the understanding of mechanisms of action. Improvement in drug delivery techniques will allow for more convenient routes of administration.
The impact of cancer on the patient's family and caregivers is important and it can be analyzed in terms of coping strategies. It depends on the patient (age, sex, role ...), the illness (site, stage, state ...), the people (parents, siblings, caregivers ...), the time (diagnosis, treatment, death, after death rehabilitation), the prognosis. The survey of the literature underlines the methodological difficulties met in this complex field and the distress of the family and caregivers at each stage of the illness, even during long term remissions.

From the clinical point of view, the perverse personality is characterised by constant manipulation of others, transgression of laws and disrespect of limits. Perversity does not refer to the presence of a sexual perversion, whether primary or associated, but to the fact that a same basic principle underlies the organization of the perverse personality and of the perverse sexuality. Both forms could thus be understood as two different manifestations of intolerance to disappointment. This general hypothesis avoid some questionable speculations and ad-hoc hypotheses.
A 55-year patient with obsessive-compulsive disorder showed clear improvement during 4 weeks of treatment with intranasal oxytocin compared to 4 weeks of intranasal placebo. This improvement was concurrent with the development of severe memory disturbances, supporting the amnestic properties of the peptide. However, the patient also developed psychotic symptoms and a marked decrease in plasma sodium and osmolality, which may have masked the obsessive symptomatology. This case highlights the need for careful monitoring in long-term oxytocin therapy.

The present work has been planned in order to elucidate the effect of phenobarbital (PB: 15 mg per rat of ingested dose) on carcinogenesis when it is administered simultaneously with diethylnitrosamine (DEN: 10 mg/kg/day). Wistar rats (180 g) were treated by DEN alone or by DEN + PB during 2, 4 and 6 weeks according to our schedule for hepatocarcinogenesis. After the end of the treatment, the number and the size of induced PAS positive preneoplastic foci was significantly reduced when PB was given simultaneously with DEN for 4 and 6 weeks. The mitotic inhibition and the production of micronuclei normally observed after partial hepatectomy in DEN treated rats were also significantly decreased in DEN + PB treated rats. When the treatment last only 2 weeks, the presence of PB did not change significantly the last parameters. In DEN + PB treated rats, the survival was prolonged and the tumor incidence decreased as compared with the results obtained by DEN alone. It is concluded that PB, which promotes carcinogenesis when administered after the DEN treatment, reduces the carcinogen effect when given simultaneously with DEN. This 'anti-carcinogen' effect acts on the initiation as well as on the promotion of the precancerous lesions. Biochemical investigations are in progress to obtain more information about this 'paradoxical' PB effect.

Are the objectivity and sensibility of quantitative psychopathology influenced by certain methodological conditions (time-blind evaluation or not, chronological order or random order, suppression of the time gap between two evaluations or not)? Different evaluations of AMDP videotaped interviews were not able to demonstrate systematic effects of these temporal conditions on the evaluation itself. Other variables (monotony, order of sequences, verbal inertia, contingencies) might well play a greater role. Within the methodological limits of the present study, the time-blind evaluation was the most sensitive.

The authors have elaborated a micro-computerized program which enables --on the basis of the Psychopathology Form (plus optionally the Somatic Form) of the AMDP-System and of an open-language diagnosis and treatment --to print out instantly a semi-standardized letter and a syndromic profile of 10 or 13 factors. This realization is favorably acknowledged by general practitioners and medical inspectors thanks to its speed of transmission, to its brevity and to the clarity of the syndromic reduction of the clinical picture. Other advantages lie in the greater validity of factor scores as compared to items scores, the standardization and readability of the clinical files, and the extension of the AMDP data bank.
