Maturation of caffeine N-demethylation in infancy: a study using the 13CO2 breath test.

Four premature neonates and eight infants 1-19 months old received caffeine for apnea. The usual morning oral dose was substituted by 1,3,7 13C-trimethylxanthine (13C-tri CAF) as the citrate salt. Five breath samples were collected the day before (day 1) and the day of 13C-tri CAF administration (day 2). Plasma (after each breath collection) and urine were collected on day 2. 13C-CO2 exhalation was determined by isotope ratio mass spectrometry. Caffeine and its metabolites were measured using high-pressure liquid chromatography. Assessment of the labeled CO2 in the breath revealed no detectable 13C-tri CAF N-demethylation activity in infants before 45 wk postconceptional age. However, demethylation (as urinary metabolites) has been detected before that age. Two-, 4-, and 6-h cumulative excretion of 13C-tri CAF as 13C-CO2 increased with postnatal age and correlated with caffeine plasma clearance (r = 0.840, p less than 0.01). These results were consistent with those obtained for urinary metabolites. In one infant (19 months old) the cumulative excretion of 13C-CO2 while crying was 65% of the value observed during quiet breathing. The measurement of caffeine demethylation using the caffeine CO2 breath test is feasible in infants and is a safe and noninvasive method to determine age related changes in P4501-dependent N-demethylase activity.

Spontaneous regression of gastric lymphoma.

Two cases of spontaneous regression of histologically diagnosed large cell lymphoma of the stomach are reported. Regressions occurred after nonspecific therapy including the H-2 antagonist cimetidine and were documented surgically. Spontaneous regression of intermediate and high-grade non-Hodgkin's lymphoma is distinctly uncommon. The possible role of cimetidine in these cases is discussed.

Cellular confluence determines injury-induced prostaglandin E2 synthesis by human keratinocyte cultures.

When keratinocyte cultures become confluent, their prostaglandin E2 synthesis is suppressed. To determine whether the injury response is characterized by increased prostaglandin E2 synthesis, an in vitro injury model was developed. When confluent keratinocyte cultures were focally lethally irradiated using ultraviolet light B, a dose-dependent increase in prostaglandin E2 synthesis was induced by the injury. After irradiation, confluent cultures' prostaglandin E2 synthesis increased for 2 days to 8-fold more than controls, then decreased to control values by day 6. Increased prostaglandin E2 synthesis was first detected 8 h after injury. Focal irradiation of non-confluent cultures (killing isolated colonies) caused no change in prostaglandin E2 synthesis, indicating that culture continuity must be disrupted before synthesis increases. In addition, partial irradiations of petri dishes demonstrated that enhanced metabolism was confined to cells adjacent to the injury site and was not mediated by a soluble factor. When confluent and injured cultures were incubated with [14C]arachidonic acid, and the products formed analyzed by thin layer chromatography, 10-fold more prostaglandin E2 microgram protein was seen in irradiated cultures relative to confluent controls. The products formed by each group were the same, and no consistent increases in metabolites other than prostaglandin E2 were observed. The increased synthesis of prostaglandin E2 by injured cultures was apparently due to an increase in cyclooxygenase activity as determined by kinetic experiments. These data indicate that the pattern of metabolism of arachidonic acid seen in non-confluent cultures is similar to that seen in injury, and that cell-cell contact modulates enhanced prostaglandin E2 synthesis.

Long-acting bromocriptine for the acute treatment of large macroprolactinomas.

Six patients with hyperprolactinemia, visual impairment and large macroprolactinoma were treated with a long-acting bromocriptine, in dosage of 50 mg as a single im injection. All patients underwent a full assessment of pituitary prolactin (PRL) secretion before treatment and the studies were repeated about every week during 40 days. Following bromocriptine injection a rapid amelioration of the clinical and visual defects occurred in all patients. Likewise, a significant reduction of the hyperprolactinemia was documented in all six patients and in three of them serum PRL levels reached values as low as 20 ng/ml, and a tumor size reduction was demonstrated by cranial computerized tomography in three of the six cases. No side effects were associated with the administration of bromocriptine. From this study we conclude that the long-acting bromocriptine represents an additional option for the initial management of large prolactinomas in particular when they are associated with severe visual impairment.

Depersonalization and agoraphobia associated with marijuana use.

This paper examines the role of uncontrolled depersonalization associated with marijuana use in the development of agoraphobia. Cases of six people are described, all of whom reported first experiencing depersonalization while using marijuana, and subsequently experiencing depersonalization while not using the drug. A fear of this 'uncontrolled' depersonalization resulted in considerable anticipatory anxiety and panic attacks. Patients ultimately presented for treatment of agoraphobia. A temporal relationship between marijuana use, uncontrolled depersonalization, panic attacks and agoraphobia does not imply causality. Comparison of these cases with other agoraphobia clinic patients provides tentative evidence for a difference between the two types of patients. There were no systematic patterns of stressors in the cases prior to the onset of symptoms. Data obtained before and after treatment indicated the cases were slightly more severe than clinic patients. Males and females were represented equally in the cases, whereas there was a higher incidence of females in the clinic patients. The cases' age of onset was younger than that of the clinic patients. Our standard cognitive-behavioural treatment programme required modification to account for the intensity of the fear of depersonalization in the cases.

Frequency of chromosomal abnormalities in embryos from superovulated merino ewes.

Chromosomal analysis was carried out on 48 Day 2-7 embryos collected from superovulated Merino ewes. Three embryos had abnormal chromosome complements (1 X 1N, 1 X 1N/2N, 1 X 3N), yielding an incidence of 6.25% abnormal embryos. It is concluded that superovulation does not cause an increase in the incidence of chromosomal abnormalities in embryos of Merino sheep.

Polyploid cells in blastocysts and early fetuses from Australian Merino sheep.

Cytogenetic examination was made of 103 13-14-day-old blastocysts and 116 24-32-day-old fetuses from untreated and androstenedione-7-HSA-immunized Merino ewes. There were no differences in the chromosome composition of blastocysts or fetuses from treated or untreated ewes and so the data were combined. At Days 13-14 a 1N/2N mosaic and a 2N - 1/2N/4N mosaic embryo were observed. In addition, 52 of the blastocysts were 2N/4N mosaics, with 8 of these also containing 8N cells, and one blastocyst was a 2N/8N mosaic. No aneuploid fetuses were observed, but 80 of the 116 fetuses contained polyploid cells, including 4N, 6N and 8N cells. The polyploid cells observed in the blastocysts and fetuses should not be considered as abnormal cells as they appear to be a normal part of the developmental processes leading to trophoblast formation and fetal differentiation.

Building a faith community: proclamation and performance.

What makes a health care organization Catholic? A publicly proclaimed emphasis on a Gospel environment is one aspect. Leaders in these organizations thus must direct their efforts toward developing and facilitating the growth of a faith community in the workplace. A faith community's task is to promote and sustain personal growth through interpersonal relationships that assist members to share Jesus. Religious institutes (congregations) must involve laity in building a community of believers who can share on a deep level. By following Jesus' example and giving themselves freely to one another, persons can carry out the mission of both the Church and the healing ministry. Five criteria characterize a Catholic organization: Purpose and goals linked explicitly with Jesus; Values, attitudes, and beliefs derived from the Hebrew and Christian Scriptures; Opportunities for faith celebrations; Corporate responsibility to apply the Gospel; Perception of being part of a larger missionary enterprise. Using these criteria, leaders can help develop a faith community by articulating and living the organization's philosophy and by determining with managers ways to translate these values into concrete action. Leaders should model tangible commitment to mission effectiveness, bridging the gap between proclamation and performance.

Cytogenetic analysis of 34 early stage bovine embryos from superovulated Hereford donors.

One hundred and thirty-four bovine embryos were collected from 17 superovulated Hereford cows and processed for metaphase chromosome spreads. Mitotic figures were obtained from 54 embryos (40%), but only 34 embryos (25%) provided analyzable chromosomes spreads. Thirty embryos were 2n = 60, one was a possible 2n/4n mosaic and three were hypodiploid. The hypodiploid cases and the 2n/4n mosaic could be due to technical artifacts.

The familial occurrence of agoraphobia.

Agoraphobia is believed to run in families, but this belief is not well supported by the literature. Data were gathered on the risk of agoraphobia in parents and siblings of 60 agoraphobic probands; 12 1/2% of the relatives were reported to have or have had agoraphobia. This risk is significantly greater than estimates of the population incidence, and requires explanation. The details of the pedigrees do not directly support simple genetic models while investigations into environmental factors in agoraphobia by other workers have been inconclusive. As a putative case has been established for the familial occurrence of agoraphobia, further work is required to shed light on whether genetic or cultural factors are paramount.

Occurrence of haploid and haploid/diploid mosaic embryos in untreated and androstenedione-immune Australian Merino sheep.

Chromosome counts were obtained from 73 out of 177 (41%) early cleavage-stage Merino embryos. A further 13 embryos were classified as probably diploid. Chromosome aberrations were found in 8 (11%) embryos, one of which was aneuploid and the remainder (9.6%) had euploid abnormalities. If the probable diploid embryos are included, the incidence of euploid aberrations falls to 8.1%. Of the abnormal embryos there was one aneuploid with 2N = 55, two haploids, four haploid/diploid mosaics and one zygote with 4 haploid metaphase plates. Two additional zygotes had 4 interphase pronuclei. Four of the euploid abnormalities were attributable to the entry of two or more spermatozoa and therefore polyspermy is the largest single factor leading to chromosomally aberrant embryos in this population of Merino ewes.

Enzyme variability in natural populations of Daphnia carinata king.

Fifteen populations of the cladoceran crustacean, Daphnia carinata, were examined for allozyme variation at sixteen gene loci. Individuals of this cyclic parthenogen were heterozygous at only 2.1 per cent of their loci. Large differences in gene frequencies existed among the populations. Within populations genotypic frequencies were in good agreement with Hardy-Weinberg expectations, and remained stable during annual cycles of population growth. Strong linkage disequilibrium was observed between the allelic variants at two leucine aminopeptidase loci in certain populations, suggesting that the loci are tightly linked and that populations of D. carinata are founded from few individuals.

A comparison of chromosomal and allozymal variation across a narrow hybrid zone in the grasshopper Caledia captiva.

A hydrid zone between the Moreton and Torresian taxa of the grasshopper Caledia captiva in S.E. Queensland has been characterised in terms of allozyme and chromosome variation within the same individuals.--On chromosomal criteria (pericentric rearrangements), the zone is asymmetrical with evidence of high levels of introgression of Torresian chromosomes into the Moreton taxon. This is apparent from the analysis of two independent transects across the hydrid zone. Major changes in chromosomal frequency occur over distances of less than 0.5 km. and the level of introgression differs between the two transects, with much higher levels in the northern Moreton populations, characterised by an acrocentric X-chromosome, when compared with the southern metacentric-X Moreton populations. Chromosome analysis of samples taken from the same transect over two years has revealed no major changes in the structure of the zone. Moreover, a Moreton population located only 0.5 km. from the null point was found to be stable over 6 generations with evidence for a new balanced genome having originated following the differential incorportation of Torresian chromosomes.--Contrary to the chromosomal situation, the same hybrid zone was found to be symmetrical with respect to allozyme variation with evidence of movement of diagnostic alleles in both directions across the zone. The alleles are independent and not tightly linked to any of the pericentric rearrangements. Thus these 5 alleles are acting as markers of the background genome and reveal the relatively free movement of genes which are located outside the pericentric rearrangements.--It is proposed that the hybrid zone in Caledia captiva is unstable and is moving slowly in a westerly direction into the Torresian territory. This is due to the ability of the Moreton taxon to incorporate more readily into its genome those Torresian chromosomes or chromosome segments which increase the fitness of the Moreton taxon. On chromosomal criteria, the Torresian taxon does not share the same capacity.--It is suggested that, so long as the two taxa retain their ability to hybridise with subsequent asymmetrical introgression, the zone will continue to move westwards and eventually lead to the selective incorporation of the Torresian genome into the Moreton taxon. This will result in a polymorphic situation with clinal variation in chromosomal frequencies. The structure of the zone is dependent upon a fine balance between genomic reorganisation in recombinant genotypes and the relative dispersal capacities of the two hydridising taxa.

Epidermoid cysts of the spleen.

In the two cases presented here, the sonographic findings were unique. At low gain settings the rounded masses appeared sonolucent and smooth walled. As the gain was increased, multiple internal echoes appeared close to the inner wall, indicating the thick and irregular wall lining. It is presumed that these echoes were produced by the trabeculae. It is felt that the sonographic findings in epidermoid cysts of the spleen may be diagnostic, obviating the need for arteriography.
