[Recombinant human erythropoietin: 18 months of continuous use in substitution hemodialysis].

It has been shown that the regular administration of EPO permits the correction of anemia in end stage renal failure patients. We analyzed the effect of chronic administration of EPO in 13 stable, regularly-dialysed end stage renal failure patients over an 18 month period. The effects of EPO were evaluated according to standard criteria including clinical status, blood pressure control, hematology and biochemistry data, protein nutritional status and dialysis efficiency. Following a 2 week control period, EPO was administered intravenously after the dialysis session according to a 2 phase protocol. The first period (correction phase) consisted of a stepwise EPO dose increment, starting at 24 IU/kg x 3 times and doubling the dose every 14 days according to hemoglobin response in order to achieve a target hemoglobin level of 11 g/dl. In the second period (maintenance phase) EPO dose was optimized to maintain the hemoglobin level between 10 and 11 g/dl, by adjusting either the dose or the frequency of injection. Anemia was corrected in all patients within 10 weeks with EPO dose increasing from 72 to 360 IU/kg/week. The stabilisation of hemoglobin was achieved with an average EPO dose of 275 IU/kg/week (50 to 476 IU/kg/week). Concomitantly, a subjective and clinical improvement was noted in all patients. The dialysis efficacy, although remaining in an acceptable range, fell significantly by 10% over the first 3 months of treatment, remaining stable afterwards, yielding an effective urea clearance near to 120 1/week. The dietary protein intake calculated from urea kinetic modelling ranged between 1.1 and 1.2 g/kg/day.(ABSTRACT TRUNCATED AT 250 WORDS)

Double-blind, placebo-controlled study of nicergoline in the treatment of pruritus in patients receiving maintenance hemodialysis.

Pruritus is a common symptom among patients undergoing long-term hemodialysis. The effect of nicergoline, an ergoline, on pruritus was studied in products released during dialysis. In a first experiment series, 20 age-matched normal individuals, 25 patients receiving hemodialysis without pruritus, and 15 patients receiving hemodialysis with pruritus had intradermal tests with 500 micrograms of papaverine. All patients with pruritus had a small papaverine skin test response, and mean values were significantly (p less than 0.0001, Mann-Whitney U test) smaller in patients with pruritus. All patients with pruritus entered in a crossover, double-blind trial with nicergoline. In a first period of six dialyses, they received either nicergoline (daily oral dose, 30 mg, and intravenous dose during dialyses, 5 mg) or placebo. In the second period of six dialyses, patients received the crossover treatment. Nicergoline was effective in 13/15 patients, eight of these patients having a complete remission of pruritus. When nicergoline was stopped, patients relapsed within 24 to 48 hours. All patients who improved during the trial were then treated with a daily dose of 30 mg of nicergoline for 6 months. Seven patients had a complete remission, five had moderate symptoms, and one patient relapsed. This study demonstrated that some hemodialyzed patients with pruritus of unknown etiology had improvement with nicergoline.

High-molecular-mass proteins in haemodialysis-associated amyloidosis.

1. Protein constituents were determined in eight amyloid deposits from eight patients (five male and three female), 53 +/- 4 years of age, treated by haemodialysis for 9-20 years using only cuprophane membranes and operated for carpal tunnel syndrome. 2. Soluble proteins were removed by solubilization in phosphate-buffered saline after osmotic lysis. The proteins of the insoluble fibrils were characterized by sodium dodecyl sulphate/polyacrylamide-gel electrophoresis and two-dimensional gel electrophoresis, and immunologically identified by Western blotting. 3. In addition to beta 2-microglobulin, alpha 2-macroglobulin was identified in the fibrillar material. The presence of these two proteins in amyloid deposits was confirmed by immunofluorescent microscopic studies. 4. Our data confirm the presence of beta 2-microglobulin in haemodialysis-associated amyloidosis, and also suggest a possible role for alpha 2-microglobulin: it may protect beta 2-microglobulin from proteolytic digestion, leading to its accumulation in intact form and to amyloid fibril formation.

Germicidal effectiveness of Dialox, a new stable peroxyacetic acid solution, in the re-use of high-flux dialysers.

In this study we evaluate the effectiveness of a newly available peroxyacetic acid solution (Dialox) as a disinfecting agent in the re-use of highly permeable dialysers. The germicidal properties of Dialox were tested in an in vitro trial on previously used haemodiafilters (HF80, Fresenius) highly contaminated with Pseudomonas aeruginosa, Mycobacterium smegmatis or sporulated Bacillus cereus. Complete freedom from bacterial contamination was observed 5 min after the reprocessing treatment on a Renatron reprocessing machine, using the currently marketed Dialox concentrate.

Contrasting effects of acute angiotensin converting enzyme inhibitors and calcium antagonists in transplant renal artery stenosis.

Deterioration of renal function is a major concern during treatment by converting enzyme inhibitors of hypertensive kidney recipients with transplant renal artery stenosis. However, there has been no assessment of the frequency of this complication and its specificity for converting enzyme inhibitors as compared to other antihypertensive drugs. The effect of acute administration of captopril on mean arterial pressure, glomerular filtration rate (GFR) (creatinine clearance) and effective renal plasma flow (clearance of 131I-hippuran) was assessed in eight hypertensive patients with transplant renal artery stenosis. Captopril induced a decrease in mean arterial pressure (128 +/- 6-121 +/- 7 mmHg) and a reduction in GFR (59 +/- 8-44 +/- 8 ml/min per 1.73 m2, P less than 0.05). The decrease in GFR was observed in seven out of eight patients and varied between 0% and 100% of the pre-captopril value. Effective renal plasma flow was maintained (157 +/- 47-141 +/- 24 ml/min per 1.73 m2) and filtration fraction decreased by 15 +/- 7%. The effect of captopril was compared to that of nifedipine (N = 20 mg) in four patients. Despite a larger decrease in mean arterial pressure (130 +/- 7-109 +/- 10 mmHg), no reduction in GFR was observed (68 +/- 13-71.4 +/- 8). Effective renal plasma flow was unchanged and filtration function slightly increased. Surgical or percutaneous transluminal angioplasty in five patients suppressed the captopril-induced decrease in GFR.(ABSTRACT TRUNCATED AT 250 WORDS)

Prostacyclin (epoprostenol) as the sole antithrombotic agent in postdilutional hemofiltration.

Prostacyclin (PGI2), the most potent and short-lived antiplatelet agent known today, has been used successfully as an antithrombotic in hemodialysis (HD). However, its vasodilatory effect has been the source of blood pressure instability in acetate HD and has restricted its use to bicarbonate HD. The authors took advantage of the better cardiovascular stability obtained with hemofiltration (HF) to compare the effects of PGI2 versus heparin either with acetate or bicarbonate HF in 4 patients. Efficacy of PGI2 in preventing thrombosis of the extracorporeal circuit was demonstrated in all cases with a dose of 4 ng/kg/min. HF performances remained unaffected whatever antithrombotic agent was used. Platelet activation as shown by BTG and PF4 release was inhibited by the PGI2 infusion. Platelet proteins release was greater with acetate HF, suggesting that acetate may have a specific role in platelet activation. Although the use of PGI2 was straightforward, it is worth noting that PGI2 partially suppressed the cardiostability usually associated with acetate HF. We conclude that the efficacy of PGI2 was well maintained in spite of conditions of high platelet shear stress conditions, but also that PGI2 potentiated the vasodilatory effect of acetate and suppressed partially the cardiovascular benefits of HF.

Kaposi's sarcoma of the bronchial tree in a renal transplant recipient.

An unusual case of disseminated Kaposi's sarcoma with endobronchial lesions is presented; these localizations are quite uncommon in the course of the disease, particularly in kidney transplant recipients. Some of the multifactorial etiologies of Kaposi's sarcoma are present: immunosuppression, ethnic origin and CMV infection. We emphasize that the diagnosis of bronchial involvement can be made during fiberoptic bronchoscopy and confirmed by biopsy without excessive complications.

[Acute cytomegalovirus glomerulonephritis in a renal transplant patient].

We report on an acute glomerulonephritis with renal failure in a living-related renal allograft recipient with a cytomegalovirus infection. The clinical presentation was typical of an acute nephritis. Renal biopsy showed lesions of diffuse endocapillary proliferative glomerulonephritis. The disease resolved spontaneously. The pathogenesis of cytomegalovirus glomerulopathy is briefly discussed.
