A critical reevaluation of some assay methods for superoxide dismutase activity.

We have compared the direct method of pulse radiolysis to the indirect methods of cytochrome c and nitroblue tetrazolium for assaying the superoxide dismutase activity of a compound. We have shown that with pulse radiolysis, where high concentrations of O2- are generated, the "turnover" rate constant, kcat, can be determined directly, while with the indirect methods, where relatively low steady state concentrations of O2- are formed, the value of kcat determined by these methods, can be orders of magnitude lower than that determined directly. The main reason for the lower values obtained with the indirect methods is due to the fast reoxidation of the reduced compound by molecular oxygen. Additional problems which arise with the use of indirect methods for determining superoxide dismutase catalytic activity are discussed.

Brain glucose utilization in childhood Huntington's disease studied with positron emission tomography (PET).

Brain glucose metabolism was measured in two children with early-onset Huntington's disease, using positron emission tomography with fluorodeoxyglucose (FDG) as the tracer. A marked (48%) hypometabolism was found at the level of the caudate nuclei, but other areas of the brain, particularly the cerebral cortex, were not significantly affected. Despite its different clinical presentation, Huntington's disease in children is characterized by brain metabolic alterations similar to those found in adult patients.

Atrial natriuretic peptide and adaptation of sodium urinary excretion in patients with chronic renal failure.

1. In order to examine the potential role of endogenous atrial natriuretic peptide (ANP) in modulating the increased sodium excretion per nephron in chronic renal failure, we studied healthy subjects with normal renal function (group I) and patients with moderate (group II) or severe chronic renal failure (group III) before, during and after administration of an intravenous sodium load. All subjects had been on a controlled diet containing 120 mmol of sodium per day for 5 days before the study. 2. Under basal conditions, plasma ANP and fractional excretion of sodium (FENa) were highest in group III. Both parameters increased in response to the sodium load in the three groups studied (P less than 0.001). Changes with time differed from group to group (P less than 0.05), the more marked response for both parameters being observed in group III. After adjustment with respect to plasma ANP (analysis of covariance), FENa was no longer modified in response to the sodium load, whereas adjustment of FENa with respect to mean blood pressure was without consequence on the significance of its change with time. This demonstrates that plasma ANP, but not mean blood pressure, represents the main factor producing variation in FENa during and after the sodium load. 3. These results suggest an important role for plasma ANP in promoting adaptation of short-term sodium excretion in response to an acute sodium load in patients with chronic renal failure who ingest a normal sodium intake.

Assessment of thallium-201 redistribution versus glucose uptake as predictors of viability after coronary occlusion and reperfusion.

Both 201Tl redistribution and persistent glucose uptake have been proposed as markers of viability after reperfusion. In the present study, they have been compared in the same open-chest canine preparation of occlusion and reperfusion. Ten fasting dogs were subjected to 2 hr of left anterior descending coronary artery occlusion and 4 hr of reperfusion. Myocardial blood flow was determined by a microsphere technique 100 min after occlusion and 3 hr after reperfusion. 201Tl was injected intravenously 20 min before reperfusion. Serial biopsy samples were obtained from ischemic and normal areas. 18F-2-deoxyglucose, a tracer of exogenous glucose uptake, was injected 3 hr after reperfusion. Thirty minutes before the animals were killed, simultaneous blood samples were taken from the femoral artery and the regional coronary veins draining the reperfused and the remote areas. Dogs were killed 4 hr after reperfusion was established. Area at risk was assessed by dye injection in vivo and area of necrosis by triphenyl tetrazolium chloride (TTC) staining, with confirmation by electron microscopy. Immediately after death, endocardial and epicardial samples were taken from regions characterized as risk regions, areas of necrosis, areas of patchy necrosis, and normal areas. These samples were counted in a scintillation well counter. Four hours after reperfusion, in ischemic myocardium (TTC positive) the relative 201Tl gradient between ischemic and normal regions was 26 +/- 13%, whereas in necrotic samples, this gradient was 71 +/- 26%.(ABSTRACT TRUNCATED AT 250 WORDS)

Regional brain glucose utilization in adenylosuccinase-deficient patients measured by positron emission tomography.

Regional brain glucose utilization was investigated by positron emission tomography with fluorodeoxyglucose in three children with adenylosuccinase deficiency. A consistent pattern was found in the three patients, namely a marked decrease of fluorodeoxyglucose uptake in all gray structures, with the exception of the cerebellum, which was minimally affected. Anomalies predominated in the cerebral cortex, particularly in the anterior regions; they were less pronounced in thalamus and basal ganglia. The observations suggest that positron emission tomography may be a useful tool for the localization of the deleterious effects of metabolic diseases and for the investigation of their pathophysiologic mechanisms.

New approach for oral administration of insulin with polyalkylcyanoacrylate nanocapsules as drug carrier.

Polyalkylcyanoacrylate nanocapsules (mean size 220 nm), composed of spheric polymeric structures, have been used as a drug carrier for insulin. The rate of encapsulation of insulin is 54.9%, and we studied the therapeutic efficiency of the nanocapsules in diabetic and normal rats. When administered subcutaneously, insulin-loaded nanocapsules prolonged the hypoglycemic effect of insulin; the duration of this effect increased with the doses. When administered orally by force-feeding to diabetic rats, insulin nanocapsules (12.5, 25, and 50 U/kg) decreased fasted glycemia 50-60% by day 2. This effect was maintained for 6 or 20 days with 12.5 or 50 U/kg, respectively. Only the dose of 100 U/kg decreased fed glycemia by 25% in diabetic rats. In normal rats, hyperglycemia induced by an oral glucose load was reduced by 50% with the same dose of oral insulin nanocapsules. We concluded that polyalkylcyanoacrylate nanocapsules preserve the therapeutic effect of insulin when administered orally and prolong this effect when administered subcutaneously and orally.

[Pyogenic sacroiliac arthritis in children. Apropos of 6 cases].

Pyogenic arthritis of the sacro-iliac joint is uncommon in children; 6 cases were observed over 4 years. The diagnosis may be difficult and should include specific examination of the sacro-iliac joint. Posterior pain with a febrile illness needs radiological examination and a skeletal scintigraphy, which has always been positive in our experience. All the patients had a very good functional recovery after antibiotherapy and rest (bed or plaster).

Gallium67 scintigraphy in the diagnosis of glomerulonephritis.

To assess the role of gallium-67 (67Ga) scintigraphy in the diagnosis of glomerular diseases, we performed the following technique in 39 patients with glomerulonephritis (GN) who underwent simultaneous Ga scan and histologic examination. 72 hours after IV injection of Ga citrate (2 mCi), isotopic kidney activity (normally undetectable) was compared to the activity of the liver and quantified as follows: less than (1+), equal to (2+) or greater than (3+) that of the liver. Renal biopsy was performed at the same time to evaluate the type of the GN and to quantify interstitial cellular infiltration. Proteinuria, serum albumin and creatinine were measured. There was a significant correlation between the level of 67Ga kidney activity and the degree of proteinuria and hypoalbuminemia. On the contrary, no correlation was found between isotopic activity and the degree of renal failure or the degree of interstitial cellular infiltration. Increased kidney 67Ga activity did not appear characteristic of a specific histologic type of GN. Increased glomerular permeability may alter renal uptake of Ga; therefore 67Ga scanning does not appear to contribute significantly to the diagnosis and the follow-up of either primary or secondary GN.

Accumulation of polymorphonuclear leukocytes in reperfused ischemic canine myocardium: relation with tissue viability assessed by fluorine-18-2-deoxyglucose uptake.

Polymorphonuclear leukocytes may participate in reperfusion injury. Whether leukocytes affect viable or only irreversibly injured tissue is not known. Therefore, we assessed the accumulation of 111In-labeled leukocytes in tissue samples characterized as either ischemic but viable or necrotic by metabolic, histochemical, and ultrastructural criteria. Six open-chest dogs received left anterior descending coronary occlusion for 2 hr followed by 4 hr reperfusion. Myocardial blood flow was determined by microspheres and autologous 111In-labeled leukocytes were injected intravenously. Fluorine-18-2-deoxyglucose, a tracer of exogenous glucose utilization, was injected 3 hr after reperfusion. The dogs were killed 4 hr after reperfusion. The risk and the necrotic regions were assessed following in vivo dye injection and postmortem tetrazolium staining. Myocardial samples were obtained in the ischemic but viable, necrotic and normal zones, and counted for 111In and 18F activity. Compared to normal, leukocytes were entrapped in necrotic regions (111In activity: 207 +/- 73%) where glucose uptake was decreased (26 +/- 15%). A persistent glucose uptake, marker of viability, was mainly seen in risk region (135 +/- 85%) where leukocytes accumulation was moderate in comparison to normal zone (146 +/- 44%). Thus, the glucose uptake observed in viable tissue is mainly related to myocytes metabolism and not to leukocytes metabolism.

Effects of smoking on mental performance and vegetative functions in high and low CO absorbing smokers.

The present study investigated effects of smoking on mental performance and concomitant psychophysiological reactions in smokers differing in the strength of their habit. Toward this goal performance in two 30-min rapid information processing (RIP) trials separated by a 10-min smoking period was compared among preselected high and low CO absorbing smokers, nonsmokers, and smokers not allowed to smoke (n = 12 per group). Heart rate, finger pulse amplitude, and respiratory frequency were continuously recorded throughout the experimental sessions in order to assess physiological arousal and to estimate nicotine absorption through smoking. The RIP test consisted in the detection (button pressing) of triads of odd or even digits out of a series of single digits presented in a subject-paced manner on a screen in a pseudorandom sequence. Performance significantly increased from the first to the second trial in all groups, and this increase tended to be greater in both the high and low CO absorbers than in the two control groups. The similar development of RIP in the low and high CO absorbers is contrasted by differential vegetative responses to smoking. Smoking increased heart rate and respiratory frequency and produced peripheral vasoconstriction in the high CO absorbers only, suggesting the absorption of nicotine, whereas no nicotinic effects were noted in the low CO absorbers. The results are discussed in the light of the observed dissociation between psychological and physiological effects of smoking in the two groups of smokers, and the possibly differential role of nicotine for smoking motivation in the two groups.

Cardiovascular, electrocortical, and behavioral effects of nicotine chewing gum.

The cardiovascular, electrocortical, and behavioral effects of orally administered nicotine during rapid information processing were assessed in deprived female smokers. In a pre-post treatment design, 10 subjects received a 4-mg nicotine chewing gum and 10 subjects a placebo. The mental task required the subjects to watch single digits presented in a pseudorandom order on a screen and to press a button whenever the last three digits were either odd or even. The presentation rate decreased after each error and increased after each correct response and was used as the index of performance. Event-related brain potentials (ERP) to each of the three digits of the correctly answered triads were analyzed. The ERPs showed a distinct CNV potential for the second digit only (expectancy) and a P300 response for the third digit only (response decision). The mean EEG power spectrum was computed for each 5-min resting period, set before each trial and at the end of the session. A single administration of 4-mg nicotine chewing gum was followed by heart rate increase, acrodermal vasoconstriction, increase in theta and alpha frequency, decrease in delta power, and increase in the CNV magnitude. However, the chewing gum neither increased performance or reaction time nor decreased any ERP latencies or amplitudes, as has been reported after cigarette smoking.

In vivo measurement of carbon-11 thymidine uptake in non-Hodgkin's lymphoma using positron emission tomography.

Carbon-11 thymidine (TdR) uptake using positron emission tomography (PET) has been measured in ten patients with non-Hodgkin's lymphoma (NHL). The rate of TdR uptake (mean +/- s.d.) was of 0.009 +/- 0.006 mumol.100 cc-1.min-1 in low-grade NHL. This rate was 0.063 +/- 0.049 mumol.100 cc-1.min-1 in intermediate-grade NHL and 0.159 mumol.100 cc-1.min-1 in a patient with high-grade NHL. Lymphoma radioactivity reached a plateau at 0.42 +/- 0.22%. 100 cc-1 of the injected dose from 10 min after injection. The highest 11C uptakes were observed in the kidneys and in the liver (3.30 +/- 1.30 and 2.10 +/- 0.05%. 100 cc-1 of the injected dose, respectively). The lymphoma-to-muscle ratio was of 11.8 +/- 1.7, whereas the lymphoma-to-intestine ratio was of 1.5 +/- 0.7. Accordingly, the measurement of [11C]TdR uptake in the abdomen may need other imaging methods for adequate interpretation. The results suggest that [11C]TdR uptake using PET might be a method for noninvasively measuring cell proliferation in vivo.

Nonneoplastic circulating Sézary-like cells in cutaneous T-cell lymphoma. Ultrastructural, immunologic, and T-cell receptor gene-rearrangement studies.

Approximately 67% of peripheral blood lymphocytes in a case of nonmycosis fungoides-type cutaneous T-cell lymphoma, exhibited Sézary-like changes. Immunotyping showed a helper phenotype with an abnormally faint expression of the T3 membrane antigen, while quantitative electron microscopic study was suggestive of neoplasia. However, T-cell receptor gene study did not show any DNA rearrangement in Ficoll-separated blood lymphocytes, whereas clonal T-cell proliferation was simultaneously evidenced in cutaneous (non-Sézary-like) tumor cells. It is concluded that reactive nonneoplastic Sézary-like cells may be present not only in various benign conditions, as previously known, but also in T-cell lymphomas. This study provides further evidence for the nonspecificity of Sézary-like changes, and stresses the utility of T-cell receptor gene rearrangement study for diagnostic and prognostic procedures during the course of T-cell lymphoproliferative disorders.
