[Post-operative superior mesenteric arterio-venous fistulas. A report on one case (author's transl)].

Presentation of a particularly evoking case of a post-opérative superior mesenteric arterio-venous fistula revealed by recurrent intestinal hemorrhages, diagnosed by selective arteriography and treated by resection of the anevrysmal pouch and the adjacent bowel. Review of the literature concerning 25 other cases from which it can be concluded that the digestive angiography was the only complementary examination that allowed the diagnosis. The curative surgical treatment has a 15% mortality rate in relation with hemorragic recurrences.

[restoration of duodenal passage in severe postoperative syndrome after surgery for gastro-duodenal ulcers. Experience with 40 cases (author's transl)].

The authors analyse 40 cases of restoration of duodenal passage for severe functional sequelae after surgery for duodenal ulcer. The type of reoperation depended upon the initial operation and thus after gastric resection with gastro-jejunal anastomosis, 26 procedures involving transposition of the efferent loop to the duodenum (Soupault-Bucaille operation) were performed, 8 conversions of a gastro-jejunal anastomosis to a gastro-duodenal anastomosis and one interposition of a small intestinal loop according to the technique of Henley. After gastro-enterostomy with or without vagotomy, 3 gastric resections with gastro-duodenal anastomosis were performed, and one replacement of a gastro-enterostomy, by a pyloroplasty. Operative mortality was 3/40 cases and long term results proved to be good in 80% of the patients.

CCNU, vinblastine, and delalutin therapy in renal cell carcinoma.

Advanced renal cell carcinoma is relatively resistant to most adequately evaluated chemotherapeutic agents. The combination of CCNU and vinblastine, which has antitumor activity in a frog renal carcinoma model system, has demonstrated activity in initial studies in man. The current study investigated this combination of drugs together with a progestational agent, Delalutin. Seventeen patients with metastatic renal cell carcinoma were treated with CCNU, vinblastine and Delalutin. There were no objective responses. Four patients with stable disease had a mean survival of 18 months compared to 13 patients with progressive disease who had a mean survival of 5 months. The survival of the four patients with stable disease was in large part due to the slowly progressive natural history of their disease.

[Surgery of complicated diverticular sigmoiditis. Report of 138 cases (author's transl)].

The authors analyse a series of 138 cases of complicated diverticular sigmoiditis treated surgically. They noted 50% of pyostercoral peritonitis, 32% of persigmoid abscesses, 13% of case of intestinal obstruction, and 5% of cases of fistual or hemorrhage. The overall mortality was 28% and depended mainly more on the surgical management adopted than on the type of complication. In this respect, simple colostomy with drainage of the septic focus had a mortality of 18%. The results suggest surgical operation in two stages, in the form of colonic resection, with, depending on each case, an anastomosis straight away with transverse colostomy or segmental colectomy with bitubular colostomy (Mikulicz procedure) or, in rarer cases, simple colostomy. The authors emphasise the interest of early surgery in sigmoiditis with complications and contrast the mortality of cold surgery which is now about 5% compared with 28% in emergency cases with complications.

[Relevance of lower oesophageal sphincter manometry in prae- and postoperative evaluation of gastrooesophageal reflux disease (author's transl)].

In 27 patients with refluxoesophagitis the resting LES-pressure was approximatly 80% lower than in 12 patients with asymptomatic hiatal hernia and 15 controls. All patients in the first group were operated on and 21 fundoplications (Nissen) and 6 anterior plications (Dor) were performed. The postiperativ LES-pressures were 70% higher than praeoperative and almost identical for both methods. The effective restitution of a functional sphincter with these operations could be demonstrated in this way.

[Surgical treatment of carcinoma of the pancreas (author's transl)].

147 cases of carcinoma of the pancreas are presented excluding cancer of the ampulla and endocrine tumours. Only 21 (14,9%) radical pancreatectomies could be performed with 3 deaths. 83 (56,4%) patients were treated by palliative procedures with 16,8% deaths. 21 (14,9%) patients had explorative laparotomy with a 27,2% mortality rate. The mean survival time is 11,5 months after duodeno-pancreatico-resection and 16 months after total pancreatectomy. The mean survival time in palliative surgery is 14,8 months. There was no survival at all after 5 years. Our surgical indication depends on local extension, but above all on lymphnode metastases: if there are more than 3 metastased nodes in the frozen section, radical surgery seems to be useless. On the other hand, total pancreatectomy gives better results for morbidity, operative mortality and survival rates than segmental resection.

Feline oncornavirus-associated cell membrane antigen: expression in transformed nonproducer mink cells.

The feline oncornavirus-associated cell membrane antigen (FOCMA) is a target for naturally occurring immunity that protects the cat against development of fibrosarcoma and leukemia. Feline sarcoma virus-transformed "nonproducer" mink cells express high levels of FOCMA, but not the major viral structural proteins. Transformation of the same cells by murine sarcoma virus, or infection with feline leukemia virus, which is nontransforming for epithelial or fibroblastic cells, did not induce FOCMA. Thus, FOCMA expression in mind lung cells is specifically associated with transformation by feline sarcoma virus.

Ultrahistochemical demonstration of cations in the guinea pig organ of Corti.

The distribution of cations in the guinea pig organ of CORTI was studied by means of the ultrahistochemical pyroantimonate method. In the outer hair cells precipitates were localized at the cell membrane, at stereocilia, in the subplasmalemmal cisterns, in the the nucleus, in mitochondria and within the ground plasm. Coarse precipitates at the cell membrane may consist mainly of cations other than calcium ions. The subplasmalemmal cisterns are suggested to sequester cations in the outer hair cells. The mechanism of the trapping reaction and factors injuring this histochemical method are discussed.

Biliary excretion of ampicillin: experimental and clinical study.

The biliary excretion of ampicillin was studied in various conditions. Using an isolated perfused rabbit liver preparation, 4.1% of the amount of ampicillin added to the circulating blood (10 mg) was recovered in the bile where the maximum antibiotic activity (mean: 104 +/- microng/ml) was 4 times higher than in the serum (mean: 24.1 +/- 3.4 microng/ml). In man, provided with T-tube drain, the oral or intravenous administration of 500 mg ampicillin resulted in a biliary excretion of 0.10% of the administered dose. The biliary concentrations thus obtained equalled or exceeded for several hours the minimal inhibitory concentration of the organisms included in the antibacterila spectrum of ampicillin justifying thus the use of this antibiotic in the treatment of biliary tract infections. Per-operatory assays showed that in patients with gallbladder disease or with cystic duct obstruction, ampicillin levels in the gallbladder bile were markedly lower than in the common duct bile.
