[Gastroesophageal reflux after the surgical treatment of esophageal atresia].

The experience of the authors concerns 100 neonates treated for esophageal atresia between 1968 and 1980. A retrospective study of the sequelae observed in 73 survivors (age 3 to 15 years) found 26 cases of gastroesophageal reflux. This study demonstrates a high incidence of reflux in children who initially presented with a fistula and long gap esophageal atresia. It also shows that chronic respiratory disorders are more frequent in these patients and that esophagitis is the principal factor leading to stricture of the anastomosis stubborn to dilatation. In 14 patients, the severity of the clinical and endoscopic symptoms justified a surgical antireflux procedure, the only guarantee of clinical and radiologic recovery, with long term confirmation in 9 patients.

[Primary phlebitis of the arm].

Primary phlebitis or sprain phlebitis in the upper member are caused by a compressive mechanism in the thoraco-brachial passage. Thrombosis most often originates in the area of the cost-clavicular hinge, and spreads towards the extremity of the member. In this series, 19 cases were treated: 8 recent cases of phlebitis, in which results were all too often imperfect (5 failures), 11 post-phlebitic syndromes, in which there were 9 good results. The surgical procedure which we therefore recommend aims at the decompression of the collateral circulation flowing around the thrombosed area: this should be enough to improve results in a large proportion of cases.

[DARC/PELCO structure activity relationships of aldosterone agonists and antagonists].

To detect the sites essential for binding affinity, we use the DARC/PELCO topological correlation search method and apply it to 52 17-spirolactone or 17-hydroxymethylcarbonyl steroids, aldosterone agonists or antagonists. The optimal model explains 98% of the total variance with a precision close to experimental. The main favourable contributions come from direct substitutions on rings C and D of the steroidal skeleton, the most unfavourable from those on rings A and B. The secondary structural modifications - modifications of existing substituents or second substitutions on the focus - have a slight deactivating influence, except the simultaneous existence of two contiguous double bonds on the 11 beta chain. Structures predicted to have a high affinity are built up by combining the most favourable structural elements: 9 alpha-F, 11 beta-allenyl, delta 11, 12 and 15 beta-, 16 beta-CH2. Some are predicted to have an affinity 100- to 1000-fold higher than aldosterone.
