The authors report a case of posterior inveterate sternoclavicular dislocation, which came to their observation after tangential resection of the clavicle. They discuss the surgical technique of reduction and stabilization and clinical results. CT scan was the method most-suited to evaluate dislocation of the clavicle.
The authors describe the surgical method used and present the preliminary results obtained in 29 patients affected with recurrent anterior dislocation of the shoulder treated by Neer capsuloplasty and Statak screws. The long-term results were evaluated based on the Rowe system: the score was excellent in 18 cases, good in 2, fair in 4, and poor in 5. All of the patients resumed work activity, with no restrictions, within 4 months of the onset of functional rehabilitation. The use of Statak screws allows us to simplify surgery, while guaranteeing repair of the Bankart lesion, when present. The method reported permits correction of instability, without excessive reduction of joint movement.
Decreased insulin sensitivity patients, although normal-weight, are treated with a "prudent" hypocaloric diet and exercise, in order to reduce insulin resistance. Metabolic (glycolipidic) alterations can be easily diagnosed by combination of clinic and laboratory tests. A diet model aimed at controlling insulin levels and inducing a light weight loss is proposed.
Atherosclerosis is the main cause of death in western industrialized Countries. Different studies have shown the fundamental role of cholesterol-containing lipoproteins in the atherogenic process and the beneficial impact of lowering plasma cholesterol levels. To achieve reduction of plasma cholesterol, in the general population as well as in high-risk subjects, dietetic measures should first aim at controlling such a risk factor. Guidelines to be followed in prescribing a diet for hypercholesterolemic patients are presented.

The authors describe the clinical and radiographic findings of posterior dislocation of the shoulder by examining 19 cases observed at the Rizzoli Orthopaedic Institute between 1981 and 1990; of the total number of cases, 16 were acute and 3 inveterate, equal to 1.7%. After analyzing the pathogenesis of the lesion, which was due to direct trauma, indirect trauma, or which occurred during an episode of convulsions, the authors call attention to the rareness of the lesion, and consequently the little amount of knowledge of the clinical and radiographic elements characterizing it. Three varieties are distinguished: subacromial (or retroglenoid), subglenoid, and subspinous. Finally, the results obtained with non-surgical treatment, which always allows for recovery of the joint relationships and complete functional recovery, are reported.
Transposition of the coracoid process in the preglenoid site according to the Latarjet method in 15 cases of anterior recurrent dislocation of shoulder is reported. Indications, surgical stages, and results are discussed with emphasis on the simplicity of surgery followed by nearly complete functional recovery and rapid resumption of professional and sports activity.
