The effects of direct versus indirect hypnotic suggestion on pain in a cold pressor task.

Past studies have investigated the usefulness of hypnosis in pain reduction. Although hypnotic analgesia has been found to be effective, it is generally only those subjects who are highly susceptible to hypnosis who benefit. Some experimenters have found that even low-susceptible subjects can use hypnotic analgesia, if the hypnotic induction uses indirect rather than direct hypnotic suggestions. In the present study, high- and low-susceptible subjects were tested for analgesia using either direct or indirect hypnotic suggestion on pain in a cold pressor task. Findings suggest that high susceptibles experience greater pain reduction than do low susceptibles. However, no significant differences were found between the pain reduction in the direct versus the indirect hypnotic suggestion groups. Possible explanations for this lack of differences are discussed.

[HIV prevalence and HIV incidence in blood donors in Baden-Württemberg].

Since several years the frequency of HIV infections among screened blood donors in Baden-Württemberg (a state in southwest Germany) shows no remarkable differences between the German Red Cross donors and the donors of other blood banks. The authors advocate the hypothesis that different HIV rates among donors depend on their social environment. The number of HIV infections expected increases with the size of the donors' places of residence. The donor populations of different blood banks of Baden-Württemberg seem to be equal. Within the framework of epidemiological surveillance, the results of HIV screening of blood donors should be thoroughly analysed.

[Load stable, minimal fibula osteosynthesis in Weber type B malleolar fracture by hemicerclage].

Simple lateral malleolar fractures type B according to Weber can be fixed with a hemicerclage alone. Technique and results at sixteen own cases are shown. At this indication the hemicerclage is regarded as a good alternative to common methods of osteosynthesis. Advantages are simplicity, minimal damage of soft tissue and bone, high interfragmentary compression and allowed full weight bearing.

[Stomach cancer: 10 years experiences with surgical treatment and possibilities for improving the prognosis].

We retrospectively evaluated data on 111 consecutive patients operated between 1976 and 1985 for gastric carcinoma. Patients were staged according to the TNM classification (stage I - 17%, II - 15%, III - 30%, IV - 37%). 66 patients (59.5%) underwent radical resection. Most of curative resections were subtotal distal gastrectomies. Extended lymphadenectomy was not performed. The operative mortality rate was 7.4% in case of curative resections, and 9.6% overall. 20 out of 66 radically operated patients (30%) were treated with adjuvant cytotoxic chemotherapy, while 9 out of 45 non radically operated patients were treated with palliative cytotoxic chemotherapy. The median follow-up time was 36 months (range: 1-166 months) for patients operated for cure and 25 months for the whole population. The estimated adjusted 5-year survival rate was 41.3% after operation in a curative intent, 82.9% for stage I, 62.8% for stage II, 17.8% for stage III, 3.3% for stage IV. The duration of symptoms before diagnosis was longer than 6 months in 18 patients (16.2%) and longer than 10 months in 9 patients (8.1%). Patients who were symptomatic since more than 10 months were on the average about 10 years younger than the rest of the population. The median duration of symptoms was 3.2 months overall, but reached 7.1 months in patients less than 40 years old. Patients with an early gastric cancer (11/111: 9.9%) or stage I disease (19/111: 17.1%) had always symptoms since less than 5 months. The proportion of early gastric cancer increased from 6.3% (3/48) to 12.7% (8/63) from 1976-1980 to 1981-1985, simultaneously to the diffusion of the upper gastrointestinal endoscopy.(ABSTRACT TRUNCATED AT 250 WORDS)

Kinetics of anti-HBs after hepatitis B vaccination: a comparison of two recombinant and one plasma-derived vaccines.

Geometric mean titers were determined for three groups of medical students who had been vaccinated against hepatitis B with two different yeast-derived recombinant vaccines and one plasma-derived vaccine. The antibody kinetics for the three groups were similar over a period of 4 years. A formula for prediction of titers from the post-booster anti-HBs concentration is provided.

Positron emission tomography imaging-directed stereotactic neurosurgery.

The diagnostic yield of stereotactic biopsies depends upon safely sampling the most representative regions of masses. Potentially, localization information presently available from CT and MR scans might be improved by considering regional levels of cerebral metabolism by PET. In 10 selected stereotactic biopsies of intracranial mass lesions, PET scans were obtained using the Siemens CTI system; the Kelly-Goerss Compass system was used as the stereotactic development platform. PET-[18F]-fluorodeoxyglucose (FDG) images provided clinically significant information by appropriately directing biopsy efforts when CT and MRI was inconclusive or misleading. PET-FDG images can be successfully incorporated into routine image-directed stereotactic surgery.

Persistence of antibodies after immunization with a recombinant yeast-derived hepatitis B vaccine following two different schedules.

Four years after vaccination of healthy adults with either a recombinant yeast-derived (YDV) or plasma-derived (PDV) hepatitis B vaccine, the persistence of antibodies to hepatitis B surface antigen (anti-HBs) was examined. Subjects received three 20 micrograms doses in the deltoid region according to either a 0, 1, 6 month or 0, 1, 2 month vaccination schedule, with a 20 micrograms booster dose of YDV administered at month 12 for subjects on the second schedule. The recombinant vaccine was found to be immunologically comparable to the PDV, with anti-HBs persisting equally in both groups.

Type I diabetes in NOD mice is not associated with insulin-specific, autoreactive T cells.

In the present study the hypothesis was tested that T cells specific for autologous insulin may be involved in beta cell destruction. Lymphoid cell populations from non-immunized, non-obese diabetic (NOD) mice were investigated for spontaneous T-cell reactivity in vitro to rat insulin (identical to mouse insulin) and to porcine insulin (identical to mouse insulin in the immunogenic part of the A chain loop). No significant T-cell proliferation was detected. In vivo priming of non-diabetic NOD mice with rat insulin in CFA, or of diabetic or non-diabetic NOD mice with porcine insulin failed to elicit insulin-specific T-cell responses upon restimulation in vitro. Lymph node cells from NOD mice primed with porcine insulin and treated with anti-Lyt 2 antibodies and C also failed to show insulin-specific reactivity, indicating that suppression by Lyt 2 cells is not involved in the non-responsiveness observed. In addition to porcine and rat insulin, NOD mice were also non-responders to bovine insulin; however, they responded to equine and ovine insulin and to the oxidized chain B of bovine insulin, the latter showing no cross-stimulation in vitro to any of the intact insulin variants. In conclusion, this study indicates that autologous insulin does not serve as autoantigen in the autoimmune destruction of beta cells.

[Recidive inguinal hernia with subcutaneous displaced spermatic cord. Operative procedure and results].

Following Bassini-Kirschner repair the rate of recidives is higher than after currently applied procedures. From the first of January 1971 to the first of November 1986 54 male patients were operated upon a recidive of inguinal hernia with the spermatic cord situated in the subcuticular tissue. 3 of them had a repair of both sides. 42 operations have been done for the first, 13 for the second an 2 for the third recidive. Beginning on the 10. 9. 1974 hernial repair was progressively accomplished by reconstructing a short oblique channel. In appropriate cases with a lateral recidive and a well healed plasty in the medial area this region remained untouched. This type of repair has been done in 12 cases. 38 patients (40 cases) were controlled personally 13 to 166 months postoperatively (mean 77.7 months), 8 of them with a short oblique channel. 12 patients were dead, 2 had returned to foreign countries. 2 patients refused control. There were 3/40 recidives: 2 recidives occurred out of 6 cases with Bassini-Kirschner procedure again, 1 recidive out of 22 cases with a Bassini repair. 2 patients with simple closure of a medially situated leakage and one patient each with McVay repair or semicastration were doing well, as well as the 8 patients with a short oblique channel.(ABSTRACT TRUNCATED AT 250 WORDS)

Differential diagnosis of benign epithelial proliferations and carcinomas of the breast using antibodies to cytokeratins.

The immunohistochemical reactivity on frozen sections of diverse benign and malignant epithelial proliferations of human breast tissue from 156 patients was examined using antibodies to different cytokeratins. Antibodies recognizing cytokeratins 18 and 19 reacted with luminal epithelial cells but not with myoepithelial cells of normal mammary gland, cystic disease, adenosis, papilloma, and fibroadenoma or with a subpopulation of proliferating cells in sclerosing adenosis and epitheliosis. These antibodies reacted with the tumor cells of all in situ and invasive carcinomas. KA1 antibody, which by one- and two-dimensional gel electrophoresis and immunoblotting was shown to bind preferentially to cytokeratin 14 in a complex with cytokeratin 5, reacted with the nonproliferating myoepithelium of normal gland, cystic disease, adenosis, papilloma, fibroadenoma, and in situ carcinoma; it also reacted with a subpopulation of proliferating cells in sclerosing adenosis and epitheliosis (papillomatosis) but was negative with the tumor cells of all preinvasive and most invasive carcinomas. In adenotic and epitheliotic proliferations, groups of cells were identified that reacted strongly with KA1 antibody in addition to antibodies to cytokeratins 18 and 19. The data are discussed with respect to epithelial cell heterogeneity in the breast. We show that by using such antibodies, benign epithelial proliferations are clearly distinguished from carcinomas.

Reactogenicity and immunogenicity of a new recombinant yeast-derived hepatitis B vaccine.

Under randomized double-blind conditions, 220 medical students were vaccinated with either a 2.5, 5, 10, or 20 micrograms dose of a recombinant yeast-derived hepatitis B or a 20 micrograms dose of a plasma-derived vaccine. Vaccines were administered at months 0, 1, and 2. After 11 months, all vaccinees received a 20 micrograms booster dose of the recombinant vaccine. There were no significant differences in adverse reactions between the study groups. Induction of IgE antibodies to yeast was not observed. One month after the third vaccination, seroconversion rates reached 100% in all vaccines. Mean anti-HBs levels varied between 150 and 1470 IU/l after 3 vaccinations, with the lowest dose resulting in the lowest titres. Following the booster vaccination, dose-dependent effects were no longer observed. Anti-HBs concentrations were reanalyzed 29 and 36 months after the study had started. The data indicate that the recombinant hepatitis B vaccine is safe and immunogenic for use in man and comparable to the plasma-derived vaccine in terms of safety and efficacy.

[Uterine bleeding in the elderly after long-term application of oestrogen-containing hair lotion].

Long-term use of a widely available oestrogen-containing hair lotion caused specific systemic oestrogen effects with proliferation of the endometrium in two patients aged 66 and 82 years. Uterine bleeding was the cardinal sign. These findings confirm the possible percutaneous absorption of externally applied oestrogen-containing substances. They probably also explain an oestrogen effect on uterine epithelium, which is not rarely seen during cancer-preventive gynaecological examination and has no other explanation.

[Trigemino-facial reflex as a diagnostic aid in Recklinghausen's neurofibromatosis].

The blink reflex evoked by unilateral electrical stimulation was studied in 7 patients with Neurofibromatosis and 50 normal persons. Studing data of the early R1 reflex, there was no difference between normal persons and patients with von Recklinhausen disease. A slightly, but significantly increased reflex latency of the direct R2 was recognized in the majority of the patients with Neurofibromatosis.

[The frequency of irregular erythrocyte antibodies (author's transl)].

Investigations to demonstrate irregular erythrocyte antibodies showed an increase in the number of sensitizations due to blood transfusions as well as an increase of the antibodies anti-E and anti-Kell. A reorganization of the blood transfusion system should be considered. It seems necessary to detect patients with irregular antibodies as early as possible. This should be done by follow-up investigations after blood transfusion and by antibody screening tests before transfusion. Blood groups estimated previously should not be relied upon during subsequent hospital admissions.
