[Causality relationship of drug adverse reactions: experience in the use of HIV-protease inhibitors].

PURPOSE: To establish the exact cause and effect relationship between protease inhibitors (PIs) and adverse events. MATERIALS AND METHOD: Prospective, cohort, multicenter study on HIV-positive patients who are beginning treatment with a PI. Causal relationships are evaluated using the RUCAM algorithm. RESULTS: Since the beginning of the study 1207 patients have been enrolled. Average time of observation is 10.7 months. To date, 784 adverse events have been observed, distributed as follows: excluded 3.8%, improbable 18.5%, possible 41.3%, probable 30.1%, and highly probable 6.3%. Saquinavir shows a statistically significant difference in the rate of non-correlated events with respect to other groups. CONCLUSIONS: Over 20% of adverse events during PI treatment are shown to be non-correlated to these drugs. Saquinavir shows the highest rate of non-correlated events.

[Results of an interlaboratory fibre count comparison program concerning the SEM technique].

BACKGROUND: The necessity of quality control for laboratories carrying out airborne asbestos fibre monitoring is recognized in Italian law concerning the worker and the environmental protection against asbestos. The scanning electron microscopy technique is prescribed to control the airborne fibre concentration after asbestos removal from buildings. OBJECTIVES: In 2002 the Italian laboratories carrying out SEM analysis on asbestos were requested by the Istituto Superiore di Sanità to take part in a fibre count comparison program. Thirty-nine agreed to participate. METHODS: Fifteen samples were exchanged among the laboratories participating in the program. Seven samples consisted of polycarbonate membranes and eight of cellulose ester membranes. The sample loads did not exceed 50 fibres/mm2. The fibre counting results were evaluated with reference to the mean value of each sample calculated from the results provided by all the participants. RESULTS: Only 19% of the fibre count results were considered "insufficient", and the results falling within the 95% confidence interval were the 65% of the total. University and public laboratories obtained on the whole the best results.

[Value and limits of CT in the preoperative assessment of primary malignant tumors of bone. Observations in 138 cases].

To estimate the capabilities and limitations of CT in the preoperative evaluation of malignant primary bone neoplasms, 138 patients were examined with high resolution CT and subsequently underwent surgery. In all cases a pathologic examination of the surgical specimen was performed, whose results were compared to CT findings. CT appears to yield important diagnostic contribution to preoperative staging. As a matter of fact, CT shows the tumor extension and its relationship to the adjacent anatomical structures, particularly, it shows the involvement of the muscular and vascular bundles, and that of the articular cavity and medullary canal. These data are of primary importance for planning therapy. The commonest CT limitations are: overestimation of the tumor extension, false positives and false negatives when searching "skip" metastases, and, sometimes, inaccurate definition of the relationship between tumor and vascular bundles and/or articular cavities. These limitations may be overcome by using, when necessary, other techniques--i.e. angiography, bone scan and, above all, MR imaging.

[Tooth extraction wound healing after administration of vitamin B6 (pyridoxine). Histological study in rats].

Male rats with 250 grs of weight were injected daily with 0.1 ml of B6 vitamin by peritoneal (way paths). The animal were sacrificed at three, seven, 10, 15 and 21 day after the upper incisor extraction. Its hemimaxila were retired and fixed in 10% formalin and after had been embedded in paraffin sections were cut with 6 micrometers pick. The analysis of the sections stained by hematoxylin and eosin when compared with the controls showed that: 1) the blood clot and fibrin net are substituted more rapidly by the granulation tissue; 2) granulation tissue is more plentiful, early and mature; 3) the bone tissue is more plentiful and mature.

[The determination of asbestos fiber concentration by scanning electron microscopy: a comparison of the results obtained by different laboratories].

Recently the use of scanning electron microscopy to evaluate asbestos fibre concentration in the environment has been prescribed by national law. So far data on the correlatability of results provided by different laboratories are not available. In the present study 55% of results obtained by the participants was in the range of +/- 30% of the mean and 90% in the range of +/- 50%. One of the main factors causing variability of results turned out to be related to the behaviour of technicians.
