[Total thoracic compliance. Determination by calculator and comparison with values obtained with the syringe method (author's transl)].

The performance of a calculator was studied on 19 patients requiring therapy with mechanical ventilation. Values of total compliance displayed by this calculator device were compared with values obtained with the syringe method. The determination of compliance were made at a series of values for Tidal volume (T. V.): 3, 4, 5, 6, 8, 10, 12, 15, 18, 20, 23 and 26 ml.kg-1. Measurements were performed while patients were paralysed by pancuronium bromide. In each patient the calculator device displayed values that correlate closely with the value obtained by the syringe method. Compliance increased between 3 and 26 ml.kg-1 of T. D. Between 4 and 15 ml.kg-1 of TD the changes of compliance were nearly identical whatever the method of measurement used. The calculator can be used for reliable monitoring or recordings of total compliance.

Deficit of suppressor T cells in active multiple sclerosis.

Suppressor T cells in forty-seven patients with multiple sclerosis (MS) were studied by indirect immunofluorescence by means of monoclonal antibodies directed at T-cell subsets. Suppressor cell numbers were depressed in patients with acute exacerbation of MS and in patients with continuous progressive deterioration. In one case the T-cell anomaly was observed one week before the onset of the acute phase; it had been absent 4 months before exacerbation.

The effect of low flow oxygen on sleep-disordered breathing and oxygen desaturation. A study of patients with chronic obstructive lung disease.

Oxygen desaturation occurs during sleep in many patients with chronic obstructive lung disease (COLD) and is often caused by sleep-disordered breathing (SDB). Nocturnal oxygen therapy should improve nighttime hypoxemia, but might also worsen SDB. Using standard polysomnographic techniques, we evaluated the frequency and duration of oxygen desaturation and SDB during sleep in 11 patients with stable COLD. During half of the night the patients breathed air through a nasal cannula and during the other half of the night they breathed oxygen at 2 liters per minute. Five patients had arterial lines inserted for determination of arterial blood gas levels during periods of SDB or desaturation. The ten men and one woman slept 70 minutes (52 percent of time in bed) while on air and 111 minutes (80 percent of time in bed) while on oxygen (p < 0.001). Oxygen therapy reduced the number of episodes of desaturation per hour and the time spent in desaturation. However, there was no difference between air and oxygen in episodes of SDB per hour, the duration of episodes of SDB, baseline sleeping PaCO2 or PaCO2 during episodes of desaturation or SDB. Therefore, in most patients with stable COLD, administration of oxygen at 2 liters per minute improves oxygenation, prolongs sleep, but does not adversely affect SDB.
