[Identification of Candida albicans from various materials].

We examined 300 stipes of leavens coming from various material. Among these we identified 264 brances belonging to Candida king, utilizing the test for pseudomicelio production. Combininb with this test that of clamidospore production, the auxonogramma, culture media, we arrived to the classification of the species: C. Albicans 71%, C. Tropicalis 15.8%; C. Pseudotropicalis 0.6%; C. Krusei 0.3% C. Parapsilosis 0.3%. An analysis of the experiments utilized for the classification of the kind, proved that the test of germination is the most simple, rapid and safe method of ascrive the branches belonging to Candida kind to Albicans species.

Tumour response to heat and radiation: prognostic variables in the treatment of neck node metastases from head and neck cancer.

A total of 38 patients with 81 multiple neck node metastases from squamous cell carcinoma of head and neck were treated with radiotherapy alone or with radiotherapy plus hyperthermia. Irradiation was delivered following a three fractions per day schedule of 2 + 1.5 + 1.5 Gy/day, with 4 h intervals between fractions, up to a total dose of 60 Gy. Heat was applied by means of a 500 MHz apparatus. Temperature data were converted to equivalent minutes at 42.5 degrees (Eq 42.5). Initial complete response rates and local control distribution were compared for subgroups of tumour volume and thermal dose. The data indicated that the volume effect was less pronounced in the combined modality than in the radiation alone arm, suggesting that the addition of heat was more damaging to the large than to the small lesions. A striking thermal dose-response relationship was shown, although complete response rates increased only after a certain thermal dose was accumulated, clearly indicating the presence of a threshold dose.

Combined radiation and hyperthermia: effects of the number of heat fractions and their interval on normal and tumour tissues.

The effect of one or more heat treatments at 43 degrees C combined with conventional multifraction irradiation was studied in murine normal and tumoral tissues. The endpoint of this study was the TCD50 of the C3H mammary carcinoma, inoculated into the foot. For normal tissue the reaction of the skin of the foot was assessed according to a graded scoring system. Twenty X-ray fractions delivered in 20 or 26 days were combined with one, four or eight fractions of quasi-simultaneous or sequential hyperthermia (i.e. heat applied 4 h after irradiation). Tumour dose-response curves show a decrease in TCD50 and in increase in TER linked to the increasing number of heat sessions. A qualitatively similar effect of the number of sessions was observed for the different protocols (quasi-simultaneous and sequential) and for the two overall times. No significant difference was measured between one and four HT sessions plus 20 fx of RT in the normal tissue damage, while a slight difference was observed between one, four and eight HT sessions delivered with 20 fractions of RT in 26 days.

"Eight drugs in one day" chemotherapy in a nonfamilial bilateral retinoblastoma with recurrent cerebrospinal fluid metastases.

A case of bilateral retinoblastoma with recurrent cerebrospinal fluid (CSF) metastases is presented. The patient underwent left eye enucleation and received external beam radiotherapy to the right eye. A sequential combination chemotherapy, including cyclophosphamide, vincristine, carboplatin, and etoposide with intrathecal drug administration, was then adopted. During treatment a relapse within the central nervous system (CNS) occurred. The "8-in-1-day" chemotherapy, used in our department in medulloblastoma patients with CSF involvement was then considered. The patient received 8 total courses, each with intrathecal drug administration. No toxicity was experienced. The patient is alive and free from CSF metastases after a period of 48 months and has been in continuous complete remission (CCR) for 41 months. The authors conclude that the use of the "8-in-1" regimen is feasible and can be used in retinoblastoma patients with recurrent CSF metastases.

An analysis of breast cancer coverage in selected women's magazines, 1987-1995.

PURPOSE: Women's magazines are a significant source of health information for many women, but there is some concern that the media may misrepresent a woman's risk of breast cancer. This review analyzes breast cancer articles in selected women's magazines to determine if the information presented is accurate and balanced. SEARCH METHODS: For the years 1987 to 1995, the quantity and content of breast cancer articles were examined in four popular women's magazines and three magazines with a large African-American audience. Fifty-nine lead factual breast cancer articles were analyzed for the information presented on prevention measures, risk factors, incidence/mortality statistics, and lifetime risk. The age at diagnosis for women featured in these articles was also determined. SUMMARY OF IMPORTANT FINDINGS: Breast cancer was the topic of 34.9% of the 585 cancer articles published in these seven magazines. Mammography screening guidelines were recommended in 68% of articles that discussed prevention; 66% presented the American Cancer Society guidelines. Risk factors for breast cancer were reported as age greater than 50 in 41% and family history in 78% of the articles that discussed risk factors. Twenty articles used the lifetime risk statistic of developing breast cancer; six explained what this statistic means. The average age of diagnosis for women featured in the articles was 40.5. MAJOR CONCLUSIONS: The proportion of breast cancer articles to all cancer articles in these magazines (34.9%) was similar to breast cancer incidence (32.2%) but was higher than its contribution to either female cancer mortality (17.2%) or overall female mortality (4.0%). Magazines that targeted the African-American audience had fewer breast cancer articles than the other four magazines. Risk factors for breast cancer were not discussed in proportion to their impact on risk. The popular lifetime risk statistic was not explained in the majority of cases where it was presented. The average age of diagnosis for the women featured in these articles was far below the median age of diagnosis of 65. Thus, magazine reports may be contributing to women's misunderstanding of their true breast cancer risk.

Smoking, blood pressure and serum albumin are major determinants of carotid atherosclerosis in dialysis patients. CREED Investigators. Cardiovascular Risk Extended Evaluation in Dialysis patients.

AIM: To investigate the relationship between carotid atherosclerosis and some major cardiovascular risk factors in uremic patients on chronic dialysis. METHODS: A cross-sectional study was carried out in 119 unselected dialysis patients (89 on hemodialysis and 30 on chronic ambulatory peritoneal dialysis, CAPD). Fasting blood sampling for serum lipids, albumin, hemoglobin, and echo-colour-Doppler evaluation of common carotid arteries were performed in all patients (during the non-dialysis day in hemodialysis patients). In hemodialysis patients BP was measured before and after dialysis; in CAPD patients home BP values were recorded during the month before the study day. RESULTS: Ninety-five patients had at least one plaque and 57 had at least four plaques. Thirty-eight had mild and eleven severe carotid stenosis. In multiple regression models, the mean internal diameter of carotid arteries was explained (R=0.52, P=0.0001) by systolic pressure (r=0.39), serum cholesterol (r=-0.28), age (r=0.27) and smoking (r=0.24) while the degree of carotid stenosis was predicted (R=0.39, P=0.0001) by age (r=0.36) and smoking (r=0.25). The number of atherosclerotic plaques was explained (R=0.51, P=0.0001) by age (r=0.36), smoking (r=0.25) and pulse pressure (r=0.20), serum albumin just failing to reach statistical significance (P = 0.06). However, serum albumin was a significant and independent predictor of the number of atherosclerotic plaques (r=-0.26) in hemodialysis patients (n=89). Sex, diabetes, Kt/V, duration of dialysis treatment, hemoglobin, serum calcium and phosphate did not add any predictive power to the models. CONCLUSIONS: In dialysis patients arterial pressure and smoking are associated with carotid atherosclerosis. Serum albumin appears to serve as an independent predictor of carotid atherosclerosis.

[Xerostomia and Xerophthalmia in haemodialysis patients].

BACKGROUND: A reduction in salivary and lachrymal secretion has been described in many pathologies; however, such alterations have not been described in patients with renal failure. This study was designed to estimate the frequency of alterations in salivary and lachrymal secretion in haemodialysed patients. PATIENTS ABD METHODS: We studied 63 haemodialysed patients and 23 healthy control subjects. In all of them we tested salivary secretion (Saxon test), lachrymal secretion (Shirmer test) as well as the presence of symptoms of xerostomia and xerophthalmia. In a subgroup of patients We investigated any evidence of ocular lesions and tissue damage of salivary glands (histopathology). We also tested the correlation between salivary and lachrymal secretion and autonomic nervous system function. Furthermore, we also studied the association between xerostomia and xerophthalmia and serum auto antibodies (anti nuclear, anti-Ro (SS-A), anti-La (SS-B)) and anti HCV antibodies. RESULTS: On average salivary and the lachrymal secretion was markedly reduced in uraemic patients compared with healthy controls. We found the alterations in salivary glands function to be strongly related to salivary glands fibrosis and atrophy and independent of amyloid accumulation. On the other hand, we observed that xerostomia and xerophthalmia were unrelated to autonomic dysfunction as well as to HCV infection and circulating auto antibodies. Moreover, xerophthalmia was frequently associated with evidence of corneal damage. CONCLUSIONS: Reduced salivary and lachrymal secretion is frequent in uraemic patients. Such alterations are often asymptomatic and could be an expression of the accelerated age-dependent decline in glandular function and the attendant fibrosis and atrophy.
