Transsphenoidal encephalocele associated with agenesis of corpus callosum: value of metrizamide computed cisternography.

Transsphenoidal encephaloceles are rare congenital malformations that are classified among the medial dysraphias and that are therefore associated with an agenesis of the corpus callosum. Clinically, the diagnosis is difficult, but hypertelorism and weakening eyesight should be of suggestive value. Diagnosis is basically radiological. In the past, it was founded on roentgenography, tomography of the base of the skull, pneumoencephalography, and angiography when necessary. Today, metrizamide computed cisternography using axial transverse and coronal sections, supplemented by sagittal reconstructions, allows for a complete examination of bony, meningeal, and cerebral abnormalities.

Metrizamide demonstration of the subarachnoid space surrounding the optic nerves.

Intrathecal instillation of metrizamide has provided a useful tool for the in vivo evaluation of smaller structures adjacent to or within the subarachnoid cisterns. The authors report a case in which metrizamide filled the thin subarachnoid space surrounding the optic nerve in its course through the orbit. The finding appears to be infrequent, however, since it was not observed in any instance in a review of 80 metrizamide computed tomographic cisternograms in patients with normal intracranial pressure.

Lumbar phlebography in the diagnosis of disc herniations.

The authors report their experience with 240 cases with lumbar phlebography in the diagnosis of lumbar disc herniations. The normal radiological anatomy and the radiological signs of disc herniations are described. Indications for phlebography are given, and the reliability of this test is compared with that of myelography performed with water-soluble agents.

Computed tomography in olfactory neuroblastoma: one case of esthesioneuroepithelioma and four cases of esthesioneuroblastoma.

Five cases of histologically confirmed olfactory neuroblastoma (esthesioneuroblastomas) have been studied by computed tomography (CT). Both the clinical symptoms (in particular, unilateral nasal obstruction and recurring epistaxis) and radiological findings (opacity of the paranasal sinuses) are nonspecific. Computed tomography shows a contrast enhancing mass-lesion and is more useful than conventional tomography in estimating its extension and the associated bone destruction, secondary reaction of sinusitis, and tumoral calcification. In the two cases in which the tumor was associated with exophthalmos, CT demonstrated that the muscular cone acts as a barrier preventing tumoral invasion. In cases wtih intracranial involvement, it appears that the attenuation values can be useful for differentiating between invasion by contiguity (two cases) versus metastasis (one case). Computed tomography is valuable in the follow-up and during and after radiotherapy of the tumor.

[A symptomatic parathyroid adenoma. Value of parathyroid hormone determination through selective catheterization of the thyroid veins].

A parathyroid adenoma is reported in a girl aged 12 years in whom hypercalcaemia was discovered by chance. Investigation of calcium metabolism suggested the diagnosis of hyperparathyroidism and studies of the urinary cyclic AMP and determination of the plasma parathyroid hormone concentration further added to the evidence. The diagnosis of parathyroid adenoma was made after determination of the parathyroid hormone concentration at various sights during selective catheterization of the tyroid veins. This was confirmed at surgery. In this patient the place of catheterization of the inferior thyroid veins in the early diagnosis of primary hyperparathyroidism is discussed.

Congenital spondylolysis of cervical vertebrae with spondylolisthesis anf frontal narrowing of the spinal canal. Report of two cases.

Two cases of cervical spondylolisthesis, due to a congenital defect in the pars interarticularis of C6, associated with spina bifida and elongation of the contralateral pars interarticularis, are reported. Tomography showed an abnormal direction of the articular facets of C6 and demonstrated a narrow frontal diameter of the cervical spinal canal at the level of the malformation. Gas myelography showed a slight compression of the cord. Unlike previously reported cases the patients had neurological findings which, in one case at least, were certainly referable to the spinal cord.

[Embolization by femoral catheterization of tumors supplied by the external carotid artery. 40 cases].

Experiences from therapeutic embolization by way of superselective catheterization of the external carotid artery, in 40 cases of cranio-cerebral, maxillo-facial and cervical tumours are reported. Using Spongel, embolization offered definite advantages compared to simple arterial ligation: it is easily performed as a supplementary procedure to diagnostic superselective angiography; a meticulous technique will provide superselective arterial occlusion with obstruction of vessels down to the precapillary size (diameter 150 to 200 mu). Embolization is advocated as a preoperative measure in meningeomas (particularly at the skull base), naso-pharyngeal angiofibromas, intracranial and cervical chemodectomas, in which cases profound bleeding during surgery may be expected and as a palliative measure in cases where surgery and radiation therapy fail to control recurrence of a tumour.

[The surgical approach to spontaneous intracerebral hematomas (author's transl)].

Following a recent statistical survey of 56 cases over a period of 3 years the authors considered the choice between the frontal or the parieto-temporal approaches to intracerebral haematomas. From their anatomical studies they considered that the frontal route is the better for approach to haematomas of the external capsule, whereas the parieto-temporal approach is preferred for haematomas of the internal capsule.
