[Intramural crisis intervention: treatment concept and everyday therapy].

The socio-psychiatric crisis intervention ward at the University Hospital Berne is held up as an example for a concept of crisis intervention for in-patients. After representation of the clinical setting and a diagnostic survey, we enter into the rules for crisis-interventions-technique of our ward. In addition to the rules, a psychodynamic conception serves deeper understanding of psycho-social crises. Finally, we describe the translation of the theoretical conception into therapeutic practice.

[Myth and delusion in the Melanesian cargo cult].

Native myths are rekindled in the Melanesian cargo cult, the process in many cases being activated by some prophecy made by a psychotic cult leader. Indeed, the cargo cult brings about a feeling of solidarity for the natives involved going beyond the strict tribal boundaries. The culturally determined content of the cult leader's psychosis primary focuses on a grandiose sense of mission of a religious and political kind.

[Risks in postoperative pain therapy using a peridural catheter in a normal general ward].

In general surgical wards postoperative epidural analgesia was performed in 286 patients consecutively. Epidural catheters were kept in place 8.3 days in the average with a minimum of 24 h and a maximum of 45 d. Bupivacain was applied as a 0.25% solution with an average daily dose of 75-250 mg and single doses of 12.5-50.0 mg. An uncomplicated course was noticed in 250 cases, 19 cases showed haemodynamic, 7 cases neurologic and 10 cases technical complications. Severe decrease of systolic blood pressure more than 30% of the control value was due to postoperative haemorrhage, to anaemia with Hgb-concentration lower than 11 g/dl or to untreated chronical hypertension. On the basis of these observations a list of conditions has been drawn up which should be adhered to when epidural catheters are used for postoperative pain relief in normal wards.

[Vital disorders of the mouth (glossodynia) in depressive involutional psychoses].

Eight cases of glossodynia arising in the context of an endogenous depression are reported and the case histories of four of them are presented. Somatic features, in particular the sensation of having a burned tongue, were by far the most prominent and tended to relegate the depressive symptoms to the background. The clinical picture can usefully be classified as the coenesthetic form of a depressive psychosis of late life. A good treatment response can be achieved with antidepressants, possibly in combination with neuroleptics.

[A remarkable case of symbiotic psychosis].

The symbiontic psychosis of an elderly married couple is described, which was based on delusions of jealousy held by the inducing, tardive schizophrenic wife. Acceptance of the delusion by the egoweak, psycho-organically impaired husband had far-reaching consequences (surgical castration, treatment with anti-androgens). Special attention is paid to psychopathology as well as to psychodynamic and semiotic aspects of the delusional illness.

[Correlation of computer tomographic findings and psychologic test results in patients with craniocerebral injury].

102 Patients with severe head injuries were examined using computed tomography and psychological tests in the rehabilitation phase. Significant correlations were found between the psychological test results on the one hand and cortical atrophia demonstrated by computed tomography as well as the transverse diameter of the third ventricle on the other. The index of cella media and the contusions of differing localisation did not show any significant correlation with the psychologic test results. Professional re-integration could be most clearly placed in relation to the duration of disease, the intellectual capacity, and irreversible changes of personality; however, there was but little relation to the findings obtained via computerized tomography.

[Digitoxin in hepatorenal insufficiency].

Serum digitoxin levels were measured during maintenance treatment with digitoxin, 0.1 mg daily i.v., in 12 patients under intensive postoperative care who had hepatorenal failure resulting from multiple organ failure. Thirteen patients in intensive care with comparable basic diseases served as controls; they had developed predominantly renal failure. Serum digitoxin levels in the two groups were no different and remained within therapeutic range during the total period of observation of 8-40 days. During this time toxic serum digitoxin levels were measured in 2.9% of patients in renal failure and 2.7% of patients with hepatorenal failure. The pathogenesis of the liver failure and the severity of the underlying disease did not influence serum digitoxin levels. Digitoxin was tolerated similarly in the two patient groups.

[Anaesthesia in a patient susceptible to malignant hyperthermia (author's transl)].

A 13 year old girl who had suffered from an abortive form of malignant hyperthermia during tonsillectomy eight years before was scheduled for orthopaedic surgery. Dantrolene sodium, 3 mg/kg orally, was given prophylactically the day before surgery; preanaesthetic medication consisted of Thalamonal, a fixed combination of droperidol and fentanyl; anaesthesia was induced with methohexitone and maintained as neurolept anaesthesia with fentanyl and droperidol; tubocurarine was administered for tracheal intubation and intraoperative neuromuscular blockade. Using this anaesthetic regimen no adverse reaction was triggered.

[Suicide at the Beverin Clinic, 1920-1979. The increase of suicides in psychiatric clinics].

Suicides in the psychiatric clinic Beverin. A contribution to the discussion of the increasing suicides of psychiatric in-patients. In the psychiatric clinic Beverin GR suicides have significantly increased within the past two decades in comparison with all hospital discharges and with the suicide rates in the general population of the canton the clinic is situated in (Graubünden). The causes considered here are the changes in the hospital management resulting in a more liberal treatment, which may have put too great a stress upon a number of patients. In some cases of depressive psychosis the use of activating antidepressive drugs was related to suicide. Rehabilitative situations should not be created at the expense of a feeling of security, especially for patients suffering from psychotic disorders.

The Sézary syndrome.

Symptoms of the Sézary syndrome are described in a case history. Sézary's syndrome is a lymphoproliferative process of the skin, which eventually develops into an erythrodermia. Strange lymphocytes with a cerebriform chromatin structure, often with T-cell characteristics, circulate in the blood stream. In few instances they are also found in lymphnodes and bone marrow. This primarily benign disease can become malignant, possibly due to an overaggressive cytostatic therapy. Besides the hitherto unknown endogenous causes, exogenous factors cannot be excluded.

Phospholipid metabolism and lysosomal enzyme secretion by leukocytes. Effects of dibutyryl cyclic adenosine 3':5'-monophosphate and ATP.

The effects of dibutyryl cyclic adenosine 3':5'-monophosphate and ATP on isotope incorporation into phospholipids and the release of beta-glucuronidase into the extracellular medium were studied in polymorphonuclear leukocytes from guinea pig peritoneal exudates. Exogenous dibutyryl cyclic adenosine 3':5'-monophosphate (0.1--1.0 mM) reduced beta-glucoronidase release induced by cytochalasin B in the absence of inert particles. It selectively inhibited 32Pi incorporation into phosphatidic acid and the phosphoinositides and the incorporation of myo-[2-3H]inositol into the phosphoinositides. Added ATP (0.1--1.0 MM), but not other nucleotides, was found to potentiate beta-glucuronidase release provoked by cytochasin B, but it impaired the labeling of the phosphoinositides by myo-[2-3H]inositol. The mechanism of the inhibition the isotope incarparation into these acidic phospholipids by the two mucleotides has not been defined. Dibutyryl cyclic adenosine 3':5'-monophosphate at 2--4 mM concentration was not found to appreciably alter the incorporation of [gamma-32P]ATP into phosphatidic acid, phosphatidylinositol, diphosphoinositide, and triphosphoinositide.

[The Sézary syndrome].

A case of the rare Sézary syndrome is described. The Sézary syndrome is a lymphoproliferative disorder of the skin, the lymph nodes, and to a smaller extent the bone marrow, which begins with erythrodermia. In the blood, strange circulating lymphocytes with a "cerebriform" chromatin structure and T-cell properties are found. The disease is primarly benign but may become malignant. The origin is unknown. Endogenic and exogenic factors are considered and relationships to other cutaneous lympho-proliferative diseases and treatment are discussed.

[Hematological problems in geriatrics].

By way of introduction, the physiologic alterations of blood cells in old age are described. Besides the well known anemias in younger persons, protein deficiency may be an additional cause of anemia in the elderly. Acquired sideroblastic anemia of varying etiology is more often seen in the elderly than in younger people. In pernicious anemia the daner of gastric cancer has been overestimated. Aggressive treatment of acute leukemias is not indicated in patients over 60. The special form of smouldering leukemia is mentioned. The syndrome of anemia, thrombocytopenia and enzymatic dysfunction of granulocytes may, it is suggested, be a symptom of preleukemia. Anemia with accelerated sedimentation rate responsive to steriods is helpful in diagnosing polymyalgia rheumatica in the oligosymptomatic form.
