An LH-RH analogue (Zoladex) in the management of carcinoma of the prostate: a preliminary report comparing daily subcutaneous injections with monthly depot injections.

The luteinizing hormone releasing hormone, Zoladex, has been used in three centres, Pontefract, Antwerp and Mistelbach, to treat carcinoma of the prostate. An initial protocol using a soluble daily injection has been followed by a second study employing a monthly administered depot preparation. After an initial stimulation it has been shown that both daily and monthly injections reduce plasma testosterone to castrate levels. Circulating luteinizing hormone levels are also initially stimulated and then suppressed. Treatment toxicity has been minimal and in these short term studies reduction of acid phosphatase and subjective and objective tumour responses have been similar to those expected from effective hormonal therapy of prostatic cancer.

Determination of free thyroid hormones and their binding proteins in a patient with severe hyperthyroidism (thyroid storm?) and thyroid encephalopathy.

A patient with severe hyperthyroidism (thyroid storm?) and thyroid encephalopathy is described. During her illness only a slightly raised level of total thyroxine and a normal level of total triiodothyronine was found in contrast with very high levels of free thyroid hormones. Very low levels of thyroxine binding globulin, albumin and low levels of thyroxine binding prealbumin in contrast with nearly normal values of T3 resin uptake were observed. All parameters of thyroid function returned to normal after therapy.

IgD plasma cell leukemia and uterine malignant mixed Müllerian tumor.

The case of a 74-year-old female with an IgD kappa plasma cell leukemia is presented. In contrast to the majority of the cases in the literature, this patient responded rather well to melphalan and prednisolone and survived for 20 months after starting treatment. Another remarkable feature was the simultaneous occurrence of a double uterine neoplasm, a malignant mixed Müllerian tumor of the homologous variety (also called carcinosarcoma).

Hypothalamic-pituitary function in myotonic dystrophy.

Function of the hypothalamic-pituitary axis was investigated in seven patients with myotonic dystrophy (MD). HGH and ACTH secretion were normal. TSH response to TRH was impaired in about half the cases, without concomitant thyroid dysfunction. LH and FSH levels were often elevated, with inconsistent response to LH-RH stimulation, Gonadotrophin disturbances in MD have previously been attributed to a primary gonadal lesion, characteristically seen in this disease. High prolactin levels in six of our seven patients however suggest that gonadal failure may be also be due to hyperprolactinemia through the direct anti-gonadal effect of prolactin and its interference with hypothalamic-pituitary regulation of gonadotrophin secretion.

Pituitary abscess. Case report.

A case of pituitary abscess is reported in which the diagnosis was made during surgery. The difficulties in diagnosis and management of patients with this rare entity are stressed.

Hormone refractory disease.

Hormone refractory disease is observed in less than 20% of newly diagnosed cases of advanced prostatic cancer. In the majority of cases, hormone refractory disease appears after a median time of 18 months of endocrine manipulation and is attributed to the selection and/or cloning of pre-existing or de novo appearing hormone-independent or resistant cell lines. There are no generally accepted rules for second-line management. The varying sets of criteria used by different study groups make comparisons of widely different regimens very difficult. Actually, it seems reasonable to consider length of survival as the only objective response criterion. This implies, however, that there should be an unanimous definition about the moment of primary treatment failure. Indeed, the detection of hormonal escape is a gradual event and the relative length of survival time depends on the chosen moment of therapy administration. To date, monitoring of prostate specific antigen (PSA) has become the best and primary tool to document progression of disease. Earlier diagnosis based on a rise in PSA levels in patients that are still asymptomatic with a good performance status, might provide the opportunity to treat patients that could profit from therapy and give a fair chance to the investigated drug to show efficacity and tolerability. In this paper we will discuss the rationale of the current available second-line therapeutic options in relapsed prostatic cancer.

Detection of early myocardial tissue changes in acute canine myocardial infarction by ultrasonic tissue characterization methods.

To determine the time after left anterior descending (LAD) artery occlusion in which myocardial texture changes first occur, two-dimensional echocardiograms on five sham operated dogs and eight dogs before and after proximal LAD occlusion were performed. Serial two-dimensional echocardiograms were performed every 15 mins by placing a 5 MHz transducer directly on the chest wall. The dogs were sacrificed at the end of the study and the area of myocardial infarction was confirmed by triphenyltetrazolium chloride staining. The two-dimensional echocardiogram images were digitized. The mean pixel intensity +/- SD in the area of myocardial infarction (region of asynergy) and a normally moving area (control region) equidistant from the transducer were calculated; thus, each dog served as its own control. In sham operated dogs, no significant changes in mean pixel intensity, skewness or kurtosis were observed. There was a significant increase (P less than 0.01) in mean pixel intensity in the infarcted regions (anteroseptal) compared to values obtained in the normally moving (lateral) regions at 30 mins (mean pixel intensity 25.2 +/- 2.1 versus 23.0 +/- 0.7, P less than 0.05); this difference persisted at 45 mins (mean pixel intensity 25.8 +/- 0.9 versus 22.6 +/- 0.5, P less than 0.01), 60 mins (mean pixel intensity 27.2 +/- 1.4 versus 22.8 +/- 0.8, P less than 0.01), 90 mins (mean pixel intensity 28.6 +/- 1.6 versus 22.5 +/- 0.9, P less than 0.01) and 180 mins (mean pixel intensity 28.7 +/- 1.8 versus 21.3 +/- 1.4, P less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)
