A tridimensional study using cuts at a low temperature of the infratemporal region.

Although the infratemporal region is well defined anatomically, its complex topography has been the subject of numerous, and sometimes, opposite works. That is the reason why it appeared necessary for the authors to re-evaluate this topic using the original method of Combelles and Boyer, allowing to define three referential planes, and thereby, a tridimensional shape and the volume of this region. This study allows to conclude that the infratemporal region is a triangular prism with an horizontal main axis 47 mm long. Its anterior base has a mean area of 733 mm2. The posterior top consists of the Juvara slot and has a mean area of 490 mm2. This infratemporal prism contains another one, the pterygomandibular space, prismatic too. It widens out from coronoïd plane (93 mm2) to mandibular foramen plane (169 mm2) before ending as a narrow groove between the neck of the mandibular condyle and the interpterygoïd fascia. The volume of the pterygomandibular space is quite superior to the value usually reported in the dental literature. It is of 4.8 ml to 5.8 ml according to denture. These results point out the opportunity to accomodate more important volumes of anesthesic solutions, than the 1.8 ml usually performed, without any leak out of the infratemporal region.

[A study of the cranio-pharyngo-mandibular space by sections at low temperature].

After acrylic injections into the vessels and mandibular block anaesthesia the pterygo-mandibular space is studied in 38 anatomical pieces. 538 colour slides have been obtained from anatomical cuts at low temperature with the scannigraph system. The results point out that a cranio-pharyngo-mandibular space is clearly distinguished laterally, from a medial maxillo-pharyngeal space. There is no transversal communication between them. On the contrary, the cranio-pharyngo-mandibular space communicates, forward and upward, with the temporal space; forward and downward, with the sub-mandibular space. The cranio-pharyngo-mandibular space contains an interpterygoid space communicating with the oval foramen by a "chimney" largely filled by the emissary pterygoid veins. A pericondilar plexus of veins is also observed. The cranio-pharyngo-mandibular space is characterised by the cellulo-adipose tissue by which it is filled and which guides the diffusion of injected solutions to the oval foramen.

[Ophthalmic accidents during local-regional anesthesia in dentistry: clinical aspects, anatomic routes].

Ophthalmic complications of intra-oral loco-regional anesthesia are rare. Three personal cases have incited the authors to review the world literature on this topic and emphasize a few epidemiological data as well as the broad outline of the characteristic clinical picture. Various etio-pathogenic hypotheses are discussed: arterial, venous or sympathetic routes. It seems that, with our current knowledge, no univocal explanation is perfectly satisfactory.

[Anatomical considerations in the Gow-Gates technic for anesthesia of the mandibular nerve. Preliminary note].

Described in 1973, the GOW-GATES mandibular block uses extra-oral landmarks. The anesthetic is deposed to the condylar neck. Gow-Gates claims that this single injection is enough for anesthesizes the sensitive branches of the mandibular nerve. The study of anatomical cuts shows that the interpterygoïd facial guides the anesthetic drug in bottom an ahead toward the mandibular foramen. That the liquid occupies all the pterygo mandibular space and that it escapes about it behind only through the Juvara slot.

[The fasciae of the pterygomandibular space].

After a rapid description of the pterygomandibular space, the authors describe the anatomy of the interpterygoid fasciae. The interpterygoid fasciae divide the pterygomandibular region into two compartments. The interpterygoid fascia is the most important layer; its limits are defined with precision. The individuality of the pterygotemporomaxillary fascia is less well defined. The authors then report their own study about the functional anatomy of these fasciae by means of frozen sections of color-injected fresh anatomical specimens. The results confirm the importance of the interpterygoid fasciae in guiding the anesthetic solution to the mandibular foramen in mandibular nerve block.

[An evaluation of an outbreak of American cutaneous leishmaniasis in a rural community in Bolivar State, Venezuela].

An epidemiological survey based on the Montenegro intradermal reaction (IDR) was carried out in San José de Hacha, south of the Bolivar State, Venezuela, on the occasion of an outbreak of American tegumentary leishmaniasis (LTA) in that locality. The IDR was applied to 121 of the 184 local inhabitants (65.8%), with a 33.9 rate of positive reactions (44/121). The highest percentage of positivity was observed among males (39.5%) (P < 0.05) and among adults, especially in the 31 to 40 year age group (52.4%) (chi2 = 18.28; g.l. = 6). With respect to occupation, agriculturist were the most reactive with 69%. of subjects IDR positive, in 65.9% (27/41) the reaction areas ranging from 5 to 9 mm. Active lesions of LTA were identified in 22 inhabitants and scars were detected in only seven cases. It is suggested that San José de Hacha is a recent focus of LTA.
