The response of five adults with chronic aphasia to a 4-week intensive treatment course is presented. Using four language tests, pre-intervention stability was demonstrated over the month preceding the treatment programme. Following the intervention period, two subjects showed improvement in one test, and more widespread changes occurred in three subjects. On further assessment 1 month after treatment had finished, one of the two subjects who had made least progress performed better than at the end of the course, and one had returned to pre-treatment level. Although there were indications of regression in two of the three other subjects, this was not to pre-treatment level. Only one subject maintained the gains made fully. Given the short duration of the course, the changes in performance noted suggest that, providing transport is available, intensive treatment of aphasia should be available to patients with sufficient motivation and stamina.

Systemic infection with Cryptococcosis neoformans is described in a young man receiving maintenance haemodialysis. The organism was identified in thin sections of a cervical lymph node using resin embedding and silver staining. There were no predisposing factors other than uraemia. His clinical infection responded to the combination of oral 5-flucytosine and a total of 2.5 g of intravenous amphotericin B. He remains free of relapse after 30 months.
Three patients with end-stage renal failure complicating systemic amyloidosis have been treated with continuous ambulatory peritoneal dialysis for periods of 10, 14 and 18 months respectively. In each case satisfactory control of uraemia and fluid balance has been achieved.

A prospective trial was carried out in 70 patients undergoing elective total hip replacement, in order to correlate bacterial contaminants in the wound at operation with the subsequent clinical results. A new method of collecting bacteria via a swab left in the wound for the duration of the operation proved to be as effective as the conventional swabbing technique in patients with known previous hip sepsis who, following revision surgery, showed evidence of further infection.

An investigation of bacteria-like structures, which are found in large numbers in the endoplasm of the eurgregarine, Gregarina garnhami, was carried out using light and electron microscopy. Gram staining indicates that these structures are gram negative; they show orange fluorescence when stained with acridine orange. Ultrastructural observations show that they resemble the endosymbiotic bacteria found in other protozoan species. Some of these structures appear to be dividing, and the possibility that these structures are in fact bacteria is discussed.

Spoken discourse is regarded as an important component of communication assessment, but data on the discourse characteristics of the adult population, and in particular those who fall into the fastest growing section of the population (those aged over 75), are scant. Therefore, detection of deficit is commonly dependent on the assessor's opinion as to what constitutes normal performance. To determine the effects of age, education and gender on spoken discourse, the conversational interaction and picture description skills of 189 neurologically normal adults were examined. Conversation was appraised with reference to five parameters (initiation, topic maintenance, verbosity, turn taking, reference). Picture description was assessed for relevant content, length, efficiency and the inclusion of extraneous information. Level of education did not affect conversational interaction but in picture description those not educated beyond minimal school leaving produced shorter and less complete descriptions. Picture description content and length were not affected by advancing age but the older elderly (aged 75+) conveyed information with reduced efficiency. Conversational interaction style altered with advancing age, with some suggestion of change in the young elderly (age 60-74), but highly significant differences in respect of the old elderly. Gender had no effect on the parameters of conversation and no significant picture description differences were present in males and females though there were trends towards longer and higher content descriptions in females. The results have important diagnostic implications for those concerned with establishing whether pathology such as stroke has affected communication. On a wider scale, awareness of the effects of advancing age and limited education may facilitate successful communication for all who communicate with the public in a professional capacity.
Data on aphasia management were collected by questionnaires to UK speech and language therapists. Returns representing over 90% of health districts were obtained. Variation amongst districts is evident in levels of staffing for the adult neurological case load, and in speed of response to referral, frequency and types of treatment available, and in other forms of support for patients and carers. Although individual therapists or districts are able to adhere to some of the professional recommendations for a 'quality' service, few are able to provide this high level of care in all aspects of aphasia management.
