Epinephrine as a tool to investigate the question of recycling of sympathetic storage vesicles in the heart: chemical and morphological studies.

The aim of the present investigation was to examine the question of reutilization of norepinephrine storage vesicles in the cardiac sympathetic nerves. More specifically, the question was whether exocytotically depleted stores of norepinephrine could be restored by epinephrine and the restoration of epinephrine could be demonstrated by chemical and morphological examinations. Stimulation of guinea-pig cardiac sympathetic nerves (1 Hz, 15 sec/min for 75 min) in the presence of 20 mM tetraethylammonium lowered norepinephrine fro 2.24 to 0.46 micrograms/g. Exposure of such hearts to 6 or 60 microM epinephrine increased epinephrine contents to 1.4 and 2.2 micrograms/g, respectively. Accumulation of epinephrine by normal or norepinephrine-depleted heart was depressed over 90% by desipramine. If intermittent stimulation was given in the presence of tetraethylammonium for only 10 min, norepinephrine and epinephrine values of epinephrine-loaded hearts were reduced over 50%. Perfusion of such hearts with epinephrine-Krebs' solution raised epinephrine content to 3.3 micrograms/g. Stimulation-evoked overflows of epinephrine after first and second loading with epinephrine were almost identical. Ca-free plus 1 mM ethylene glycol bis)beta-aminoethyl ether)-N,N'-tetracetic acid completely blocked stimulation-evoked overflow of epinephrine. Dense-cored vesicles were observed in sympathetic nerves located in the region of the S-A node. Stimulation of nerves in the presence of tetraethylammonium resulted in almost complete disappearance of dense-cored vesicles. Perfusion of such treated hearts with epinephrine led to reappearance of dense-cored vesicles. The number of dense-cored vesicles after exposure to epinephrine was, in fact, greater than that seen in the control tissue. Taken together, our data provide additional support to the concept that storage vesicles are reused in sympathetic nerves of the guinea-pig heart.

[The blood coagulation system in liver diseases with special reference to liver transplantation].

The central function of the liver as place of the formation of the most plasmatic coagulation factors in expressed by coagulation disturbances in severe liver diseases. Apart for intoxications, vitamin-K-deficiency and relatively rare congenital dysproteinoses the synthesis of the coagulation factors is apparently a relatively stable and rapidly recoverable function of the hepatocyte. On the other hand, however, the pathogenetic dominance of the accelerated balance in the sense of a disseminated intravasal coagulation and a secondarily increased fibrinolysis is emphasized for the severe disturbances of haemostasis in hepatopathies. In all kinds of shock, in tumours, inflammations, sepsis and intoxications as well as in portal hypertension of any reason the activation of the coagulation system up to the consumption coagulopathy develops. In the liver transplantation in addition to the operation shock deterioratingly comes the intensive blood contact with the often heavily damaged graft cells, the effectors of the immune system (rejection), the temporary disturbance of the clearance function of the liver RES and the basic disease of the recipient which in most cases disposes to the disseminated intravasal coagulation. The reduction of thrombocytes and coagulation factors extensively transgresses the changes which are to be expected by a weakness of the synthesis during and immediately after the anhepatic phase. For the therapy the demand of rapid removal of the coagulation-activating moments (minimum times of ischaemia, careful anastomoses) and of an adequate substitution with simultaneous prevention of the disseminated intravasal coagulation by heparinisation.

Restimulation properties of cloned alloactivated lymphocytes: detection of a novel type of HLA-D/DR region determinant and allelic subtypes for HLA-Dw3 using cloned reagents.

Lymphocytes alloactivated by Dw3 homozygous typing cells were cloned by the method of limiting dilution and cultured for prolonged periods using T-cell growth factor and irradiated pooled leukocytes (as feeder cells). Restimulation specificity of two clones functioning as primed lymphocyte typing reagents was investigated in panel and family studies. Cells from one of the clones (12-2) were always specifically stimulated by HLA-Dw3 antigens shared between the original priming cells and the stimulating panel cells. In an informative family KOH, however, cells from this clone seemed to detect a split in the Dw3 cluster. Cells from the other clone (12-8) failed to respond to Dw3 antigens as expressed by the original priming cells or by panel stimulating cells; rather, specificity of restimulation seemed to be associated with the expression of Dw4 antigens. Family segregation analysis did not support this conclusion however, since stimulating products segregated with one of the three Dw3-bearing haplotypes and with none of three Dw4-bearing haplotypes. This suggested that 12-8 cells may be responsive to antigens different from those detected by homozygous typing cells.

Strong association of idiopathic membranous nephropathy (IMN) with HLA-DR 3 and MT-2 without involvement of HLA-B 18 and no association to BfF1.

The present study investigated the distribution of HLA-antigens and Bf alleles in a group of 21 patients suffering from biopsy proven Idiopathic Membranous Nephropathy (IMN). A statistically significant increase of HLA-DR3 (76.2%) was detected in the patient group as well as an increase of MT-2 (86%), which is a new supertypic specificity defined within the 8th International Histocompatibility Workshop 1980. No association between BfF1 and IMN can be deduced from this study. Only the common Bf alleles (BfFF, BfFS, BfSS) were found in the patient group presented here. Also the previously reported association between HLA-B18 and IMN was not corroborated by this study. Not a single IMN-patient typed positive for HLA-B18. Thus the presence of two different HLA-DR-Bf-B linkage groups (HLA-DR3/BfF1/HLA-B18 and HLA-DR3/Bf; all alleles/HLA-B8) may point to at least two different immunological mechanisms underlying IMN.

Concentration of cefuroxime in cerebrospinal fluid in patients with bacterial meningitis.

10 adults with bacterial meningitis were given 1.5 g cefuroxime intravenously 4 time daily in addition to the normal antibiotic treatment. CSF levels of cefuroxime ranged from 1.5 to 13.5 mg/l (mean 6.0 mg/l) in the aucte stage of the disease. In the convalescent stage, the cefuroxime levels in the CSF varied from less than 1 to 7.5 mg/l (mean 3.2 mg/l). The CSF levels of cefuroxime in all but one measurement, by far exceeded the MIC values reported for the vast majority of strains of the pathogen commonly associated with meningitis.

Human T lymphocytes grown in T-cell growth factor: functional attributes in MLC, CML, PLT and allogeneic suppression.

Conditions for the production of highly effective human T-cell growth factor (TCGF) and its screening in a thymidine incorporation assay are described. Culture medium supplemented with optimum concentrations of TCGF was used in bulk culture systems for the expansion of alloactivated and non-alloactivated T-cell populations. Surface marker studies of cells grown in TCGF-dependent culture showed that the majority were T cells expressing DR antigens. These cells retained their proliferative responsiveness in mixed leucocyte cultures but were unable to stimulate proliferation of normal allogeneic lymphocytes. Cultured cells which derived from mixed leucocyte cultures retained immunological specificity compared with cells not exposed to TCGF both in terms of proliferative responses caused by allogeneic cells in the absence of TCGF, and in terms of cytotoxic activity against 51Cr-labelled target cells. In addition they were capable of suppressing tritiated thymidine incorporation in mixed leucocyte cultures. These experiments show that continuous culture with TCGF can expand a variety of T-cell subpopulations with retention of distinct immunological function.

Gastric hypermotility and antropyloric dysfunction in gastric ulcer patients.

Impaired gastric emptying in gastric ulcer patients has been explained by gastric hypomotility. Amplitude and duration of the contraction and integrated motor response to an electrical vagal stimulus, however, have been found to be significantly greater in 21 GU patients than in 25 DU and 6 combined ulcer patients. This shows hypermotility of the gastric muscle in GU patients. Delayed gastric emptying in GU disease, therefore, seems to result rather from antropyloric motor dysfunction than from gastric hypotony. Abnormal motility associated with duodeno-gastric reflux appears to be a key feature in the pathogenesis of gastric ulcer.

[Proximal gastric vagotomy - an interim balance].

As a result of recent trials and meetings, the results of proximal gastric Vagotomy (PGV) can now be judged with more certainty: the rate of recurrence is higher than after partial gastrectomy or recurrence is higher than after partial gastrectomy or combined procedures, but mortality is low and sequelae are rare. Errors of indication with regard to pyloric and prepyloric ulcers and inadequate denervation because of inaccurate surgical technique are the main causes of recurrence. Recurrent ulcers after PGV usually have good prognosis.
