Percutaneous left ventricular angiography.

Recurrence of persistence of heart failure in patients after mitral and aortic valve replacement is a difficult diagnostic problem. Clinical evaluation and noninvasive tests cannot always distinguish poor left ventricular function from paraprosthetic leak or persistent pulmonary vascular disease. Full evaluation by cardiac catheterisation may then be necessary but is difficult because of the inaccessibility of the left ventricle. Under these circumstances, left ventriculography by direct transapical puncture is an alternative to crossing the prosthesis by a catheter and may be the only way of obtaining diagnostic information. We describe our experience of left ventriculography by percutaneous transapical passage of a flexible angiographic catheter (as distinct from a rigid angiographic needle) in 15 such patients (on 16 occasions). Non-fatal complications occurred in three patients. The information thus obtained allowed a clinical decision to be made and distinguished the inoperable from the operable group. Surgery was subsequently performed in seven patients with beneficial results.

Detection of pulmonary arteriovenous fistulae in patient with cirrhosis by contrast 2D echocardiography.

A case of multiple pulmonary arteriovenous fistulae associated with cirrhosis of the liver is described. The diagnosis was made by contrast two-dimensional echocardiography and confirmed by cardiac catheterisation and pulmonary angiography. As it is a sensitive and non-invasive technique, contrast two-dimensional echocardiography may be a useful screening test in cyanotic patients with liver disease in whom the presence of pulmonary fistulae is suspected.

Clinical application of amplitude processing of echocardiographic images.

In order to assess the possible clinical value of measuring regional amplitude of ultrasound reflected from intracardiac structures, two-dimensional echocardiographic images from 20 normal subjects and 70 patients with heart disease were processed by modulation of both colour and intensity to represent grey scale. Maximum echo intensity was consistently recorded from the pericardial interface behind the posterior left ventricular wall, and this was taken as 100 per cent. In the normal heart, central fibrous body reflected at 64 +/- 5 per cent, and mitral and aortic valves at 35 +/- 5 per cent and 36 +/- 8 per cent, respectively. Normal septal myocardium gave a valve of 33 +/- 8 per cent and posterior wall of 23 +/- 6 per cent. Consistent increases were recorded from prosthetic mitral valves in 20 patients, and also from the anterior cusp of the mitral valve (54 +/- 11%) in 20 patients with rheumatic heart diseases. In all of 15 patients with left ventricular involvement caused by coronary artery disease, septal echo amplitude was increased in all to 71 +/- 11 per cent, and localised increases were also noted in the posterior wall, subendocardially, or in one or both papillary muscles. Similar focal changes were noted in five, and increases in septal density in 10 of 15 patients with left ventricular hypertrophy and in 12 of those with mitral valve disease. Thus, measurement of regional echo amplitude is possible without degradation of the quality from standard two-dimensional cardiac images. Abnormalities are particularly common in left ventricular disease where their distribution corresponds to that described for fibrous tissue.

Echocardiographic demonstration of aortic root abscess after infective endocarditis.

We describe a patient with infective endocarditis in whom the diagnosis was apparent only on two-dimensional echocardiography. There was no clinical or haematological evidence of active endocarditis and the diagnosis was made by the demonstration of an abscess cavity and vegetation posterior and lateral to the aortic root, deforming the left atrial cavity. The findings were confirmed by cardiac catheterisation and operation.

Synthesis of steroidal nitrosoureas with antitumor activity.

Four steroidal nitrosoureas with structures which may permit specific binding to estrogen receptor were synthesized. Inhibitory activity was observed against the growth of the DMBA-induced transplantable rat mammary tumor 13762.
