We describe a late survey of 51 patients who had hemangiomas of the eyelid in infancy, together with the visual complications found and a program designed for prevention of the latter.
A system for subjective and objective clinical analysis of the lower third of the nose before and after lengthening the columella was devised, and used the study of photographs of 77 patients who had had lengthening done by one of 4 approaches. It was found that each method was equally effective in lengthening the columella and in restoring a normal relationship between the structures in the tip of triangle (the basal view of the nose). However, there were certain peculiar advantages and disadvantages of each surgical technique, and these are discussed.
The major findings in a study of 51 infants and children with infantile hemangioma of the eyelid were as follows: Visual complications occurred in 27 patients. The most common complications were amblyopia (in 22) and strabismus (in 17). Amblyopia of 6/60 (20/200) or less was probably caused by stimulus-deprivation, but amblyopia in the range of 6/30 (20/100) to 6/12 (20/40) was likely caused by anisometropia or strabismus. Eyelid occlusion of six months or more invariably resulted in amblyopia of 6/60 (20/200) or less. Occlusion for even one month carried a risk of amblyopia. Each child must be considered individually for therapy, which must be started as early as possible. Patients should receive careful follow-up from the beginning to prevent severe amblyopia. For difficult cases, we need more efficacious, safe methods to induce regression of these tumors.
As a result of a previous trial in which factors influencing swelling of the hand after fasciectomy were studied it was found that unacceptable oedema occurred in hands elevated in a roller towel by night and a high sling by day. An alternative system of elevation in a vacuum splint was investigated and found to reduce post-operative oedema. The advantages and disadvantages of the vacuum splint in the management of the hand after surgery or injury are discussed.
The malignant complications of irradiating benign conditions are well known and fortunately less frequent than in a previous era of widespread use of superficial roentgen therapy. However, because of the long latent period from exposure to irradiation until the development of the tumour, the clinician must continue to be alert. The behaviour and treatment of a haemangiosarcoma occurring 18 years after irradiation of a haemangioma is described.
Two groups of patients with Dupuytren's disease were treated by limited fasciectomy. A tourniquet was used for one group, and an elevated hand table without tourniquet for the other. Taking into account other factors controlling oedema it was discovered over a post-operative period of twenty-eight days that "Tourniquet" hands remained significantly increased in volume compared to "Non-Tourniquet" hands. Other facts of clinical interest also emerge from the trial and are discussed. It is concluded that the use of the elevated hand table without tourniquet, not only allows an unlimited and uninterrupted period for hand surgery, but also reduces the subsequent oedema.

The notes of 158 patients who had undergone surgery of the parotid gland for benign and malignant disease were studied to discover the circumstances in which the facial nerve might be injured. Limited surgery carried as great a risk of injury to the facial nerve as during extended surgery. Extended surgery provided greater risk of injury to the facial nerve on secondary exploration (71 per cent) than on primary exploration (6-5 per cent). It is concluded that every primary exploratory procedure of the parotid gland should be a formal conservative parotidectomy unless the tumour is obviously malignant.
Arthrobacter crystallopoietes growing exponentially as cocci were changed to rods by adding succinate to the medium. Cells were sampled before, during, and after this transition for Gram-staining and ultrastructural studies. Cells were Gram stained by the standardized method of Bartholomew, and all samples were fixed and prepared for thin sectioning in an identical manner. Cocci were gram positive, and thin sections demonstrated a gram-positive type of cell wall having an average thickness of 31 nm. Cells sampled during morphogenesis appeared as cocci with most having a single rodlike projection. The coccus portion of these transition cells was gram positive and bound by a gram-positive type of wall having an average thickness of 29 nm. The rodlike projection of the transition cells appeared to be gram negative; it was also surrounded by a gram-positive type of wall, but its average thickness was only 22 nm. Gram-negative rods of the type species, Arthrobacter globiformis, were also examined and found to produce a gram-positive type of wall with a 19-nm average thickness. Evidence for the trilaminar region, characteristic of most gram-negative bacterial cell walls, was totally lacking in both species. These results suggest that variations in cell wall thickness may be an important contributing factor to the variable Gram-staining characteristics of this genus.
1. Concentrations of retinal (vitamin A(1) aldehyde) in the plasma and liver of laying hens, mature cockerels, immature pullets and pullets undergoing sexual maturation have been measured. 2. The plasma of laying hens contained about 8mug. of retinal/100ml., about ten times that found in the plasma of mature cockerels and immature pullets. In laying hens that had received large doses of retinyl palmitate 8-4 weeks previously, the mean concentration of retinal was 18.3mug./100ml. of plasma. 3. The appearance of significant amounts of retinal in the plasma of maturing pullets coincided with hypertrophy of the oviduct, increase in concentration of plasma lipid and onset of egg-laying. 4. Retinal was present in the livers of all types of fowl examined and the concentrations, which ranged from 0.2 to 5.8mug./g. wet wt., were highly correlated (r=0.79) with the concentrations of liver retinyl esters, which ranged from 92 to 1530mug./g. wet wt.
The application to soft drinks of a microtitration plate enzyme-linked immunosorbent assay for the determination of quinine is described. Dilution of sample allowed direct and simple analysis. Intra- and inter-assay coefficients of variation were 5.7% and 6.7% respectively. The advantages and disadvantages of the immunological approach over conventional techniques are discussed.
The reproducibility of a commercially available kit utilising enzyme-linked immunosorbent assay (ELISA) for determination of total aflatoxins has been analysed. Peanut butter samples contaminated with aflatoxin at three concentrations (mean 5.8, 11.7 and 24.8 ng/g) were analysed on different occasions using four kits. Coefficients of variation at all concentrations ranged from 2.6% to 13.6% within replicate, 0.3% to 16.2% within plate and 6.7% to 10.2% between kits. Standard curve precision profiles showed a minimum coefficient variation (CV) of 2-4% and a working range (below 10% CV) covering almost the entire standard curve.
A rapid method for the determination of quinine in bitter drinks by micellar electrokinetic capillary chromatography (MECC) with ultraviolet (UV) detection is described. The beverage is simply diluted with deionized water, filtered, and analyzed using a 75 cm x 75 micrometer uncoated fused-silica capillary column with a buffer consisting of 15% methanol and 85% of a mixture of 0.05 M cetyltrimethylammonium bromide (CTAB), 0.01 M sodium tetraborate, and 0.01 M potassium dihydrogen orthophosphate, pH 8.6, with an operating voltage of -25 kV. The levels of quinine determined by MECC were in good agreement with those determined by HPLC. The CVs for area calculation (2.1%, n = 7) and migration time variation (1.3%, n = 20) for multiple injections of a sample solution by MECC were satisfactory.
