The imidazoquinoline and imidazoquinoxaline food mutagens found in cooked meat are being synthesized by unambiguous methods that allow for the preparation of sufficient quantities of material for biological studies. These methods avoid difficult separations of regioisomeric mixtures of products and are designed to allow incorporation of specific high level tritium labeling.
One hundred seven patients with cystic fibrosis (CF) and 54 other patients with risk factors for Clostridium difficile-associated disease were entered into a bacteriologic study to compare the rate of recovery of C difficile and cytotoxin in feces with occurrence of diarrhea and to investigate potentially protective or permissive relationships of fecal flora. Toxigenic C difficile was recovered from 22% of CF patients and 11% of patients with other diagnoses. Unlike the latter group, the majority (12/15) of CF patients who had cytotoxin recovered had formed stools and no history of diarrhea. Explanations for the lack of symptoms are speculative. Stool flora of CF patients was significantly more likely to include several bacteria with known inhibitory effects on C difficile. Recovery of C difficile and cytotoxin, however, was not associated with the decrease in rate of recovery or the mean bacterial count of any bacterium of fecal flora.
This study examined 17 characteristics of 519 clients in a large Midwestern partial hospitalization program which were thought to predict differential suitability for treatment. The Goal Attainment Scale, the Kincannon/Mini-Mult, and state hospital recidivism were used to establish outcome groups. Twelve of 17 characteristics were singularly associated with treatment outcome. Strongest associations occurred with the variables previous state hospitalizations, age at admission, age at onset, intelligence, and a parental incidence of mental illness. Models for predicting success were developed for discriminating successful from less successful clients.
Assays for cytotoxin of Clostridium difficile were performed on stool samples submitted to the laboratory for routine microbiologic study. Cytotoxin was recovered from 8.6% of 208 pediatric patients studied. Cytotoxin was identified significantly more often in younger patients (median age, 11 months) and in those with hospital-associated illness (17%) and antibiotic-associated illness (18%). Hospitalization and antibiotic use were significant independent risk factors. Clinical diagnoses in patients with C difficile cytotoxin included classic pseudomembranous colitis, acute self-resolving diarrheal syndrome, chronic diarrhea with failure to thrive, infant botulism, and asymptomatic carriage.

We report a case of an anephric child who accumulated significant concentrations of vancomycin during oral therapy for Clostridium difficile pseudomembranous colitis. Sustained serum vancomycin levels of 34 microgram/ml and CSF level of 4.2 microgram/ml were documented during an unexplained clinical syndrome of fever and encephalopathy. Discontinuation of drug and performance of hemodialysis reduced serum concentrations to 24 microgram/ml and were associated with rapid resolution of symptoms. Patients with renal impairment and intestinal disease who receive oral vancomycin may absorb and accumulate significant amounts of drug.

The particulate matter content of six commonly used types of large-volume parenteral solutions was evaluated using five methods. The five methods were: (1) visual examination with the naked eye under diffuse light, (2) visual examination with a 2.5 diopter lens under diffuse light, (3) examination with a light scattering apparatus, (4) instrumental examination using the Prototron particle counter, and (5) a membrane filtration and microscopic examination technique. The visual methods (1) and (2) proved to be inadequate because of the insensitivity and high degree of subjectivity. The light-scattering apparatus was reasonably accurate and a considerable improvement over the first two techniques. The Prototron instrument was shown to be capable of predicting whether a solution would pass or fail the recently adopted USP-NF standards for particulate matter content, when compared with the membrane filtration and microscopic examination technique. All of the solutions tested were found to contain very low levels of particulate matter in relation to the USP-NF standard. It is recommended that this recently adopted standard be reevaluated with a view toward increased stringency.

1. Cytochrome c was isolated from horse heart by a chromatographic method. 2. Oxidized and reduced cytochrome c were chromatographed on CM-cellulose that was in equilibrium with several buffer systems of constant composition at pH values of 8.4, 6.75 and 4.9. 3. Separation was better at the higher pH values; the oxidized form was retarded more than twice as much as the reduced form, though they differed by only a single charge. 4. Self-competition between cytochrome molecules is suggested to account for the peak distortion observed at high loads (above 20mum protein concentration).

Previous studies have shown that impurities in commercial organophosphorus insecticides induce a variety of toxicological manifestations. Few studies have contrasted common impurity types and their comparative chemical and biochemical properties. In this study, five O,O-dimethyl phosphorothioate compounds were converted to their corresponding O,S-dimethyl phosphorothioates (isomerides) by a stepwise dealkylation-alkylation process (yields 58-76%). The O,S-isomerides and parent material were characterized by reverse-phase high-performance liquid chromatography (RPHPLC) and phosphorus (31P) nuclear magnetic resonance (NMR) spectroscopy. Methanol-water mixtures were found to adequately separate isomeride from parent structure with the isomeride eluting first. In general, the O,S-isomerides were found to be shifted about 40 ppm upfield relative to the O,O material. Isomerides were also determined to be significantly more potent as anticholinesterases (rat brain), with ki values approximating 1000-fold those of the parent material.
