Lethal arthrogryposis multiplex congenita: a pathological study of 21 cases.

Twenty-one cases of arthrogryposis multiplex congenita, which had resulted in death soon after birth or had been aborted following prenatal diagnosis, were studied. Histochemical and histological study of muscle indicated that 11 cases were of myogenic origin, including congenital muscular dystrophy in 10 cases from six families and nemaline rod myopathy in one. Neurogenic causation was established in five cases, including three with intra-uterine anoxic-ischaemic damage and two siblings with a severe form of cerebro-ocular-facio-skeletal syndrome. Causation remained uncertain in five. Unusual features included atrophy or amyoplasia of the diaphragm associated with lung hypoplasia in 10 cases and evidence of birth trauma in seven cases. One pair of siblings had subcutaneous tissue of doughy consistency and another pair had bladder hypertrophy. Familial recurrence was seen most often in cases with evidence of myogenic origin. We consider that neuropathology and muscle histochemistry are essential aids in determining the risks of recurrence in this group of lethal conditions which defy analysis by syndrome recognition techniques.

The effect of mycophenolic acid on the cell cycle of Candida albicans.

Mycophenolic acid inhibited the growth of Candida albicans. Cultures exposed to a concentration of 8.4 micrograms ml-1 mycophenolic acid were found to exhibit cell cycle arrest with two or more buds. Nuclear staining revealed that these were nucleate implying a possible defect in cytokinesis. The results are discussed in relation to the possible mode of action of mycophenolic acid.

Renal dysplasia--a clinicopathological review.

A review of 15 cases of unilateral renal dysplasia is presented. Seven patients came to medical attention with urinary tract infection. Only 4 presented with an abdominal mass. Eight patients were found to have ipsilateral ureteric anomalies and 4 of these also had contralateral ureteric problems. Five patients had extra-renal anomalies and these included Turner's syndrome, scoliosis and congenital dislocation of the hip. Histological examination, in all cases, revealed the presence of primitive ducts surrounded by concentric layers of cellular mesenchyme. Metaplastic cartilage was noted in only 7 of the 15 cases.

Children's asthma: new approaches, new understandings.

The purpose of this study was to discover how asthmatic children perceive their disease in the hope that this information can aid physicians' work with their young asthmatic patients. The researcher's intent was to discover children's perceptions of asthma through studies of their drawings and their verbalizations concerning their disease.

Effects of route of fluid administration on severity of side effects with post-metrizamide myelogram.

The effect of route of hydration on the severity of side effects of nausea, vomiting, and headaches for 13 patients receiving a metrizamide myelogram was studied. Six patients were hydrated by mouth only and seven by mouth and intravenously. Severity was measured 24 hours postmyelogram with a visual analogue scale for each side effect. Controlling for the volume of all fluid received, significantly more severe headaches were reported by patients hydrated only orally. For this small sample, no significant differences were found for severity of nausea or vomiting. The study and its implications for nursing are described.
