The brow-lift 1978.

The direct brow-lift for correction of brow ptosis can be one of the most beneficial surgical procedures in cosmetic surgery. Careful preoperative evaluation and proper symmetrical marking of the correction are key elements for successful results. We discuss the surgical technique in detail.

Renal stone epidemiology: a 25-year study in Rochester, Minnesota.

There are no adequate studies of the incidence of urolithiasis in the United States, in spite of earlier claims that a "stone belt" exists in the southeastern section of the country. This report is the first description of the incidence and recurrence rates for symptomatic noninfected renal stones in a well-defined population. A total of 798 patients were enrolled in the study group, of whom 672 were incidence cases having had their first episode as documented residents of Rochester, Minnesota, between 1950 and the end of 1974. The annual age-adjusted incidence rate for females was stable over the 25-year study period at 36.0 per 100,000 population. That for males increased significantly (P less than 0.02) from 78.5 per 100,000 to 123.6 per 100,000. Recurrence calculations showed a high rate for both sexes in the first year, followed by lower but constant rates for all succeeding years.

Behavioral contrast during the acquisition of speechreading.

Two hearing-impaired children were taught to speechread the names of 18 objects divided into three sets. A multiple baseline across sets of objects was obtained for each subject. Tokens presented contingent upon correct responses to certain sets of names increased the frequencies of those correct responses. Both subjects exhibited positive behavioral contrast during the acquisition of speechreading; correct responses to one set increased above the previously established level when the contingencies for a second set changed from token reinforcement to extinction. The obtained contrast effect was conductive to learning and the procedure was effective in teaching the children to speechread the training words.

Behavior Therapy

Use of micro-ELISA for quantitating antibody to Trypanosoma cruzi and Trypanosoma rangeli.

Of 229 residents of a Panamanian village where both Trypanosoma cruzi and Trypanosoma rangeli are endemic, 52% had antibody to one or both species by the enzyme-linked immunosorbent assay (ELISA), 26% were positive by complement fixation, 17% were positive by direct agglutination, and 32% were positive on the basis of clinical impression. Although the sensitivity of ELISA makes it the procedure of choice for sero-epidemiologic studies, there does appear to be some serologic cross-reactivity between the two species. Sera from 55 Panamanians and 33 Bolivians were reactive with antigens of T. cruzi and T. rangeli, thus making definitive diagnosis by serologic methods impossible. Although the presence of antibodies to both species suggests serologic cross-reactivity, the possibility of dual infection must be considered also. Fifty-four Panamanians and 20 Bolivians had antibody to only one species.

Antibodies

A self-administered history questionnaire for cosmetic facial surgery candidates.

The collection of information concerning a patient's history is every bit as important in the evaluation of candidates for cosmetic surgery as it is in any other branch of medicine. Consequently, a self-administered questionnaire was developed to save time and to ensure thoroughness. The questionnaire helps elicit a patient's motivations and expectations, and also satisfies some of the requirements of the legal doctrine of informed consent. A particularly noteworthy feature of the form is the "Personal Inventory" section, which attempts to "red flag" patterns of psychogenic conditions in prospective patients.

Face

A flap suspension technique in blepharoplasty on lower lids.

Blepharoplasty on lower lids frequently results in a downward pull on the free border of the lid and inferomedially on the lateral canthus. Extending the incision in the lower lid upward and lateral to the outer canthus allows most of the excision to occur laterally. Closure then pulls superolaterally, which tends to prevent the commonest complications of blepharoplasty on lower lids.

Self-incompatibility systems as bioassays for mutagens.

Many flowering plants are unable to set seeds with their own pollen because a system known as gametophytic self-incompatibility is operating. The basis of this system is a single multiallelic locus S, and if the S allele carried by a pollen grain matches one of the two S alleles carried in the style, as it is certain to do upon self-pollination, then pollen tube growth is inhibited. Should one of the self-pollen grains carry a mutated S allele, however, it would not match either of those carried in the style and would therefore, not be inhibited. Gametophytic self-incompatibility thus provides a mechanism for discriminating between such mutant and nonmutant pollen grains. Knowing the numbers of pollen grains available to the stigma, and also the numbers of seeds produced, it becomes possible to estimate the frequency with which mutations occur at the S locus. Assay systems of mutagenesis which employ gametophytic self incompatibility will allow very large numbers of pollen grains to be screened for S allele mutants, which should indicate the mutagenicity of the environment. These systems have the added benefit that screening is done by the stylar tissues, rather than technicians. Finally, they may be used to construct largely autonomous assay systems which would provide continuous monitoring of the environment.

Alleles

When the patient wants facial cosmetic surgery.

Cosmetic facial surgery can improve appearance but it cannot make everyone beautiful (or handsome) or solve all problems. The patient must be well motivated and must comprehend the limitations of the procedures. Procedures useful in providing a more youthful appearance include blepharoplasty, browlift and face-lift. Sometimes all three are indicated. Chemical face peeling is used for correction of fine skin lines.

Adult

The deviated nose-its correction.

The deviated nose remains a challenge to any rhinoplastic surgeon. The authors approach this problem via a one-stage septorhinoplasty based on the fundamental anatomic deformity present. The technique involves taking the nose apart, realigning the building blocks, and carefully splinting of the nose for proper realigning.

Humans

Acquisition of lipreading in a deaf multihandicapped child.

Operant techniques were used to develop lipreading in a nine-year-old deaf boy who had failed to respond appropriately to previous speechreading training. Remediation was initiated with two experiments: the first entailed a reversal and the second a multiple baseline. A component analysis revealed that both tokens and social reinforcers presented simultaneously or separately were effective in shaping lipreading. In addition, the child learned to lip-read either voiced or nonvoiced names of objects.

Affective Symptoms

M-plasty techniques.

Conventional closure of surgical defects in skin may cause unsightly protrusions (dog ears). Fusiform excisions with angles of 30 degrees or less reduce protrusion but may require excision of larger volumes and lengths of healthy skin and subcutaneous tissue than may be desirable. M-plasties reduce these requirements while still allowing use of 30 degrees angles at maximum. Applications and technical details are described and illustrated.

Dermatologic Surgical Procedures

Dimethyl sulfoxide in central nervous system trauma.

Dimethyl sulfoxide has been tested in various experimental injuries of the central nervous system in relation to other therapies. It appears to be a useful drug in acute extradural mass-forming lesions, middle cerebral artery occlusion, respiratory anoxia, and spinal cord injuries, in rhesus and squirrel monkeys, dogs, and rats. The data from these studies suggest that in the experimental models used, DMSO is clearly superior to no treatment, and appears to be more generally effective than other comparable treatments. No satisfactory answer has yet been found to explain the beneficial effects of DMSO, but several hypothetical suggestions are offered; their validation hinges primarily on further confirmatory evidence. Further experiments with our present models and alternative research lines are discussed.

Animals

Pharmacologic treatment and evaluation of permanent experimental spinal cord trauma.

Permanent paralysis was induced in dogs by a 500 gram centimeter force injury on the spinal cord, and drug treatments were given 1 hour after injury and were continued for 3 days. Dogs were evaluated for 90 days. Isotonic saline or mannitol administration were ineffective in reversing the paralysis. In dogs receiving either dimethyl sulfoxide or dexamethasone, six of eight animals in the former and two of eight in the latter group regained partial or full recovery. The presence of somatosensory evoked responses taken before and at various intervals following trauma showed a good correlation in the prognostic recovery of each animal. It is concluded that dimethyl sulfoxide and dexamethasone can reverse a permanent experimental injury to the spinal cord when given within an hour after trauma.

Some tips on face-lifting.

The face-lift operation is not difficult for the well-trained otolaryngologist. However, as in every surgical procedure, there are certain techniques that facilite its performance and improve results. This paper discusses some of these techniques that have been developed during the past 20 years.
