Effect of extradural morphine on somatosensory evoked potentials to dermatomal stimulation.

The effect of the extradural (L2-3) administration of morphine 6 mg on early (less than 0.5 s) somatosensory evoked cortical potentials (SEP) to electrical stimulation of the L1- and S1-dermatomes was examined in eight patients. Extradural morphine did not influence SEP amplitude. SEP latency did not change, except for a minor increase in the latencies of the onset and the P2 components following S1 stimulation.

Drawbacks of traction-absorbing wiring (TAW) in displaced fractures of the olecranon.

Drawbacks of traction-absorbing wiring (TAW) in displaced fractures of the olecranon were observed in 29 out of 55 consecutive patients. Sliding of the K wires, with resulting skin troubles in 24 patients (10 patients with perforation of the skin), required premature removal of the implant. Minor operative modifications in the TAW technique are suggested in order to avoid these drawbacks.

Silastic arthroplasty in rheumatoid MCP-joints.

Resection arthroplasty using Swanson silicone prosthesis was performed in 74 MCP-joints in 22 patients, all with rheumatoid arthritis. Follow-up at 2 (1-7) years showed complete pain relief in 15 patients and considerable improvement in hand function; the average range of motion was unchanged but extension was improved and ulnar deviation and volar displacement were reduced. Complications included fracture of four implants, two superficial infections, one hematoma and one skin necrosis.

Acute traumatic dislocations of the patella.

Twenty-three consecutive patients with acute traumatic dislocation of the patella were followed up for a mean period of 39 months. All patients had a medial capsulorrhaphy and in eight cases a supplementary lateral capsulotomy. Dislocation recurred in two cases, one of whom had had a supplementary lateral capsulotomy. Chondromalacia was demonstrated at followup in eight patients, six of whom belonged to the lateral capsulotomy group. We conclude that lateral capsulotomy confers no advantage in the operative repair of acute dislocation of the patella.

Amputation of the forefoot. Predictive value of signs and clinical physiological tests.

Fifty-two transmetatarsal and 11 transtarsal forefoot amputations were reviewed. Primary healing was obtained in 1/4 of the amputations. No difference could be detected between diabetic and non-diabetic gangrene regarding wound healing. The local blood pressure at the ankle had a useful predictive value, whereas the blood pressure at the toe level was of no value. Skin perfusion pressure was impossible to measure in several cases and was thus of less value, as was the clinical judgement.

Synovectomy with resection of the distal ulna in rheumatoid arthritis of the wrist.

Wrist synovectomy with resection of the distal ulna was performed in 47 cases, where rheumatoid affection in the wrist joint caused visible capsular swelling, pain and reduced range of motion. At follow-up after a mean observation time of 33 months, 31 cases had complete pain relief, eight cases had moderate pain relief. In eight cases pain during wrist motion was unchanged at follow-up and in five of these, recurrence of capsular swelling was noted. The range of motion (both supination/pronation and vola- and dorsiflection) was significantly improved, as was the subjective assessment of function. The radiographic findings showed progression in 41 cases. Deviation in the wrist was unchanged. In three cases reoperations were performed and one case had spontaneous rupture of the 4th and 5th extensor tendons 1 month postoperatively. In 15 cases dislocation of the extensor carpi ulnaris tendon in volar direction by rotational movement was noted. Stability was good in all cases and no serious postoperative complications occurred. Wrist synovectomy with resection of the distal ulna is recommended in cases where regular medical treatment has been attempted for a minimum of 6 months without successful results.

Resection arthroplasty of the forefoot in rheumatoid arthritis cases.

Resection arthroplasty of the forefoot was performed in 30 patients (48 feet) with rheumatoid arthritis. A Keller/Clayton procedure yielded good results, whereas a less radical operation (Hybbinette) created poor results. Reoperation following failed Hybbinette operations was possible.
