Development of a new implantable bio-telestimulator.

The authors describe preliminarily an implantable bio-stimulating device for stimulation of peripheral nerves and the central nervous system. The receiving unit is totally implantable. The system works on radiofrequency transcutaneous coupling across the skin. This new telestimulating device has a wider band width and more flexibility than units currently being used in neurosurgery. The unit is independent of coupling efficiency and is designed to accept any symmetrical or nonsymmetrical waveform within limits listed in this presentation. The unit is designed so as to provide this waveform as a constant current signal at the output of the receiver.

[Prolactin adenomas in children (author's transl)].

During the last four years 12 Prolactin (PRL) secreting adenomas whose first clinical manifestation appeared before the age of 15 years have been studied. The first signs were a decreased growth rate and arrest of pubertal development in 4 girls and 2 boys in whom the adenoma occurred before or during puberty. In the other six cases (5 girls and 1 boy) the first signs were secondary amenorrhea and galactorrhea in girls and in the boy gynecomastia followed by galactorrhea. Symptoms of the tumour (headaches, and visual disturbances) were almost invariable (11/12) and led to the diagnosis in two cases. Plasma PRL levels were always elevated (80 to more than 5,000 ng/ml) and did not rise in response to TRH. 11 out of 12 were large tumours and pituitary tomography showed that tumour was circumscribed in five cases and invasive in the other seven. The prognosis is related to the short term outcome. The condition may be cured by surgery only (25% cases only) or by surgery and bromocriptine.

[Structural abnormalities of the Y chromosome. Observations in ten cases].

The authors report ten cases of structural anomalies involving the Y chromosome: five cases of a dicentric Y chromosome, one ring Y chromosome, one case of a Y isochromosome containing the long arms, one deletion of the long arms (Yq-), one case of an abnormally long Y chromosome (Yq+) and one Y-autosome translocation. Analysis of clinical and chromosomal correlations, especially with respect to sexual differentiation, led to discussing the role of the Y chromosome.

[Pubertal prolactin adenoma. Eight cases (author's transl)].

Among 70 operated patients with a histologically verified prolactin secreting adenoma, 8 cases (7 girls, 1 boy) presented the onset of clinical signs before, during or immediately after puberty. Two different clinical syndromes were detected. In the younger patient group (4 cases), the first clinical signs at the onset of puberty were arrest of both growth and pubertal development. In the four remaining causes where in puberty had been achieved, primary-secondary amenorrhea and galactorrhea were observed. Sellar tomograms revealed the presence of invasive adenoma in 3 patients, and circumscribed (enclosed) adenoma in 5 cases. In 7 cases, surgical management was by the transsphénoïdal approach, while one patient underwent subfrontal surgery. Post-operative results were a function of tumour size. In the seven patients having undergone transsphenoïdal surgery, 4 patients displayed a return to normal prolactin function. In the remaining patients, post-operative management was conducted with bromocriptin and without radiotherapy.

A girl with mosaicism for a dicentric X chromosome (45,X/46,X,dic(X) (Xqter to p22::p22 to qter)).

A 12-year-old girl was examined for growth retardation and a few very discrete dysmorphologic stigmata of Turner's syndrome; the genitalia were infantile yet both ovaries possessed functioning follicles. R- and C-banding techniques and Brdu treatment demonstrated a 45,X formula in 95% of lymphocytes, with 5% presenting a 46,X,dic(X) formula. Cytogenetic and clinical problems raised by this observation are discussed in relation to data from the literature.

Four new cases of Dicentric Y chromosomes.

Dicentric Y chromosomes are rare in man. Four new cases of dicentric Y chromosomes are described. The cases of the literature so far reported are reviewed. Among the cases, a wide range of variation in phenotype, external genitalia, histology, and chromosomal findings was observed. The relationship of the clinical picture and structural abnormalities of the Y chromosomes is discussed.

Thoracic aortic clamping for prophylaxis against sudden cardiac arrest during laparotomy for acute massive hemoperitoneum.

Experimental studies in dogs were performed to develop a model simulating clinical patients with penetrating wounds of the major abdominal vessels who develop sudden cardiac arrest following laparotomy and decompression of an acute massive hemoperitoneum. Hypovolemic dogs with a saline-distended tense abdomen and major arterial bleeding after laparotomy develop a sudden deterioration of cardiac function despite raped blood infusion, thus simulating the clinical problem. However, thoracic aortic occlusion before laparotomy and until control of distal arterial bleeding prevented such a cardiovascular collapse. Increased intra-abdominal pressure it itself was determental to cardiovascular function; its sudden release by laparotomy was associated with an improvement in cardiovascular function. An approach of thoracic aortic control before laparotomy is recommended in patients with acute massive hemoperitoneum who remain hypotensive before laparotomy.
