The effect of captopril on peripheral hemodynamics in patients with essential hypertension: comparison between oral and sublingual administration.

A comparative study of the effects of oral and sublingual captopril on the hemodynamics of the peripheral musculocutaneous vasculature was carried out on ten patients with essential hypertension. Both routes of administration of captopril lead to lower blood pressure and decreased regional resistance, and to an increased arterial blood flow at rest. The first measurable effect and the peak effect on blood pressure and peripheral hemodynamics appear slightly earlier with sublingual administration. The data provided in this study support the usefulness of the sublingual route in clinical situations in which oral administration of captopril is not feasible.

Clinical value of the pitch of the third heart sound in ischemic heart disease.

The study investigates the genesis of the third heart sound (S3) in ischemic heart disease based on a mass-spring model. In such a system, the natural frequency of vibration, Fn, depends on the elastic constant, k, and the mass, m, according to the following relationship: Fn = 1/2 pi square root of k/m. To identify the cardiac structures representing k and m, the correlations between the energy of the S3 spectrum and the echocardiographic parameters were searched for. The results are consistent with a model in which k is represented by the thickness of the left ventricle and m by its blood content. The k/m ratio emerges as an important determining factor of the acoustic quality of S3, and yields information on the dysfunction of the left ventricle in ischemic heart disease.

Peripheral hemodynamic effects of ibopamine in patients with congestive heart failure. A placebo-controlled, double-blind study.

Under double-blind conditions, 150 mg of ibopamine (di-isobutyric ester of N-methyldopamine) or placebo were given orally to 11 patients with congestive heart failure; after 3 hours, 50 mg of sulpiride were administered intramuscularly. Peripheral hemodynamics were evaluated at the level of the forearm using strain-gauge plethysmography. Ibopamine increased arterial blood flow and venous capacity and decreased arterial peripheral resistance; these effects were counteracted by sulpiride. No significant changes were observed after placebo and sulpiride. These findings confirm the vasodilating effects of ibopamine on peripheral hemodynamics and its utility in patients for the treatment of congestive heart failure.

A mass-spring model hypothesis of the genesis of the physiological third heart sound.

A study on the genesis of the third heart sound (S3) based on the mass-spring model is presented. In such a system the natural frequency of vibration depends on the stiffness constant and the mass according to a physical law. The amplitude versus frequency spectra of S3 of 19 patients were obtained using the FFT algorithm together with mono- and two-dimensional echocardiographic parameters. Each echo parameter was correlated with the relative energy contained in each of the 15 Hz bands in which the normalized average spectrum of S3 of each subject was divided. The relative energy of each band was related to echocardiographic parameters. Significant correlation coefficients were found between the diameter of the left ventricle measured at the end of the rapid filling phase. The thickness of the posterior wall and of the interventricular septum at this moment, and the energy contained in certain frequency bands. The statistical correlations we have revealed are consistent with the model postulated in our hypothesis.

[Evaluation of the cardiac and peripheral vascular effects of the administration of single-dose enalapril and captopril in essential arterial hypertension].

The effects of single orally-administered dose of enalapril and captopril on central and district-peripheral circulations have been studied in 20 patients with essential arterial hypertension. Either drug caused a decrease in arterial pressure, telesystolic and telediastolic parietal stress, and a reduction in the peripheral vascular resistance of upper limb district. Furthermore, at the upper limb district level, increases in arterial flow and venous-system capacity have been observed. The entity of these modifications induced by enalapril and captopril appeared comparable, according to the respective peak-time and used dosages. Under this study conditions, the difference between the drugs resulted to be essentially of chronological nature.

Effects of ibopamine on peripheral hemodynamics: a strain gauge plethysmographic study in patients with congestive heart failure.

Peripheral hemodynamics were studied using strain gauge plethysmography in patients with congestive heart failure after administration of ibopamine (SB-7505), the orally active 3,4-diisobutyryl ester of N-methyldopamine, a dopaminergic agonist, and of sulpiride, a specific dopaminergic antagonist. 50 mg of sulpiride were administered parenterally in 12 patients 3 h after a single oral dose of 150 mg of ibopamine. Ibopamine increased significantly resting arterial blood flow and venous capacity and decreased peripheral resistance. Sulpiride was found to significantly counteract the activity of ibopamine.

Multicenter study on the clinical efficacy of chronic ibopamine administration.

The purpose of this multicenter study was to evaluate the therapeutic efficacy of ibopamine (SB-7505), the 3,4-diisobutyrylester of N-methyldopamine, chronically administered to patients with severe congestive heart failure (CHF) in whom traditional therapy had been ineffective. 55 patients chronically treated with cardiac glycosides and diuretics were assigned to an investigation covering three stages each lasting 7 days. Stage A: continuation of traditional therapy (glycosides and diuretics). Stage B: addition of ibopamine to the glycoside/diuretic therapy. Stage C: withdrawal of ibopamine. Heart rate, blood pressure, body weight, diuresis and 8 target symptoms of CHF (clinical scores) and hematochemical parameters were recorded throughout the three stages. The addition of ibopamine to traditional therapy led to a significant improvement. By contrast, after ibopamine had been withdrawn patients worsened. No hematochemical parameters were modified during the trial. None of the patients experienced any significant side-effects.

Atrial septal aneurysm: echopolycardiographic study.

Atrial septal aneurysm (ASA) with diagnosis during life is a rarely reported anomaly, especially when not associated with other congenital or acquired valvular cardiopathies. Among 4,000 routine echocardiograms, 23 cases of ASA were found (8 men and 14 women, aged 19 to 79 years). Three patients had coronary artery disease, 2 had dilated cardiomyopathy, 2 had systemic hypertension, 11 had mitral valve prolapse and 5 had no other identifiable cardiovascular diseases. Echocardiography revealed various motion patterns of ASA. No phonomechanocardiographic findings were related to the presence of ASA, and no correlation between echographic and phonocardiographic findings was found.
