Preparation of the cerebroside sulfate activator (CSAct or saposin B) from human urine.

The purification of cerebroside sulfate activator (CSAct) or saposin B from pooled human urine is described. Urinary proteins are concentrated by ammonium sulfate precipitation. A suspension of the precipitate is heat-treated and the heat-stable proteins are fractionated through a series of chromatographic steps. An initial concanavalin A column retains little of the CSAct activity, but is important for subsequent purification. Passing the Con A effluent directly onto an octyl Sepharose column removes the protein of interest which is recovered by affinity elution with octyl glucoside. Subsequent ion-exchange and gel filtration chromatographies yield a protein of 80-90% purity, although it is sometimes necessary to repeat one or more steps. A small amount of CSAct can sometimes be recovered from the initial Con A Sepharose column by methyl mannoside elution and purified by a parallel chromatographic protocol. Mass spectral analysis suggests that the final material is a mixture of two major and several minor glycoforms of a 79 amino acid protein with the structure predicted from the human prosaposin cDNA by truncation of both N- and C-terminal regions. Sugar analysis revealed the presence of glucosamine, mannose, and fucose, consistent with the major isoforms bearing a five-sugar Man(2)GluNac(2)Fuc or a single GluNac substituent. The human urinary material is similar to the previously characterized pig kidney protein in most respects, but varies in some details.

Choroid plexus papilloma of the cerebellopontine angle: a twelve patient series.

BACKGROUND: Choroid plexus papillomas (CPPs), of the cerebellopontine angle (CPA), are a rare entity and no surgical series have been reported so far. We reviewed all the pertinent literature of 12 patients operated on in the last 20 years at our institution. METHODS: All the patients were adults, ranging from 19 to 61 years. The group included 6 males and 6 females. Preoperatively, on computerized tomography (CT) (n = 10) or magnetic resonance imaging (MRI) (n = 4), differential diagnosis was difficult to achieve, especially with meningiomas. Hydrocephalus was disclosed in 8 cases. Angiography (n = 11) showed tumor blush with typical vascular supply in almost half the cases. RESULTS: In 6 patients a midline approach via the cerebellomedullary fissure was used; in the remaining 6 patients the retromastoid route was adopted. We found 2 "unconnected" tumors; no hilum was identified at surgery. Total tumor removal was achieved in 6 patients, predominantly in the last cases. Two patients died in the postoperative period, 3 patients had mild additional deficits, whereas 7 patients were stable or improved. All the patients were followed up for a mean period of 8.2 years. Conventional radiotherapy was carried out in 5 patients; 1 of them after tumor recurrence. Stereotactic radiotherapy was performed in 3 patients; 2 of them after recurrences. Two patients showed tumor progression and died during the follow-up. One of them presented a suprasellar metastasis and died much earlier (2 versus 7 years). CONCLUSION: Careful assessment and surgical planning is accomplished with the combined information from CT, MRI, and angiography. Typical characteristics are the following: vascular supply from the choroidal arteries, ventral extension, adhesion to the brainstem, and lower cranial nerves. Nowadays, total removal of CPPs of the CPA can be achieved with acceptable morbidity and mortality. In our experience, conventional radiotherapy did not prove effective.

Cyclic peptides as non-carboxyl-terminal ligands of syntrophin PDZ domains.

Syntrophins, a family of intracellular peripheral membrane proteins of the dystrophin-associated protein complex (DAPC), each contain a single PDZ domain. Syntrophin PDZ domains bind C-terminal peptide sequences with the consensus R/K-E-S/T-X-V-COOH, an interaction that mediates association of skeletal muscle sodium channels with the DAPC. Here, we have isolated cyclic peptide ligands for syntrophin PDZ domains from a library of combinatorial peptides displayed at the N terminus of protein III of bacteriophage M13. Affinity selection from a library of X10C peptides yielded ligands with the consensus X-(R/K)-E-T-C-L/M-A-G-X-Psi-C, where Psi represents any hydrophobic amino acid. These peptides contain residues (underlined) similar to the C-terminal consensus sequence for binding to syntrophin PDZ domains and bind to the same site on syntrophin PDZ domains as C-terminal peptides, but do not bind to other closely related PDZ domains. PDZ binding is dependent on the formation of an intramolecular disulfide bond in the peptides, since treatment with dithiothreitol, or substitution of either of the two cysteines with alanines, eliminated this activity. Furthermore, amino acid replacements revealed that most residues in the phage-selected peptides are required for binding. Our results define a new mode of binding to PDZ domains and suggest that proteins containing similar conformationally constrained sequences may be ligands for PDZ domains.

Comparison of three different methods for radiolabelling human activated T lymphocytes.

One approach in the treatment of ovarian cancer patients involves the infusion of autologous T lymphocytes coupled with a bispecific monoclonal antibody MOv18/anti-CD3 (biMAb OC/TR), which recognizes a 38-kDa glycoprotein expressed on ovarian carcinomas and the CD3 T cell receptor. However, little is known about the in vivo biodistribution of injected activated lymphocytes, information that could be obtained by scintigraphic imaging of radiolabelled T cells in order to visualize the migratory pattern. We compared the efficiency, stability and toxicity of technetium-99m hexamethylpropylene amine oxime (HMPAO), indium-111 oxine and fluorine-18 2-fluoro-2-deoxy-d-glucose (FDG) in radiolabelling activated lymphocytes targeted with biMAb OC/TR. The mean labelling efficiencies of 111In-oxine and 18F-FDG using 2.5x10(8) lymphocytes (68% and 64%, respectively) were more than twice that of 99mTc-HMPAO (31%). Retention of the radionuclide in the cell was highest in the case of 111In-oxine labelling (less than 25% of the initial cell-bound activity released after 240 min, as compared with 44% of the 99mTc label in the same period and 45% of 18F radionuclide released after 150 min). None of the three radiolabelling reagents induced any significant alteration in cell viability or immunophenotype. However, both 111In-oxine and 18F-FDG induced a loss of cytotoxic activity of lymphocytes against the ovarian carcinoma cell line IGROV1, and all three radiolabelling reagents caused a significant reduction in the proliferative ability of labelled lymphocytes compared to controls, with cell death occurring after 8-9 days. Radiolabelling with the more stable 111In-oxine reagent using a higher number of lymphocytes (1.4x10(9)) but the same total activity (around 55.5 MBq) resulted in improved labelled T cell viability and proliferative ability, although the mean labelling efficiency decreased (35.8%). Together the data suggest that 111In-oxine at low activity per cell is the most appropriate reagent for radiolabelling activated retargeted T lymphocytes useful for in vivo biodistribution studies.

Endosellar meningiomas: report of 2 cases and review of the literature.

Two cases of endosellar meningiomas are presented. The clinical and radiological picture is not always sufficiently specific to distinguish meningiomas from other types of sellar lesions, but pre-operative diagnosis is of fundamental importance in choosing the best surgical approach. In both cases, the transsphenoidal approach was used, and due to the dense consistency of the tumors, only biopsies were performed. One of the two patients was reoperated on using the sub-frontal approach for radical removal of the tumor. In the authors' experience, craniotomy proved to be the more favorable approach than the transsphenoidal route for radical excision of endosellar meningiomas. Literature on the classification of sellar meningiomas for radiological diagnosis and surgical strategy of endosellar meningiomas is reviewed.

Left movers' advantage in heartbeat discrimination: a replication and extension.

Lateral eye movers are subjects who consistently shift their gaze either leftward or rightward while reflecting on visuospatial or verbal questions. When reliably assessed, prevalent direction of gaze can be assumed as a valid index of hemispheric asymmetry. The present study evaluated discrimination of heartbeat (HB) and of spontaneous changes of finger temperature in 24 right-handed females reliably selected as lateral movers. Results replicate and extend previous findings by showing that left movers' superiority in HB discrimination is present also in females and concerns the perception of a different vascular function, that is, peripheral finger temperature. Differences between right movers and left movers were not due to activation level during the test or to body composition.

How uniform are post-graduate training programs in medical oncology in the European Union?

BACKGROUND: Medical oncology is a relatively new discipline whose rapid evolution is not paralleled by advances in the academic curricula. The present study was designed to provide a description of the status of post-graduate training programs in medical oncology in the European Union. RESULTS: Our survey shows that 'some form' of training in medical oncology is available in 10 countries of the European Union, but that there are officially recognized Schools of Specialization in this discipline in only four (Great Britain, Portugal, Republic of Ireland and Spain). Italy and France offer training programs in medical oncology as specific tracks of the School of Specialization in Oncology. A 'special competence' in medical oncology, within the specialty of internal medicine, is issued in 3 additional countries (Denmark, Belgium and the Netherlands). Finally, training in medical oncology is linked to that in haematology in Germany. A wide variation was found among the different European countries regarding the requirements for admission to these training programs. Similarly, the duration of the programs in the various schools ranges between 2 and 6 years and the total number of years from graduation to achievement of the title of specialist ranges from 4 to 9 years. CONCLUSIONS: Our survey discloses a broad heterogeneity of academic attitude regarding medical oncology in the European Union and points to the need for recognition of this discipline as a specific and essential specialization.

Prevalent direction of reflective lateral eye movements and ear asymmetries in a dichotic test of musical chords.

Subjects who consistently move their gaze either to the right or to the left while reflecting on visuo-spatial or verbal questions are usually called "lateral eye movers". This study evaluated auditory asymmetries to a dichotic test of musical chords in 23 right-handed females, selected through test-retest as reliable lateral eye movers; 12 were "left movers" (LMs) and 11 were "right movers" (RMs). During the assessment of the prevalent direction of gaze as well as during the dichotic test, the oculomotor activity was controlled through a video camera. The hypothesis was that the left ear advantage usually found with dichotic chords is enhanced in LMs and reduced in RMs, and that this effect is not due to the facilitating influence of lateral eye movements occurring during the task. Results show that: (a) left movers exhibit a marked advantage of the left ear while right movers do not exhibit any significant ear advantage; (b) despite the instruction to fix a central point, lateral movers tend to show unwarranted eye movements in their usual direction; (c) the effect of the prevalent direction of gaze on the dichotic advantage is not due to eye movements made during the dichotic test. These findings give further support to the hypothesis that the tendency to consistently shift the gaze to one side is related to hemispheric asymmetries as measured by lateralized tests.

Feedback and autonomic awareness: the ability to discriminate the direction of spontaneous changes in peripheral body temperature.

Forty-eight Ss were requested to discriminate spontaneous changes of at least 0.1 degree C in their peripheral finger temperature (PFT). They were randomly assigned to one of 3 groups: in the first group ("KR") Ss were given a feedback after each discrimination; in the second group ("NKR") Ss were given no feedback after each discrimination; in the third group ("RKR") Ss were given a random feedback after each discrimination. Results indicated that in the "Knowledge-of-Results" group discrimination was significantly higher than in both "No-Knowledge-of-Results" and "Random-Knowledge-of-Results" groups. Within the first group, there was a significant difference in a discrimination index between Ss "aware" and Ss "unaware" of their PFT changes, which remained significant even when the effects of individual variability of PFT and of the regularity of PFT changes were partialled out.

[Pulmonary carcinosarcoma].

Sarcomatoid carcinomas of the lung are uncommon neoplasms; the concurrent presence of malignant epithelial and sarcomatoid spindle cell components show a high malignancy. Lymph node metastases at presentation is an important prognostic factor, on the other way the most patients with sarcomatoid carcinoma of the lung usually presented at an advanced stage that needed a complementary therapy. The Authors report about one surgical case of pulmonary carcinosarcoma recently observed without lymph node metastases; the literature is reviewed and the histogenesis is discussed.

Study of the HLA-DP beta 1 locus by the polymerase chain reaction technique in patients with Hodgkin's disease.

BACKGROUND: A number of reports have studied associations between Hodgkin's disease and HLA. Some of them established correlation between several antigens and Hodgkin's disease, and others found no correlations. METHODS: The HLA DP locus was determined by the polymerase chain reaction method in 31 Hodgkin's disease patients and 58 healthy controls. RESULTS: No significant difference between patients and controls was noted. CONCLUSIONS: Further investigations are needed to confirm the hypothesis of a possible role of the HLA complex as one of the factors involved in Hodgkin's disease.

Cancer research in the European Community and other non-EC countries.

AIMS: We assessed the scientific productivity in the field of cancer research of countries in the European Community (EC) and the most important non-EC countries by counting the number of papers published during the period 1988-1990 in 15 of the top scientific journals (as defined by the Journal Citation Reports, Oncology). The global and per capita data are presented for each country. METHODS: Qualitative parameters like the impact factor and the half-life were also considered in the analysis. The selection of cancer journals with a high average quotation rate gave significant results. Introducing the average impact factor and half-life of each journal modified the results only slightly. The per capita data for each country were not corrected for the number of investigators working in oncology (a datum difficult to obtain). The parameters thus do not define the performance of the average investigator but are more "economic" parameters specifically related to the field of cancer research. RESULTS AND CONCLUSIONS: Small, wealthy Western countries tended to have an advantage over large, less developed countries, as expected. However, additional individual differences that could be of interest were present in the group of developed countries.

[Volvulus of the gallbladder].

Volvulus of the gallbladder is an uncommon process that is usually found at the time of exploration for an acute surgical abdomen. It results from congenital or acquired anatomical anomalies of the gallbladder. It may be complete or partial. This entity appears most often in the elderly with acute symptoms that may mimic a number of other more common abdominal emergencies. One case is report to help define the clinical and pathological features of this process.

Sister chromatid exchange analysis in familial groups of malignant melanoma patients.

Sister chromatid exchange (SCE) analysis was carried out on peripheral blood lymphocytes of 20 familial malignant melanoma (FMM) and 39 sporadic malignant melanoma (SMM) untreated patients, belonging to 10 and 39 families, respectively. The study was extended to 39 unaffected close relatives of FMM patients, to 187 unaffected close relatives of SMM patients, and to 20 unaffected unrelated individuals (control group), all examined under the same conditions. The mean SCE rates/cell were significantly higher in MM families than in the control group, and in melanoma patients than in their close relatives. The mean SCE levels of FMM and SMM patients, (8.4 +/- 0.8 and 8.0 +/- 0.3, respectively) were similar, and so were the distributions of individuals in classes of increasing SCE values (with a modal value at 7-8 SCEs/cell). The mean SCE levels of close relatives of FMM and SMM patients were also similar (5.4 +/- 0.2 and 5.4 +/- 0.1, respectively, with a modal value at 4-5 SCEs/cell), and slightly higher than in the control group (4.7 +/- 0.2 SCEs/cell). More than 7 SCEs/cell were observed in the majority (41 of 59) of FMM or SMM patients, in a smaller fraction (25 of 227) unaffected relatives, and in none of 20 unrelated unaffected individuals. These observations favor the hypothesis that higher SCE levels may be an expression of constitutional lesions predisposing to this neoplastic disease.

Molecular epidemiology of cancer in immune deficiency.

Molecular epidemiology may help fill the gap between epidemiological and biological models for cancerogenesis, allowing useful comparisons between series of cases carrying different biological characteristics. The defective regulation of immune responses is probably the common basis of cancer origin in genetically determined immune deficiency (GDID). Lymphomas are the most common neoplasms, showing an extremely high incidence in early age, frequent unusual location (extra-nodal) and histology, and rapid progression and spread with little response to therapy. A high incidence of lymphoma is also found in acquired (AIDS) or iatrogenic (transplant recipients) immune defective patients. The emergence of a malignant clone may be linked to unregulated polyclonal B- or T-cell proliferation and to disturbances in chromosomal rearrangements and in clonal selection, which are unique features of the immune cells regulation. It is useful to compare patterns of malignancies observed in GDIDs and in chromosomal breakage syndromes (CBS). In ataxia-teleangectasia (AT), selective errors at sites of special recombination involved in immune cell rearrangements may account for both immune deficiency and frequent types of malignant transformation. Different cytogenetic alterations and different types of malignancies are more common in Bloom syndrome, and in other GDIDs unrelated to chromosomal fragility, possibly due to different regulatory impairments. Viral infection (EBV, HPV, CMV) is likely to be a factor in any of the above steps. Therefore, individual exposure to viral (or other environmental) agents may be related to frequent location (skin, ano-genital areas, digestive tracts) of nonlymphatic cancers both in some GDIDs and in acquired or iatrogenic immune deficiencies. Cells that are homozygous for GDID are not malignant themselves, but are more likely to undergo new mutations to malignancy, due both to disregulation of the immune system and to environmental agents.

[Scientific institutions for treatment and care: purpose, characteristics and specific role in the oncologic sector].

The qualities of Comprehensive Health Care Institutes facilities established to foster, carry out and coordinate activities of scientific research and assistance in the field of biomedicine are described. At present there are 23 centers, their activity being based on two fundamental features: 1) topical research in specific themes characterizing the role these Institutes continuously play; 2) specialized research concerning subjects of particular interest and/or urgency suggested by the Ministry of Health.

[Leiomyosarcoma of the spermatic cord].

The case of leiomyosarcoma of the spermatic cord in a 54 year old man is reported. The patient was treated by surgical orchiepididymectomy and funnilectomy. After a review of the literature, the clinical and anatomopathological feature of this rare tumour and the surgical approach to it are discussed. The patient has had no recurrences and is still alive 16 months after surgery.
