DNA-cytophotometry of benign compound and intradermal naevi, Spitz epithelioid naevi and malignant melanomas.

Single cell DNA cytophotometry was used to characterize seven compound, ten intradermal and six Spitz naevi as well as 23 primary cutaneous malignant melanomas. Compound and intradermal naevi were characterized by a smaller nuclear area than both Spitz naevi and malignant melanomas. Tumour ploidy could not be used as a single criterion of malignancy since both diploid and hyperdiploid melanomas were encountered. The very low mean optical density of Spitz naevi served to distinguish these lesions from malignant melanomas.

Caryometry of benign compound acquired naevi, Spitz epitheloid naevi and malignant melanomas.

In order to get basic caryometric data later to be used in a prognostic study for malignant melanomas, caryometry of 7 compound acquired naevi, 8 Spitz epitheloid naevi and 83 cutaneous malignant melanomas was done and showed mean nuclear profile areas 32.9 microns 2, 73.6 microns 2 and 64.7 microns 2, respectively. Only minimal overlapping existed between the compound acquired naevi and the two other tumour types. Similar differences existed concerning mean maximal profile diameter, proportion of large nuclei and width of nuclear histograms. Mean form factor was lower for the malignant melanomas than for the naevi. The nuclei of the superficial layer of the compound naevi and the Spitz naevi were larger than those of the deep layer. A similar consistent pattern did not exist for the malignant melanomas.

Superfractionated irradiation combined with low doses of bleomycin in the treatment of oral carcinoma.

Seven patients with advanced oral carcinoma have been given 1 Gy per irradiation up to total dose 74 to 81 Gy. Bleomycin was given intramuscularly before irradiation. The therapeutic ratio of the proposed scheme was good. The early reactions were severe but were managed by a split course technique. No late complication occurred. All patients were tumour-free 18 to 54 months after treatment. A relapse occurred only in one case. The results are not conclusive, but bearing in mind that the results in advanced oral carcinoma are poor they suggest that the proposed treatment regime may be promising.

Analysis of therapy-resistant venous leg ulcers. Can triple-layer treatment initiate healing?

Chronic leg ulcers represent a major health-care problem with considerable socioeconomic impact. Patients with seemingly therapy-resistant leg ulcers are common to all clinics. The purpose of the present study was to (1) examine a group of patients with nonhealing venous leg ulcers treated with a double-layer bandage and (2) evaluate whether the addition of an interactive hydrocolloid wound dressing could initiate healing in these patients. Twenty-two patients with ulcers caused by venous insufficiency were included. The patients had a mean ulcer duration of 27.6 years. Duration of the present ulcer was at least 1 year (mean >/= 4.1 years). Twenty of the 22 patients showed massive lipodermatosclerosis. Before inclusion, all patients had used double-layer bandage consisting of a zinc-impregnated bandage or stocking and a self-adhesive compression bandage for 1 year or longer without improvement. The new regimen was a triple-layer treatment with the hydrocolloid water applied over the ulcer and the traditional double-layer bandage unchanged. Three patients were dropped from the study. Nineteen patients were followed until healing or for 10 months. Nine of the 19 patients who completed the study healed. Ulcer area was reduced by 70% or greater in 7 patients and by 30% to 40% in two patients. One ulcer did not respond to the treatment and worsened slightly. The results of this study were encouraging and indicate that the triple-layer treatment with the hydrocolloid dressing applied to the ulcer should be evaluated in a randomized, controlled study in patients with less pessimistic prognoses.

DNA cytophotometry of acinic cell carcinoma and its relation to prognosis.

Single-cell DNA cytophotometry has been assessed on histological sections or aspiration biopsy smears from 13 parotid acinic cell carcinomas and has been related to prognosis. The minimal follow-up was 6 years. Six ACC were well differentiated and seven moderately well differentiated. The modal DNA values of all tumours examined were in the diploid area. The number of nuclei above 2.5C DNA, expressing proliferative ability or aneuploidy, was low. A difference, too small to be of value for prognostic purposes, was found between the non-recurrent tumours (6%) and the metastasizing tumours (16%). The DNA levels measured in the present tumour material correspond to the concept of ACC as being a well differentiated neoplasm. The nuclear DNA pattern of these tumours would seem to be of minor prognostic significance with regard to survival time and/or potential for metastasis.

Leg ulcer treatment in hospital and primary care in Sweden: cost-effective care and quality of life.

Because of significant reductions in the Swedish health care budget, the trend in the Swedish health care service is to move patients with leg ulcers out of hospitals and into primary care or community home care. This study compares the costs for a 6-week treatment of patients with leg ulcers in primary care. Subjects were randomly assigned to one of the two treatment groups--wet-to-dry saline gauze dressings changed twice a day or a hydrocolloid dressing (HCD) DuoDERM changed when needed or once a week. A variety of costs, including travel costs for the nurses, time for dressing changes, and material (including ancillary items) were compared.
