Acute sternal opening after cardiac surgery in children: effect on plasma atrial natriuretic peptide.

Plasma atrial natriuretic peptide (ANP) was measured immediately before and 15 minutes after re-opening of the sternotomy and delayed sternal closure were performed to alleviate possible myocardial compression in 3 children. As acute re-opening of the sternotomy is a rare occurrence our sample size was necessarily small. ANP increased following the procedure in all 3 patients, and this was significant (p less than 0.02) when compared to plasma levels obtained from 9 children who underwent sternotomy prior to elective cardiac surgery. Following acute re-opening of the sternotomy there was an increase in median urine output and a decrease in median central venous pressure. Our results illustrate that it is atrial wall stress rather than intra-atrial pressure that determines release of ANP, and in addition suggest ANP may be a factor in the rapid clinical improvement following delayed sternal closure for atypical tamponade in children.

Beta-adrenoceptor subtypes in human, rat, guinea pig, and rabbit atria.

Selective beta-adrenoceptor agonists and antagonists were used to characterize the beta-adrenoceptor subtypes in human right atrium. The activity order of selective agonists for rat, guinea pig, and rabbit atrial inotropic and chronotropic responses was consistent with the hypothesis that only beta 1-adrenoceptors mediate these responses. In human right atrium, however, the beta 2-selective agonists fenoterol and salbutamol were relatively much more active. The beta 1-selective antagonist practolol preferentially antagonized the human atrial response to noradrenaline (beta 1-selective) as opposed to fenoterol (beta 2-selective), whereas the beta 2-selective antagonist ICI 118,551 preferentially antagonized the response to fenoterol. The data indicate that the response of human right atrium to beta-adrenoceptor agonists is mediated by a mixed beta 1-plus beta 2-adrenoceptor population. The implications of these results with respect to the therapeutic uses of beta-adrenoceptor agonists are discussed.

Continuous monitoring of pulmonary artery pressure after cardiac surgery in infants and children.

Increases in pulmonary vascular resistance in the post-operative period are becoming more widely recognised as a cause of sudden clinical deterioration and death after the correction of congenital heart disease. Therefore pulmonary artery pressure was continuously monitored in the post-operative period in 45 selected infants and children who had undergone surgery for congenital heart disease by inserting an 18 gauge intravenous catheter into the main pulmonary artery through the right ventricular infundibulum at the time of operation. Episodes of low cardiac output which were accompanied by significant increases in PAP/AOP ratio occurred in 20 patients and were managed by alteration in ventilation and/or administration of pulmonary vasodilators. There was a small but significant morbidity associated with the procedure. It should therefore be reserved for those cases considered to be at risk of developing episodes of increased pulmonary vascular resistance.
