[Wirsung's duct and chronic pancreatitis. Changes in the morphology of the pancreatic ducts after pancreatic duct shunts. 23 cases followed-up].

The authors study in chronic pancreatitis the morphology of Wirsung's duct in 31 patients who had undergone repeated operations, 23 of them were submitted in a first stage to an anastomosis between the pancreatic duct and the digestive tract. The main causes of failure were obstructions of the anastomosis, biliary complications and continuation of the pancreatic disease. The difference in prognosis between pancreatitis with a dilated pancreatic duct, and those with a filiform duct, is perhaps due to lesions of different histological appearance and course. The best results were obtained in patients able to give up alcohol and in whom it was possible to carry out a broader anastomosis on a dilated and unobstructed pancreatic duct.

[Pancreatic biologic reactions after Wirsung opacification under duodenoscopy. Preventive action of Frey's inhibitor (67 cases)].

After retrograde opacification of be pancreatic ducts by duodenoscopy, pancreatic biologic reactions have been studied, especially by amylasemia and lipasemia measures. These biologic reactions have been all the more frequent and intense as Wirsung's duct and its tributaries filling have been important. Frey's inhibitor had a proptious action on these biologic reactions; therefore, it may be used as a preventive treatment.

[Chronic familial pancreatitis (6 cases, 3 families)].

The authors report 6 cases of chronic familial pancreatitis. They review the literature on this disease which is still little recognised and seek the specific characteristics, early onset, transmission as an autosomic dominant with incomplete penetrance, relatively favourable prognosis, together with the suggestive signs, pancreatic lithiasis and certain complications. Apart from these signs, chronic familial pancreatitis differs very little from common chronic pancreatitis. The only cause is apparently genetic. Etiopathogenesis is thus still obscure. Treatment should be started early, but surgical methods are the same as in primary chronic pancreatitis.
