Decrease of intestinal P-glycoprotein activity by 2n-propylquinoline, a new oral treatment for visceral leishmaniasis.

Drugs currently available for visceral leishmaniasis treatment are potentially toxic, have to be administered by parenteral route and frequently give rise to drug resistance, due to the involvement of P-glycoproteins (P-gp) in Leishmania. The purpose of this study was to investigate a possible inhibitory effect of 2n-propylquinoline (2nPQ) on P-gp activity. 2nPQ is a new oral anti-leishmanial drug that has demonstrated its efficacy in BALB/c infected mice with Leishmania donovani [Antimicrob. Agents Chemother. 37 (1993) 859]. Rat everted gut sacs and human intestinal Caco-2 cell lines were used to study the effect of 2nPQ on P-gp activity. Our results demonstrate an inhibitory effect of 2nPQ on the P-gp activity with two P-gp substrates (rhodamine 123 and digoxin), two P-gp inhibitors (cyclosporin A and verapamil), and in two different species. Alone or associated with other active drugs, 2nPQ would be very useful to control Leishmania Multi-Drug-Resistance and intestinal P-gp in humans with kala-azar.

Comparison of two special infant formulas designed for the treatment of protracted diarrhea.

The nutritional management of intractable diarrhea and short bowel syndrome remains a challenging problem. The advantage of continuous nasogastric infusion is undisputed, but what to feed remains in question, and no studies, to these authors' knowledge, have yet compared two widely used specially designed protein hydrolysate formulas. A randomized crossover trial of two periods of 7 days was carried out with Alfare and Pregestimil administered by a constant infusion pump in six malnourished infants aged 1-13 months. Two had intractable diarrhea, and four the short bowel syndrome. Identical quantities of calories were administered during the two periods. There was good tolerance for both formulas and satisfactory weight gain. Despite compositional differences related to osmolality, the source of the hydrolysates and their profile, and the qualitative pattern of the carbohydrates, no difference was observed with regard to stool weight, Na+ and K+ losses, and the enteral absorption of fat, carbohydrate-derived energy, and total energy. The percent absorption of nitrogen was somewhat higher (p less than 0.01) with Pregestimil (83.8 +/- 2.7) than with Alfare (77.3 +/- 3.4), but nitrogen retention was unaffected. Energy absorption was the same on both formulas, but maldigested or malabsorbed carbohydrates accounted for 63% of total energy loss during Alfare feeding and 72% during the week of Pregestimil. These data call for studies with protein hydrolysate formulas reformulated with a lower concentration of carbohydrates and a higher one of fat.

Efficacy and tolerability of the ifosfamide-epirubicin combination in relapsed ovarian cancer.

A retrospective study evaluating the efficacy and tolerability of epirubicin-ifosfamide (EI) in patients with relapsed advanced ovarian cancer (ROC) after prior chemotherapy was conducted. A total of 93 patients received epirubicin (50 mg/m(2), day 1), ifosfamide (1500 or 2500 mg/m(2), days 1-3), and mesna monthly. Thirty-five percent had received one line of chemotherapy (platinum 100%, taxanes 8%); 38%, two lines; and 27%, more than two lines. Fifty-three percent received 2500 mg/m(2)/day ifosfamide and 47% received 1500 mg/m(2)/day ifosfamide. Ifosfamide was administered by continuous infusion in 12 patients. Mean number of courses was 4 (1-12). Grade 4 toxicity was 69% neutropenia and 12% thrombocytopenia. Three patients on high-dose ifosfamide as a short infusion had central nervous system dysfunction resulting in death. There were 84 assessable patients: 7 (8%), complete responses; 13 (15%), partial responses; and 20 (24%), stable disease. Median time to progression was 5 months (3 days to 36 months). The EI combination appears to be effective in ROC. However, toxicity with high-dose ifosfamide administered by short infusion is not acceptable. Tolerability can be improved using ifosfamide at 1500 mg/m(2) by continuous infusion. The combination of ifosfamide with newer anthracycline agents such as liposomal doxorubicin may be an alternative and needs further evaluation for use after first-line taxane-based chemotherapy.

[Sleep disorders in patients with personality disorders].

63 patients with personality disorders have been hospitalized in a Sleep Disorder Clinic. Sleep disorders usually started during childhood, disappeared ant the reappeared around the age of 25. The polysomnographic data do not show a specific profile. Most of the patients were of the schizotypal or the borderline type. Therapeutic guidelines are presented.

[Proposal for a new descriptive psycho-physiopathological model of schizophrenia].

We present the conclusions of a study of pattern recognition in an intermittent luminous stimulation. This stimulation was stable on the one hand and on the other hand on fixed time basis (S.L.I. pulsations emitted by two flickers) and other associated tests (reaction time, Rorschach Test, etc). We have compared a population of 30 schizophrenics (French classification) and a reference group of 53 adult subjects of both sexes. We have not divided the patients into subclasses according to their symptoms. We have noticed in a significant manner the following signs: great vigilance at the beginning, decreasing very quickly, low attentiveness, a slowness of perception and motion, a weakness in the elaboration of decision processus, certain difficulties in defining the classification of objects, a modified perception of colours, a very feeble distinction of pertinent signals and of noise, an invasion by the internal stimulus, unbalanced compared to "outside", as in sensory deprivation, a great weakness in the processus of habituation and learning, a loss of the redundancy and the constancy of the outside world (or troubles of the internal coherency, as defined by Varela). The closure, the temporal troubles (historical and present) of the integration of signals are sufficient to explain these results. A model explains the deduced hypothesis on two levels: 1) historical: the troubles of habituation and learning prevent the formation of the inner stimulus (or image, representation); 2) present: closure, ambivalence (Gödel), troubles of associations, difficulties in detecting what is pertinent, hallucinations (a "delirious" internal stimulus). The specific brain-channels seem to be normal, on the contrary the non-specific channels and the channels of integration seem troubled. The temporal trouble of "present" seems to be located on a precocious precategorical iconic level. This descriptive model does not prejudge the etiology of the disease (bibliography).
