[Tamponade due to Porphyromonas gingivalis].

A 45 year old man, 10 weeks after angioplasty and endoprosthesis insertion for myocardial infarction, developed tamponade in an infectious context. RNA 16S investigation, using the universal amplification method with ribotyping, detected Porphyromonas gingivalis (a strict anaerobic bacillus associated with peridontitis) in the pericardial fluid. This appears to be the first description of such a clinical scenario, and suggests 5 aetio-pathological mechanisms: fissure syndrome, malignant tamponade, post-infarction pericardial syndrome, endoprosthesis infection and infectious tamponade. We also describe the ribotyping method, which by amplifying DNA coding for RNA 16S allows identification of an increasing number of micro-organisms implicated in infectious pathology.

Cardiac resynchronisation therapy in chronic atrial fibrillation: impact on left atrial size and reversal to sinus rhythm.

OBJECTIVE: To evaluate the impact of long term cardiac resynchronisation therapy (CRT) on left atrial and left ventricular (LV) reverse remodelling and reversal to sinus rhythm (SR) in patients with heart failure with atrial fibrillation (AF). PATIENTS: 74 consecutive patients (age 68 (8) years; 67 men) with advanced heart failure and AF (20 persistent and 54 permanent) were implanted with a CRT device. MAIN OUTCOME MEASURES: Patients were evaluated clinically (New York Heart Association (NYHA) class, quality of life, six minute walk test) and echocardiographically (LV ejection fraction, LV diameters, and left atrial diameters) before and after six months of CRT. Additionally, restoration of SR was evaluated after six months of CRT. RESULTS: NYHA class, quality of life score, six minute walk test, and LV ejection fraction had improved significantly after six months of CRT. In addition, left atrial and LV end diastolic and end systolic diameters had decreased from 59 (9) to 55 (9) mm, from 72 (10) to 67 (10) mm, and from 61 (11) to 56 (11) mm, respectively (all p < 0.01). During implantation 18 of 20 (90%) patients with persistent AF were cardioverted to SR. At follow up 13 of 18 (72%) patients had returned to AF and none had spontaneously reverted to SR; thus, only 5 of 74 (7%) were in SR. CONCLUSION: Six months of CRT resulted in significant clinical benefit with significant left atrial and LV reverse remodelling. Despite these beneficial effects, 93% of patients had not reverted to SR.

[Cardiac resynchronisation therapy: what kind of equipment to use?].

Cardiac resynchronization therapy is indicated in advanced heart failure refractory to optimal drug treatment patients with left ventricular systolic dysfunction and QRS >120 milliseconds. The choice of the device has to consider several parameters: Do we have to implant a CRT pacemaker or a intracardiac cardioverter defibrillator (ICD)? The prevalence of sudden cardiac death is high in heart failure patients. In patients with an ischemic cardiomyopathy, primary prevention of sudden cardiac death trials suggests to implant a biventricular ICD. In patients with a non ischemic cardiomyopathy, the question is more controversial althought the resullts of the SCD-HeFT and COMPANION trials yielded interesting results for iCD implantation. However, the final decision has to consider the patient's baseline characteristics such as age, presence of comorbidities and cost of the device. Today, devices with totally independent ports of the right and left ventricles have technical advantages and thus are more relevant. Cardiac resynchronization therapy is a heart failure treatment and the new devices provide new tools to assess heart failure parameters such as patient's activity, respiratory parameters or heart rate variability. Left ventricular pacing alone is currently under evaluation such as atrial fibrillation prevention algorithms, atrial fibrillation being frequent in herta failure patients with hemodynamic deleterious consequences.

Assessment of dietary intake of flavouring substances within the procedure for their safety evaluation: advantages and limitations of estimates obtained by means of a per capita method.

The procedure for the safety evaluation of flavourings adopted by the European Commission in order to establish a positive list of these substances is a stepwise approach which was developed by the Joint FAO/WHO Expert Committee on Food Additives (JECFA) and amended by the Scientific Committee on Food. Within this procedure, a per capita amount based on industrial poundage data of flavourings, is calculated to estimate the dietary intake by means of the maximised survey-derived daily intake (MSDI) method. This paper reviews the MSDI method in order to check if it can provide conservative intake estimates as needed at the first steps of a stepwise procedure. Scientific papers and opinions dealing with the MSDI method were reviewed. Concentration levels reported by the industry were compared with estimates obtained with the MSDI method. It appeared that, in some cases, these estimates could be orders of magnitude (up to 5) lower than those calculated considering concentration levels provided by the industry and regular consumption of flavoured foods and beverages. A critical review of two studies which had been used to support the statement that MSDI is a conservative method for assessing exposure to flavourings among high consumers was performed. Special attention was given to the factors that affect exposure at high percentiles, such as brand loyalty and portion sizes. It is concluded that these studies may not be suitable to validate the MSDI method used to assess intakes of flavours by European consumers due to shortcomings in the assumptions made and in the data used. Exposure assessment is an essential component of risk assessment. The present paper suggests that the MSDI method is not sufficiently conservative. There is therefore a clear need for either using an alternative method to estimate exposure to flavourings in the procedure or for limiting intakes to the levels at which the safety was assessed.

[Surgical rehabilitation procedures of the upper limbs in tetraplegic patients].

Cervical spinal cord injuries represent about 40% of all spinal cord lesions. In addition to the complete paralysis of the lower limbs they induce a dramatic decrease of upper limb function. Surgical rehabilitation of the upper limbs, based mainly on tendon transposition procedures, was advocated by only few surgeons more than thirty years ago and has developed in recent years in view of their satisfactory results. We report a consecutive series of 69 tetraplegic patients operated during the past twelve years, amounting to 188 rehabilitation procedures on their upper limbs. Our results indicate that surgery was beneficial in all cases. There was no case of functional loss and the patients were mostly satisfied with the procedures.

Dietary exposure to chemicals within the process of risk assessment: possible applications to substances that may cause allergic reactions.

Exposure assessment is one of the key parts of the risk assessment process. This task is crucial when evaluating substances for which only intake of toxicologically-important amounts can lead to adverse health effects. Ideally, dietary exposure to hazardous substances can be assessed by combining data on concentration in all food products with data on their consumption. However, it is considered to be neither cost-effective nor necessary to collect detailed data for every substance, and a stepwise procedure is commonly used to focus resources on the most important issues. Screening methods, designed to look for 'worst case' situations, are first used to target chemicals that might be of health concern for the general population or for certain at-risk groups. The quality of the dietary exposure assessments not only depends on the quality of the data collected, but also on the integration tools used for initial screening or for the eventual more precise estimations. A particular challenge is the evaluation of food allergens and components causing other forms of intolerances, since no reliable data seem to be currently available on the type of exposure (amounts and duration) required to induce a food allergy. A different approach from that used for dietary exposure to other hazardous substances has to be adopted. However, the methodologies (such as those used to collect food consumption data) and databases (in particular, information about food labels) developed in such a context could be useful to investigate the exposure conditions leading to the development of food allergies.

Valve replacement in patients with critical aortic stenosis and depressed left ventricular function: predictors of operative risk, left ventricular function recovery, and long term outcome.

OBJECTIVES: To identify predictors of operative and postoperative mortality and of functional reversibility after aortic valve replacement (AVR) in patients with aortic stenosis (AS) and severe left ventricular (LV) systolic dysfunction. METHODS AND RESULTS: Between 1990 and 2000, 155 consecutive patients (mean (SD) age 72 (9) years) in New York Heart Association (NYHA) heart failure functional class III or IV (n = 138) and with LV ejection fraction (LVEF) < or = 30% underwent AVR for critical AS (mean (SD) valve area index 0.35 (0.09) cm2/m2). Thirty day mortality was 12%. NYHA class (3.7 (0.6) v 3.2 (0.7), p = 0.004), cardiothoracic ratio (CTR) (0.63 (0.07) v 0.56 (0.06), p < 0.0001), pulmonary artery systolic pressure (63 (25) v 50 (19) mm Hg, p = 0.03), and prevalence of complete left bundle branch block (22% v 8%, p = 0.03) and of renal insufficiency (p = 0.001) were significantly higher in 18 non-survivors than in 137 survivors. In multivariate analysis, the only independent predictor of operative mortality was a CTR > or = 0.6 (odds ratio (OR) 12.2, 95% confidence interval (CI) 5.4 to 27.4, p = 0.002). The difference between preoperative and immediate postoperative LVEF (early-DeltaEF) was > 10 ejection fraction units (EFU) in 55 survivors. In multivariate analysis, CTR (OR 5.95, 95% CI 3.0 to 11.6, p = 0.006) and mean transaortic gradient (OR 1.05, 95% CI 1.0 to 1.1, p < 0.05) were independent predictors of an early-DeltaEF > 10 EFU. During a mean (SD) follow up of 4.6 (3) years, 50 of 137 (36%) 30 day survivors died, 31 of non-cardiac causes. Diabetes (OR 3.8, 95% CI 2.4 to 6.0, p = 0.003), age > or = 75 years (OR 2.6, 95% CI 2.1 to 4.5, p = 0.004), and early-DeltaEF < or = 10 EFU (OR 0.96, 95% CI 0.94 to 0.97, p = 0.01) were independent predictors of long term mortality. Among 127 survivors, the percentage of patients in NYHA functional class III or IV decreased from 89% preoperatively to 3% at one year. The decrease in functional class was significantly greater in patients with an early-DeltaEF > 10 EFU than patients with an early-DeltaEF < or = 10 EFU (p = 0.02). In addition, the mean (SD) LVEF at one year was 53 (11)% in patients with an early-DeltaEF > 10 EFU and 42 (11)% in patients with early-DeltaEF < or = 10 EFU (p < 0.001). CONCLUSIONS: Despite a relatively high operative mortality, AVR for AS and severely depressed LVEF was beneficial in the majority of patients. Early postoperative recovery of LV function was associated with significantly greater relief of symptoms and longer survival.

[Use of the six-minute walk test in cardiology].

The symptom-limited exercise test is nowadays the gold standard to assess the exercise tolerance and the effects of different treatments in patients with chronic heart failure (CHF). A simpler alternative to this test is the 6-minute walk test. The 6-minute walk test is easy to realize and well-tolerated. Moreover, it is reliable just after one familiarization practice and requires standardization. Indeed, its conduction, which is submitted to some security precautions, can be altered by variation factors. The distance walked during the 6 minutes was the only parameter studied during the test. This parameter could allow judging the CHF severity and prognostic. The analyse of cardiorespiratory parameters has shown that the 6-minute walk test relative intensity is near to the peak individual values. Moreover, the cardiac and ventilatory adaptation of patient during this test depends to his own functional capacity. Lastly, the 6-minute walk test is a submaximal constant-load exercise, which should be performed in complement to the symptom-limited exercise test in cardiac patients.

Comparative effects of ventricular resynchronization therapy in heart failure patients with or without coronary artery disease.

UNLABELLED: In patients with advanced heart failure, intraventricular conduction delay (IVCD) and left ventricular systolic dysfunction (LVSD), multisite cardiac pacing can be proposed as an additive treatment. The aim of this study was to assess the clinical effectiveness of atrioventricular pacing according to the etiology of LVSD, by comparing the outcome of patients with and without coronary artery disease. Between August 1997 and November 1998, 103 patients were included in the InSync trial and received a biventricular pacemaker and a specifically designed left ventricular pacing lead. Baseline evaluation (12 lead ECG, New York Heart Association Class, quality of life (QOL) and distance walked during the 6 min walk test) was repeated in survival patients at 1, 3, 6 and 12 months after pacemaker implantation. Patients were split in two groups, ischemic (N = 48) and non-ischemic (N = 55), according the result of a recent coronary angiography, the existence of coronary angioplasty or coronary artery bypass or the history of a prior myocardial infarction. RESULTS: The mortality rate was similar in the two groups with a mean 12 months actuarial survival rate of 78%. Nevertheless, the delay between the death and the pacemaker implantation was significantly higher in the non-ischemic group. A significant reduction in QRS duration and a significant improvement in NYHA class (-1.5). QOL score (-50%) and 6 min walking test (+18%) were observed similarly in the two groups. CONCLUSION: This study shows that biventricular pacing improves significantly functional status of patients with LVSD, IVCD and advanced heart failure, regardless the etiology of the cardiomyopathy, ischemic or not, without over-mortality in ischemic patients.

Dietary estimated intake of intense sweeteners by Italian teenagers. Present levels and projections derived from the INRAN-RM-2001 food survey.

In a previous study, Italian female teenagers regular consumers of sugar free soft drinks and table-top sweeteners were suggested to have a higher intake of intense sweeteners than other teenagers. A food frequency questionnaire designed to identify adolescents who were high consumers of these food products was filled in by a randomly extracted sample of teenagers (n=3982) living in the District of Rome (Italy) in year 2000. A consumer survey was then carried out in a randomly extracted sub-sample of males and females and in all females who reported high consumption of sugar-free soft drinks and/or table-top sweeteners. A total of 362 subjects participated in a detailed food survey by recording, at brand level, all foods and beverages ingested over 12 days. For each sugar-free product, producers provided the concentration of intense sweeteners (saccharin, aspartame, acesulfame K and cyclamate). No intake in excess of the Acceptable Daily Intake (ADI) was observed. Also medicines and supplements were taken into account and these did not result in a large impact on chronic exposure to intense sweeteners. The intake levels did not exceed the ADI even under a worst case scenario which was performed to take into consideration a hypothetical future substitution of all regular food products with their sugar-free version. It can be concluded that, with the observed current consumption patterns and occurrence levels, the risk of an excessive intake of intense sweeteners by Italian teenagers is extremely low.

[Prognosis of post-irradiation soft-tissue sarcoma: case-control study].

PURPOSE: We describe clinical and pathological aspects and survival of 12 recent of radio-induced soft tissue sarcoma. METHOD: We performed a single center case-control study, with 23 control matched by grade (according to Federation nationale des centres de lutte contre le cancer Grading) and by location. Survival comparison used Log Rank test. RESULTS: The development of these 12 cases was a result of 12 previous cancer treatment (including 4 breast cancers and 2 cervix cancers) with radiotherapy (median dose of 58 Grays). The median latency period was 10 years. There were nine women and three men. The median age was 68.5 years. The main location was chest wall (5 cases). The two main histologic subtypes were malignant histiocytofibromas (6 cases) and angiosarcomas (3 cases). Five cases were high grade. Treatments were performed with curative intent in 9 cases, including complete resection in six cases. Clinical and pathological aspects and treatments of controls were similar, but postoperative radiotherapy was most frequent and malignant histiocytofibroma was less common. Median overall survival (47 months vs. 30 months, P = 0.6), median metastatic disease-free survival (37 vs. 15 m., P = 0.6) and median locoregional disease-free survival (14 vs. 24 m., P = 0.07) were similar for cases and control. CONCLUSION: This study suggests that prognosis of radio-induced soft-tissue sarcomas is similar as other soft tissue sarcoma of same grade and same location.

Beliefs and attitudes towards the consumption of sugar-free products in a sample of Italian adolescents.

OBJECTIVE: The objective of this study was to investigate beliefs and attitudes towards sugar-free products in a sample of Italian teenagers. DESIGN: A total of 10 secondary public schools in the county of Rome were randomly selected. The probability of being extracted was set proportional to the number of students. A random sample of teenagers was selected from the schools for participating in a food consumption survey. SUBJECTS: The sample included 233 adolescents, aged 15-21 y. A total of 224 adolescents agreed to participate in the study aimed at assessing beliefs and attitudes. INTERVENTIONS: Two questionnaires were completed at the first day of the last period of the 4-day food survey. The first questionnaire contained questions on attitudes and beliefs toward a healthy diet. The second questionnaire, aimed at assessing adolescents' attitudes towards the consumption of sugar-free products, was based in part on the social-psychological model of the Theory of Planned Behaviour. RESULTS: In general, the adolescents of this sample seemed to be quite uncertain in their responses. On average, they did not express an emphatic point of view with regard to the act of eating sugar-free products. The results of the assessment of relationships between the variables of the theoretical model revealed the lack of causal links among beliefs, attitudes, intention and consumption of sugar-free products. Perceived behavioural control was not a significant predictor of intention. CONCLUSIONS: The target of further studies aimed at investigating attitudes towards sugar-free products should be tailored according to the quantity of consumption of such products in order to represent a sample of frequent and high consumers of sugar-free products. Food consumption and nutrient intake in a sample of Italian secondary school students: results from the INRAN-RM-2001 food survey.

The present study was designed to estimate the average food and nutrient intake in a sample of Italian students living in the District of Rome. In the school year 1999-2000 all students belonging to the first, second or third year of nine randomly selected secondary public schools were contacted for filling in a food frequency questionnaire (3982 subjects). The next year, 233 of them (125 males and 108 females), randomly extracted, participated in a detailed food survey. Food intake was assessed on the basis of 12 (four consecutive days in three different periods) 24-h dietary records. The average daily intakes of fruit (107 +/- 100 g/day), vegetables (186 +/- 74 g/day) and legumes (16 +/- 29 g/day) were lower than recommended. The percentage of energy intake from fat (39%) and that from saturated fats (12%) were high. On the other hand, the percentage of energy intake from carbohydrates was low (45%) even if that from sugar (14%) was rather high. The students' diet was characterized by low intakes of fibre (16 +/- 6 g/day) and calcium (710 +/- 247 mg/day) in both males and females. Iron intakes were lower than 12 mg/day in a very high percentage of females (80%), suggesting that iron deficiency may be present in this population group. Results indicate that the diet of these students is both unbalanced in terms of macronutrients and deficient for some micronutrients, suggesting the need for intervention.

Heterotopic ossification of the elbow in patients with burns. Results after early excision.

Heterotopic ossification which may develop around the elbow in patients with burns may lead to severe functional impairment. We describe the outcome of early excision of such heterotopic ossification in 28 patients (35 elbows), undertaken as soon as the patient's general and local condition allowed. The mean age at operation was 42 years. The mean area of burnt body surface was 49%. The mean pre-operative range of movement was 22 degrees in flexion/extension and 94 degrees in pronation/supination. The mean time between the burn and operation was 12 months with the median being 9.5. The mean follow-up period was for 21 months. At the last review, the mean range of movement was 123 degrees in flexion/extension and 160 degrees in pronation/supination. Clinical evidence of recurrence was seen in four patients, occurring within the first two months after operation. Nevertheless, three of these elbows gained 60 degrees or more in flexion/extension and in pronation/supination. Based on this experience, we recommend early surgical treatment of heterotopic ossification of the elbow in patients with severe burns.

[Role of cardiac resynchronisation in the treatment of cardiac failure].

For 10 years cardiac resynchronisation has seen considerable development, as much on the technological side as at the level of its scientific validation. Several prospective studies have shown the functional benefits of cardiac resynchronisation in a selected population of refractory cardiac failure patients with improvement in symptoms, exercise tolerance and quality of life. Equally, cardiac resynchronisation allows a significant reduction in hospital episodes for cardiac failure and also has a beneficial effect on left ventricular inverse remodelling. Finally, the first results of morbidity/mortality trials are very encouraging with a significant reduction in overall mortality at one year in the COMPANION study with the biventricular defibrillator. However, numerous important, unresolved questions remain such as the problem of non-responders and thus patient selection, or such as the place of cardiac resynchronisation in patients with permanent atrial fibrillation. The choice of the type of implantable prosthesis (pacemaker or biventricular defibrillator) and the choice of the mode of pacing (biventricular or solely left ventricular) are still under discussion. New indications for cardiac resynchronisation could be seen next, for example such as the optimisation of stimulation mode in already paced patients or "systematic" biventricular pacing in patients with a conventional indication for pacing..... In 2004, cardiac resynchronisation must be considered as an effective adjuvant therapy in cardiac failure patients refractory to optimal medical treatment with left ventricular dysfunction and intraventricular conduction disorders.

[Functional surgery of upper limb in tetraplegics since 50 years].

OBJECTIVE: Development of upper limb functional surgery in tetraplegics in the last 50 years. METHODS: The literature review relating to the years 1950-2002 was carried out with 3 data bases: Medline, Pascal, Embase. This review also involved a thorough study of non-indexed references. RESULTS: A large number of surgical procedures are described. Two priorities are stressed by the authors: safety of these procedures and duration of postoperative immobilization. CONCLUSION: This review of literature shows that the prospects for restoring upper limb function in tetraplegics are greater than ever, offering a larger number of patients the possibility to increase their independence in daily life. Functional surgery remains, nevertheless, demanding in terms of length of immobilization and presupposes requiring a multidisciplinary approach requiring rehabilitation teams to be up to date with surgical procedures.
