Whole serum complement (CH50) and C3, C4, and C3PA plasma values were studied in 48 patients: 9 with nonseptic shock; 20 with sepsis; 14 with septic shock caused by gram-negative bacteria; 5 with septic shock caused by gram-positive bacteria. All were compared with a control group of 25 healthy individuals. Determinations were made upon admission and again 48 and 96 h later. No significant differences in complement values were found between the patients with nonseptic shock and the control group. In the patients with sepsis, decreased CH50 (p less than 0.001) and increased C3PA (p less than 0.02) values were observed, while C3 and C4 remained unaltered. In the patients with septic shock, markedly decreased levels of CH50, C3, and C4 were seen (p less than 0.001, and p less than 0.001, and p less than 0.001, respectively) without changes in C3PA levels. There were no differences between septic shock due to gram-negative and gram-positive bacteria, or between patients who died and those who survived. After 96 h, the altered values returned to the normal range. This underlines the transitory activation of the complement system through the classic pathway and suggests its possible role in the pathogenesis of septic shock in man.
The case of a 26-year-old woman with cardiac and pulmonary echinococcosis is presented. Surgical treatment of the intrathoracic disease was done in a one-stage operation. Under cardiopulmonary bypass, a left ventricular hydatid cyst and a lower lobe pulmonary cyst were removed. The patient is completely asymptomatic and is leading a normal life two years after operation.
With the purpose of investigating if placental prolactin production had an influence on maternal prolactin serum concentrations, this hormone was quantitated in serum of a women with partial Sheehan syndrome (deficient gonadotrophin and prolactin production). Her pregnancy was induced by pituitary gonadotrophins and the deficient prolactin pituitary reserve was shown by means of pituitary stimulation test with metoclopramide of pituitary stimulation test with metoclopramide and TRH. Quantitative analysis of prolactin was made by RIA utilizing the double antibody method. Serum prolactin concentrations remained persistently low during pregnancy with a minimal increase (1 - 16 ng/ml) at the end of pregnancy when comparing with concentrations observed in normal pregnant women (20 +/- 10 ng/ml at the beginning of pregnancy, and 250 +/- 150 ng/ml mean +/- 3 SD at the end). Prolactin concentration in the umbilical cord was found to be within normal values (252 ng/ml). Results obtained showed that fetal and placental prolactin does not pass on to the mother and confirm the idea that maternal serum prolactin during pregnancy is exclusively of a maternal pituitary origin.

The herbicide paraquat is considered safe by industry and the bulk of regulators worldwide. However, determinants of exposure from 30 years ago persist in developing countries. Little is known about systemic absorption from occupational exposures. The relationships between exposure determinants, levels of external exposure, biomarkers of exposure, and outcomes are not clear. High rates of severe acute poisonings have been documented. In addition, topical injuries occur in as many as 50% of exposed workers. Non-worker populations are also at risk, particularly children. Long-term and delayed health effects include Parkinson's disease, lung effects, and skin cancer. Regulatory agencies have not fully recognized either the inherent toxicity of paraquat or the particular risks derived from exposures in developing countries. Independent risk assessment in the developing-country context and application of the precautionary principle are necessary to prevent adverse effects of dangerous pesticides in susceptible populations.
OBJECTIVE: We present the results of a prospective and a longitudinal study of IFN-beta 1a (Rebif) in the treatment of Mexican patients with relapsing-remittin multiple sclerosis (MS). PATIENTS AND METHODS: Twenty five patients were included, all of them fulfilled the diagnostic criteria proposed by Poser, had a clinical evolution of at least 2 years, had 2 to 3 acute attacks of MS in the 12 months preceding study entry and an EDSS < 5.5. Six patients were excluded from the trial, one because of anaphylactic shock, 2 progressed to the chronic form and 3 decided not to continue in the study. These patients were considered as treatment failures and represented 24% of the participants entering the trial. The remaining 19 patients (mean age 29.8, 74% females) completed 2-year follow-up. The mean time of disease duration was 48.8 months. Six million units of IFN-beta 1a were applied thrice weekly, in 8 patients (42%) dosage reduction to 6 millions units twice a week was necessary because of leukopenia or elevation in liver enzymes. RESULTS: In the year before treatment with IFN-beta 1a patients had 2.7 +/- 0.93 acute attacks, after treatment the mean number of acute attacks per year was reduced to 1.94 +/- 2.39 (28.9% reduction). When analyzed by gender there was a 62.9% reduction in acute episodes per year in women and a 27.7% reduction in men (p < 0.05). The most frequent side effects were fatigue, headache, local skin reactions, depressed mood, myalgia, weight variations and somnolence. A few patients developed leukopenia, anemia or elevated liver enzymes. In 31.25% of the patients there was an increase in the number of lesions on MRI, in 31.25% there was a reduction in the number of lesions and in 37.5% no change was noted. CONCLUSION: The best-tolerated dose of IFN-beta 1a was 12 million units/week in women and 18 million units/week in men.

Comparative study of the therapeutical effects of cisapride and metoclopramide in 50 patients with symptomatic hiatal hernia was performed. Behaviour of media before and after treatment, of resting pressure and relaxation lower esophageal sphincter, pressure of the primary wave and the number of reflux episodes with pH < 4. The group of patients treated with cisapride had a considerably higher beneficial effect (p < 0.01) than the metoclopramide-treated group. A total of 4 patients were dropped-off the study because of undesirable side effects (diarrhea 2, headache 1, and somnolence 1).
