Effect of congestive heart failure on the intrinsic metabolic capacity of the liver in the dog.

The intrinsic metabolic capacity of the liver at end-stage heart failure in the pacing overdrive dog model of congestive heart failure was evaluated ex vivo. Congestive heart failure was induced in seven adult mongrel dogs (20-30 kg) by cardiac electrical pacing at a frequency of 240 stimuli/min until the development of overt heart failure; seven other dogs served as controls. The animals were then anesthetized and the right ventricular papillary muscles and samples from the left lateral hepatic lobes were collected. The degree of myocardial dysfunction as well as the total amount and the activities of cytochromes P-450 were evaluated. Tension, maximum rate of tension rise, and Vmax were significantly lower (40-60%) in the paced than in the control dogs, indicating a marked myocardial dysfunction. Moreover, significant decreases in total cytochrome P-450 (0.31 +/- 0.04 vs. 0.53 +/- 0.03 nmol/mg of microsomal protein, p less than 0.01) and in the intensity of four different electrophoretic protein bands (molecular masses of 46, 48, 50, and 59 kDa) occurred in the dogs with congestive heart failure. The decrease in total cytochrome P-450 was accompanied by a significant reduction in aminopyrine N-demethylase activity (1.74 +/- 0.25 vs. 2.91 +/- 0.40 nmol/min/mg of microsomal protein, p less than 0.05). Immunoblot analysis using antibodies to two different dog liver phenobarbital-inducible cytochromes P-450 demonstrated that PBD-1 (a P-450IIIA) was not affected by congestive heart failure, whereas PBD-2 (a P-450IIB) was markedly decreased.(ABSTRACT TRUNCATED AT 250 WORDS)

Amiodarone-digoxin interaction in rats. A reduction in hepatic uptake.

The interaction amiodarone-digoxin results in a marked increase in the digoxin serum concentrations. The mechanism of this interaction is still unexplained. The influence of amiodarone on digoxin hepatic extraction in rats was investigated. The digoxin hepatic extraction coefficients were determined using the isolated-perfused livers of control and amiodarone-treated (25, 50, and 100 mg/kg/day for 5 days) rats. When plotted as a function of time, an inverse relationship between the extraction coefficients and the dose of amiodarone was evident. Significant dose-related reductions were observed in hepatic clearances calculated after 16 and 20 min of digoxin infusion when steady state was achieved. Analysis of the hepatic effluent by HPLC revealed only digoxin in the effluent of both control and amiodarone-treated rats; no metabolites were detected. Using liver homogenates, no differences were found in the ability of control or amiodarone-treated (50 mg/kg/day for 5 days) rats to metabolize digoxin. These changes occur without significant lesion of the hepatocytes, inasmuch as the serum concentrations of glutamic-pyruvic and glutamic-oxaloacetic transaminases were not affected by the administration of amiodarone (50 or 100 mg/kg/day for 5 days). Furthermore, a microscopic study of the hepatocytes revealed light cytoplasmic vacuolization, normal Kupffer cells, and no evidence of macrophage proliferation. It was concluded that amiodarone increases digoxin serum concentrations by inhibiting its uptake into the hepatocytes.

[Offering a hand to new parents: listening or informing?].

Nurses working in the perinatal field have a variety of objectives, one of which is helping new parents feel competent in their new role. Recognizing parents' potential and offering them the support they need to realize it is one of the strategies suggested by the Department of Health and Social Services (MSSS, 1991 and 1992a). In order to identify aspects of nursing interventions at the time of the post-natal visit that promote parental skills, the authors conducted research into the concept of empowerment. Following an in-depth survey of the literature on this concept, they came up with a definition of interventions promoting empowerment, and used this definition as the basis for their analysis of perinatal nurse-client relations. The study, subsidized by the Fondation de recherche en sciences infirmières du Québec (FRESIQ), was carried out between 1993 and 1995 in four CLSCs in the Eastern Townships region. Thirteen nurses agreed to record their conversations with first-time parents during 20 postnatal visits.
