Children with exotropia responsive to spectacle correction of hyperopia.

A series of seven exotropic children (aged 2 to 10 years) had resolution of exotropia after spectacle correction of hyperopia. Their hyperopic correction ranged from 3.00 to 7.00 diopters. Six had intermittent exotropia, which became small-angle esophoria after spectacle correction. In one patient with apparently no fusion, spectacle correction converted constant exotropia to small esotropia in the monofixational range. In all patients, Worth 4-dot and Titmus Stereo Test results, when obtainable, indicated an improvement in binocular sensory status after correction of the hyperopia. We conclude that a trial of spectacle correction is warranted in exotropic children with severe hyperopia and in those with moderate hyperopia and a low accommodative convergence/accommodation ratio or evidence of hypoaccommodation.

Child

Inadvertent penetrating injury to the globe with periocular corticosteroid injection.

We surveyed 488 physicians regarding their experience with inadvertent ocular penetration with steroid injections over a 5-year period. Sixty-six cases of ocular penetration were reported by the 209 physicians who responded. Of the 18 patients whose retinal status was known, 12 (67%) had retinal detachments resulting from the penetration. Final visual acuities in 85% of the 26 cases were worse than 20/100.

Uveitis in childhood--Part I. Anterior.

Uveitis in children creates unique problems in the management of ocular inflammatory disease. While a child's symptoms may differ only slightly from those of adults in many cases, there are substantial differences in the signs, symptoms, etiology, and treatment. These are summarized extensively in this paper. The treatment of both the primary disease and discussion of the complications and the usual course of the illness in children is highlighted. Differential diagnosis poses key problems because some of the masquerade syndromes including juvenile xanthogranuloma and retinoblastoma may confuse the clinician in diagnosis. This is the first of a three-part series of articles.

Uveitis in childhood--Part II. Intermediate.

Intermediate uveitis poses a significant problem in treatment for the clinician. Guidelines for its treatment have been outlined in this paper including visual acuity decreases to 20/40, disc edema, subretinal exudation, and significant symptoms from vitreous floaters. Periocular adrenocorticosteroids are suggested as the treatment of choice. A brief account of the signs, symptoms, and suggested etiologic evaluation is included. This is the second in a three-part series of articles.

Uveitis in childhood--Part III. Posterior.

Toxoplasma, nematodes, and cytomegalovirus are the most common causes of uveitis in children involving the posterior pole. Discussion of treatment and management is detailed. Rubella retinitis and subacute sclerosing panencephalitis are specific entities which are limited in their initial presentation in children. Panuveitis including sympathetic ophthalmia, Vogt-Koyanagi-Harada, and Behçet's syndromes, while relatively rare in their occurrence, demand early treatment intervention. Specific antimicrobial therapy and adrenocorticosteroids as well as therapeutic suggestions are also included. This is the last part of a three-part series of articles.

Familial pars planitis.

Ophthalmologists do not consider pars planitis to occur on a familial basis. The development of pars planitis in nine members of four different families is reported. Affected family members include twin sisters, a mother and daughter, two brothers, and a sister and two brothers from the same family. This suggests that a common hereditary and/or environmental factor contributed to the disease in these patients.
