Perforation of a duodenal diverticulum is a rare complication of a common disorder, with only 59 cases reported in the world literature. When the proper diagnosis is made and diverticulectomy is done, the survival rate approximates 100%. Otherwise, the mortality rate approximates 90%. Our patient had complete recovery after surgical resection of the diverticulum.
A system for measuring parameters of ventilation and respiratory mechanics in intact anesthetized hamsters was developed. Means +/- SD of five weekly measurements in eight hamsters were 0.68 +/- 0.09 ml for tidal volume, 45 +/- 14 breaths.min-1 for respiratory frequency, 29.3 +/- 10.4 ml.min-1 for minute ventilation, 0.301 +/- 0.080 ml.cmH2O-1 for dynamic compliance, 0.435 +/- 0.151 cmH2O.ml-1.s for inspiratory resistance, 0.311 +/- 0.101 cmH2O.ml-1.s for expiratory resistance, and 0.334 +/- 0.096 cmH2O.ml-1.s for average resistance. The standard deviations of five consecutive measurements of these parameters on the same day were typically 10% of the corresponding mean values suggesting that acute changes as small as 10% can be detected in a study of similar size. The corresponding value for five single measurements on different days at weekly intervals was 30%. We believe that this system yields reasonable and repeatable measurements, and, because it is atraumatic, it can be used in chronic studies of diseases in hamsters and other small animals.

A bronchopancreatic fistula developed in a 14-year-old boy two weeks following transabdominal splenectomy and repair of a ruptured diaphragm. This unusual entity should be suspected when a patient with pancreatitis or pancreatic trauma has pleural complications associated with profuse, frothy sputum. The diagnosis is established by the presence of high levels of amylase in the sputum or by the appearance in the sputum or the bronchi of dye or contrast material injected into the external fistula. The primary goal of treatment, in addition to the management of the thoracic complications, is subdiaphragmatic control of the pancreatic fistula. A brief summary of seven previously reported cases is presented.
While the Activity Preference Questionnaire (APQ) appears to provide unusually pure and subtle measures of anxiety proneness, its forced choice format involves several drawbacks. Data are presented which suggest that a rating format can be used which avoids the disadvantages of the forced choice format but incorporates its advantages.

A method was developed for the analysis of trichlorfon as the intact molecule from feed formulations of the chemical. The method involved isolation of trichlorfon by a simple extraction procedure. Subsequent detection and quantitation of the trichlorfon was by gas chromatography with a DB-1 fused-silica wide-bore capillary column and flame photometric detection. Automated on-column injections were performed. The method was linear in the range 5-75 ng injected on-column and had an average recovery of 86%. The limit of detection for trichlorfon in feed was 1 ppm. The applicability of the method was tested by determining the stability of trichlorfon over a 21-day period in a feed blend of trichlorfon at the 50-ppm concentration level. Mass spectrometric analyses were performed to confirm that trichlorfon could be quantitated as the intact molecule.
Sexually active divorced and separated women constitute a significant population at risk for sexually transmitted diseases. With components of the Interaction Model of Client Health Behavior (IMCHB) used as the conceptual framework, a convenience sample (N=254) of women--mostly educated, White, and in their 30s (20 to 49 years)--was examined relative to condom use and individual and social context characteristics. To explore the effect of fertility on condom use, the sample was divided into fertile and infertile subsamples. The IMCHB explained 49% of the variance in infrequent condom use by the fertile women (n=93) and 21% of the variance in less frequent condom use by the infertile women (n=161).
This article describes the findings of an exploratory study which aimed to discover how clinically effective nursing care is fostered among clinical nurse specialists (CNSs) and nurse practitioners (NPs) in an NHS trust in East London. The study was funded by the Central and East London Education Consortium in January 1999. Qualitative approaches included non-participant observation of practice, interviews during observaton and focus group meetings, Findings from the research identify areas that both foster and hinder the ability of the CNSs and NPs to engage in and provide clinically effective nursing care.
A total of 1467 black patients (911 in Atlanta, 556 in Memphis) were selected (1969-70) and followed longitudinally and prospectively until death (404 patients) or through 31 December 1979, when 676 were alive and active and 387 were lost to follow-up. The women/men ratio in each cohort was 4.7/1. Women had more excess body wt than men at maximum weight and at time of diagnosis. At selection, the Atlanta cohort was older (60.2 vs 56.8 yr), had diabetes longer (7.5 vs 5.2 yr), and had a higher initial mean random plasma glucose (MRPG) level (217 vs 195 mg/dl) than the Memphis cohort. The Atlanta cohort was on sulfonylurea/phenformin therapy, which was discontinued at entry. After 9-10 yr follow-up, the MRPG level was not significantly different from the initial level in either cohort, and the Atlanta cohort level was still significantly higher (221 vs 185 mg/dl). Mean weight loss after 9-10 yr follow-up was significantly greater in Atlanta (17.7 vs 6.8 lb). Those under good control in 1979, as indicated by random plasma glucose (RPG) of less than 150 mg/dl, lost more weight (means: Atlanta, 23 lb; Memphis, 8.7 lb) than those under poor control in 1979 (RPG greater than 300 mg/dl; means: Atlanta, 14.7 lb; Memphis, 1.3 lb). In the pooled alive and active cohorts (1979), 29.1% were under good control (RPG less than 150 mg/dl); 52.9%, fair control (RPG = 150-300 mg/dl); and 18.0%, poor control (RPG greater than 300 mg/dl). Of the 639 alive and active patients, paired plasma glucose levels were less than 200 mg/dl in 207 patients in 1969-70 and less than 200 mg/dl in 322 in 1979.(ABSTRACT TRUNCATED AT 250 WORDS)

This study is a psychometric evaluation of a new measure of motivation in health behavior. The Health Self-Determinism Index was completed by 202 randomly selected adults via a mail survey. The multidimensionality of the construct was supported through the factorial isolation of four subscales: self-determined health judgments, self-determined health behavior, perceived competency in health matters, and internal-external cue responsiveness. These four factors accounted for 56% of the total explained variance in the measure. The alpha reliability coefficient for the total scale is .84; the internal consistency of the four subscales is supported by alpha reliability coefficients of .75, .75, .67, and .69, respectively. With refinement, this measure will enable exploration of client antecedents and correlates of motivation in health behavior. Further, specific nursing interventions can be examined for their ability to alter clients' motivation in relation to specific health behaviors. Finally, the role of motivation in predicting health outcomes can be explored.
