Intercellular contacts in tumours of the vascular smooth muscle cells in man.

Electron microscopy has been used to study the cell to cell and cell to stroma contacts between smooth muscle cells (SMC) in normal vessels, angiomyolipoma and well and poorly differentiated vascular leiomyosarcoma. Micrographs were examined with a semiautomatic image analysis system. The length of the cell borders was calculated and the type and number of contacts per 100 micron cell perimeter and per 100 cells were determined. In all cases there was a predominance of simple appositions. Intermediate junctions, nexus junctions, interdigitations, intermediate contacts and junctional interdigitations were less frequently observed. In general, as the SMC become malignant and less differentiated the number of cell to stroma attachments decreases markedly and the intercellular contacts increase slightly.

Argyrophilic and hormone immunoreactive cells in normal and hyperplastic pancreatic ducts and exocrine pancreatic carcinoma.

Scattered argyrophil cells were present in normal, large, medium-sized and small pancreatic ducts (ductules). There was marked increase in argyrophil cells in ducts with hyperplastic epithelium. Argyrophil cells were also found in 67.7% of all exocrine pancreatic carcinomas. In a well differentiated group including cystadenocarcinoma, mucinous carcinoma and well differentiated ductal adenocarcinoma argyrophil cells were found in all cases examined. Using four antisera (against insulin, glucagon, somatostatin and gastrin), insulin, glucagon and somatostatin cells were identified in 2.65%, 0.001% and 1.2% of normal ducts, and 7.5%, 2.4% and 4.6% of ducts with hyperplastic epithelium respectively and were also greatly increased in numbers in the latter group. Immunoreactive cells were present in 66.7% of exocrine carcinomas. Cells reactive for insulin were found in 7/15 cases; glucagon in 6/15 cases; somatostatin in 5/15 cases and gastrin in 2/15 cases. Eight cases contained two or more than two types of immunoreactive cells. The presence of argyrophil and hormone immunoreactive cells in pancreatic ducts and carcinomas is indicative of the close developmental relationship between endocrine and exocrine parts of the pancreas. The inter-relationship of response in the different cell types following stimulus suggests that injury to a common precursor may be involved.

Myoendothelial contacts in the small arterioles of human kidney.

Human renal biopsies were examined electron microscopically to investigate the intercellular relationships between endothelium and smooth muscle in renal arterioles. Three types of contacts were seen occurring through perforations in the basal lamina. These were classified by their origin either, from an endothelial cell, from a smooth muscle cell or from both. The cell processes involved were usually irregular but some were identified as club-shaped. The nature of the contacts was predominantly via simple appositions with an intercellular space of 6.9-13 nm. These myoendothelial contacts may play an important role as sensors of hypertensive load.

The no-effect level and optimal use of toxicity data.

The term "no-observable-effect level" is preferable to "no-effect level" to indicate the impossibility of demonstrating no effect. The term "no-adverse-effect level" is sometimes used, indicating that not all changes are of toxicological relevance. The size of the safety factor can be justified scientifically only when good toxicokinetic and metabolic information exists. Actually, extrapolation and risk evaluation can be carried out in a proper way only when the mechanism underlying a particular effect is known. The use of recovery groups in long-term studies is recommended.

Pesticides.

1. Chemical treatment of crops is essential not only to reduce losses but also to prevent certain food related diseases. 2. Pesticides, unlike many other compounds, are designed to kill higher organisms and thus pose special toxicological problems. 3. It is important to understand the mechanism of action of the pesticide in the target organism in order to assess risk/benefit as accurately as possible. 4. In future the need for pharmacokinetic/dynamic and comparative metabolic data will increase and it will become important to gather human exposure data for certain commonly used compounds, data which are sadly lacking at present.

Effects of danazol on incidence of progesterone and oestrogen receptors in benign breast disease.

Results from a continuing clinical trial in benign breast disease indicate that danazol may induce progesterone receptors and that this effect persists after treatment. In women given danazol the number of biopsy specimens that were positive for progesterone receptors rose from 14 out of 31 before treatment to 23 out of 30 at the end of six months' treatment and remained raised at 21 out of 31 six months later. The number of biopsy specimens that were positive for oestrogen receptors rose transiently from eight out of 31 to 12 out of 30 and then fell to six out of 31. A satisfactory clinical response was achieved in 26 of the 31 patients but was maintained in only 11 six months or more after the end of treatment. It was only in this group that a significant and long-standing increase in progesterone receptors was observed. These findings suggest that in some women with benign breast disease who have been treated with danazol changes occur that may have long term benefit.

Oestrogen and progesterone receptor distribution in the cancerous breast.

To test the hypothesis that steroid hormone receptor expression is particularly pronounced in breast tumours when compared with non-neoplastic tissue, mastectomy samples were divided into 16 sectors. Multifocal tumours, of varying receptor phenotype were found in 4 patients and in addition different regions of large tumours also showed varying receptor contents. Remaining samples were found to consist of normal tissue, with fat, connective tissue and some benign breast disease. In the 9 patients with oestrogen receptor positive tumours (ER), ER content was invariably much greater in the tumours than in the remainder of the breast. Progesterone receptor (PR) content was not closely related to ER, and was lowest in the poorly differentiated tumours. This relation to differentiation was not seen in ER. The data support the view that ER concentration in ER positive tumours may reflect the transformed nature of neoplastic tissue.

Liver lesions in an autopsy population.

Examination of the liver in 1500 consecutive autopsies performed in the coronial system in southeast London revealed a wide range of lesions. Around 5% of livers contained a solitary lesion, clinically non-evident lesions included cirrhosis, adenomas and metastatic carcinoma. Hamartomas are relatively common.

In-vitro demonstration of a complicated atherosclerosis-like lesion.

Smooth muscle cells derived from the media of adult rat aorta were grown on elastin membranes for up to 3 weeks in cell culture. Cell degeneration was evident by the 4th day and increased with time. At the end of the experiment necrotic and calcified areas were prominent. The basic feature of cell degeneration was dissolution of myofilaments; the disintegrating cytoplasm usually contained numerous glycogen granules. Increased electron density of the mitochondrial matrix and fat globules were seen and nuclei showed dilution of chromatin or pyknosis. Abundant cellular debris, vesicular structures and microfibrils were found in the extracellular space.
