Improving neglected influenza vaccination among healthcare workers in long-term care.

OBJECTIVE: To increase utilization of influenza vaccination among long-term-care health workers. DESIGN: Before-after intervention study. SETTING: A 300-bed life-care community served by 195 staff employees. INTERVENTION: An educational intervention followed by a Vaccination Fair. MEASUREMENTS: Percent receiving vaccination. A post-intervention survey was used to define staff attitudes towards vaccination. RESULTS: Use of influenza vaccination among staff increased from 8% the previous year to 46% during a Vaccination Fair and to 54% during a similar Fair the next year. CONCLUSIONS: Organized institutional efforts to promote immunization among health-care workers, such as a Vaccination Fair, improves compliance among health-care workers.

Phleborheography in pregnancy.

To determine whether pregnancy and the gravid uterus have an effect on phleborheographic findings, 48 asymptomatic pregnant patients underwent phleborheography in the second or third trimester or the immediate postpartum period. Their phleborheograms were compared to normal phleborheograms of nonpregnant patients with normal venograms. No difference was detected between the phleborheograms of the two groups. Furthermore, there was no change in the phleborheograms of patients who were tested in the second and third trimesters and also in the immediate postpartum period. The fact that the gravid uterus and pregnancy did not alter the venous wave amplitude and baseline tracing indicated the absence of chronic or acute venous obstruction. The use of a lateral rather than supine position was judged important in obtaining accurate findings. The study suggests that phleborheography can be used to diagnose deep venous thrombosis in pregnant women with the same confidence as in nonpregnant patients.

A three-year experience with phleborheography: a noninvasive technique for the diagnosis of deep venous thrombosis.

The need for a reliable noninvasive method to detect deep venous thrombosis (DVT) is apparent from its prevalence, the fallibility of its clinical diagnosis, and the several drawbacks of phlebography. Experience with phleborheography (a plethysmographic technique for detecting DVT) over three years is presented. Phleborheography (PRG) is found to have an overall accuracy of 93% compared to phlebography. It is recommended that treatment decisions can be based on unequivocal PRG results, and PRG should be used to screen high risk patients. The medical, economic, and social benefits of PRG are described.
