Visual evoked potentials during hypothermia and prolonged circulatory arrest.

Visual evoked potentials were recorded in eight children during hypothermia and circulatory arrest. The potentials were lost in all children recorded in late arrest. The evoked potential is a more sensitive indicator of CNS stress as provoked by combined hypothermia and hypoxia than is the EEG. EEG activity persisted in six of the eight children in this series even during circulatory arrest. The EEG had been seen to do the same in more than half of a larger series of children recorded at that stage. The results suggest that evoked potentials may be a sensitive indicator of early impairment of cerebral function and may demonstrate useful change sooner than the EEG. The examination may be useful in following children with illnesses producing hypoxia or anoxia. The N1 component was as easily and as frequently identifiable as the P2 component. Under the stress of this procedure, the latency of the P2 component became more variable than the N1 peak. The results suggest the N1 component may be as useful and perhaps more useful than the P2 wave in following the effect of some CNS stresses in children.

Studies on the QRS wave of the spatial magnitude electrocardiogram in ventricular septal defect.

The QRS wave of the spatial magnitude electrocardiogram (ECG) was investigated in 86 cases of the ventricular septal defect (VSD). The relationship between the amplitude and the time interval of QRS wave of the spatial magnitude ECG and the various hemodynamic parameters obtained by the right heart catheterization and the defect area ascertained during the operations were evaluated.

[Studies on the spatial velocity electrocardiogram in ventricular septal defect (authors's transl)].

The QRS waves of the spatial velocity electrocardiogram (ECG) were investigated in 94 cases of the ventricular septal defect (VSD). The relationship between the amplitudes and the time intervals of QRS waves of the spatial velocity ECG and the various hemodynamic parameters obtained by the right heart catheterization and the defect area ascertained during the operations were evaluated.

The role of mucin on experimental Pseudomonas keratitis in rabbits.

The role of mucin in the manifestation of Pseudomonas keratitis was studied. Pseudomonas was cultivated in solutions of mucin, in which it grew rapidly and then inoculated into rabbit cornea by needle pricks. When the organism was inoculated as a suspension in saline, infection infrequently occurred as small ring abscesses of short duration around a few sites of inoculation. When the organism was inoculated as a suspension in a solution of gastric mucin, infection was usually observed as severe hypopyon-keratitis with formation of a huge ring abscess. Corneal perforation and panophthalmitis resulted in some cases. It was thus concluded that the pathogenicity of Pseudomonas is definitely increased when it was inoculated into the cornea with mucin solution.

Myocardial dysfunction and depressed fatty acid metabolism in patients with cyanotic congenital heart disease.

BACKGROUND: Myocardial contractile dysfunction has been frequently observed in adolescents or adults with cyanotic congenital heart disease. Impaired energy metabolism may be present in such dysfunctional myocardium. METHODS AND RESULTS: To evaluate the findings of myocardial free fatty acid metabolism, and its relations to ventricular wall motion and myocardial perfusion in cyanotic congenital heart disease, we performed a combined study of iodine 123-labeled 15-(p-iodophenyl)-3-(R,S)-methylpentadecanoic acid (BMIPP) scintigraphy, thallium scintigraphy, and contrast cine-angiography in seven patients with single right or left ventricle. The results showed that wall motion was reduced in 17 of 35 ventricular segments (49%), which were mostly identical in location to decreased BMIPP uptake. The severity of BMIPP uptake deficit correlated positively with the degree of impairment of wall motion. On the other hand, thallium uptake was abnormal only in 5 of 35 segments (14%), and the severity of the perfusion defect did not correlate with the degree of wall motion abnormality. CONCLUSIONS: Contractile dysfunction in cyanotic heart disease was primarily linked to impaired free fatty acid metabolism rather than to myocardial scar as represented by perfusion defect on thallium imaging.

MR measurement of blood flow in the true and false channel in chronic aortic dissection.

Velocity encoded (VEC) cine MR imaging is a new noninvasive technique for the quantification of blood flow velocity in the cardiovascular system. Six patients with type B aortic dissection underwent VEC cine MR imaging at 1.5 T. This technique provides cine MR magnitude and VEC phase images at approximately 16 equally spaced intervals during an average cardiac cycle. A region of interest encompassing a vascular structure, i.e., false channel, provides a spatially averaged velocity for the time interval at which the image was acquired. Interpretation of velocity values from the 16 intervals during the cardiac cycle provides a temporally average velocity. Velocity mapping across the aortic lumen in these six cases showed average spatial and temporal velocity of 13.4 +/- 1.49 cm/s in the true channel and 3.1 +/- 0.84 cm/s in the false channel (p less than 0.05). The peak systolic velocity (temporal peak) was 43.6 +/- 7.20 cm/s in the true channel and 14.3 +/- 2.30 cm/s in the false channel (p less than 0.05). The flow volume per cardiac cycle was not significantly different between the ture (23.1 +/- 5.04 ml/cycle) and false channel (27.1 +/- 10.14 ml/cycle). There was substantial retrograde flow in the false channel of two patients. The intraobserver and interobserver variability was less than 10% (r = 0.98 to 0.99) for the measurement of flow parameters in both the true and the false channel. We conclude that VEC cine MR imaging demonstrates substantial differences in the hemodynamic pattern in the true and false channel in aortic dissection.
