Nursing challenges in the care of very low birth weight infants (< 1,000 grams).

It is not unusual for neonates to be born before the 24th week of gestation and weigh less than 1,000 g. These neonates are surviving to be transferred to a tertiary care center, and many are discharged eventually to home. When one considers that these infants are only "half developed" by the standard 40-week gestational period, it is understandable that the challenges for nurses caring for them are enormous. In this article, the authors review gestational development and the critical care needs of this special neonatal population.

Nature and prevalence of ribavirin aerosol administration in U.S. pediatric hospitals.

A survey was undertaken to determine the nature and prevalence of ribavirin aerosol administration in pediatric hospitals. Ribavirin was administered in 79.4% of the respondents' hospitals. The majority used head hoods for aerosol administration. Ventilators were the primary method used by 6% of the respondents. This study demonstrates typical types of ribavirin exposure and the need to evaluate effects of chronic exposure.

Case management in the neonatal intensive care unit.

This article will provide the reader with an overview of the concept of case management as it is used by the neonatal nurse practitioner in a neonatal intensive care unit. The role of the neonatal nurse practitioner as a case manager will be compared and contrasted to the roles of the primary nurse and staff nurse. The collaborative role with physicians will be described. Future directions for neonatal case management also will be addressed.

Nursing in genetics: current and emerging issues for practice and education.

This article focuses on genetics as a dynamic, rapidly expanding health care arena, offering nurses from all specialty areas new career opportunities. Community issues are examined, and the need for nursing research in genetics is identified. A synopsis of a recent national meeting that explored practice changes and the resulting implications for advanced education for nurses in genetics is discussed. Certification remains a controversial issue. Consensus is reached concerning the need to include genetic knowledge at the baccalaureate level. A panel discussion by students and recent graduates describes existing programs in nursing that offer a genetic specialty.

Behavioral strategies for the reduction of pain and anxiety associated with orthopedic trauma.

This study examined the efficacy of behavioral strategies in alleviating pain and anxiety associated with severe orthopedic trauma. Sixty-four patients with multiple fractures were divided into four groups: (1) control, (2) attention only, (3) EMG biofeedback-assisted relaxation, and (4) audiotaped relaxation training. All were measured over at least six sessions, or as long as hospital stay permitted. Significant between group differences were found on the following: systolic blood pressure, peripheral temperature, subjective units of discomfort, state anxiety, with a trend for use of sleep medications. No differences were found on other vital signs, EMG recordings, or other medications. EMG-biofeedback relaxation and relaxation training were relatively equivalent for all measures, and little or no change was observed for those patients who received attention only or served as controls.
