HLA-A3, B7 linkage disequilibrium in hemochromatotic patients with or without insulin dependent diabetes.

Sixty-six idiopathic hemochromatotic French patients were HLA-A, B typed. The previously known strong association with A3 was confirmed (RR = 10.6, P less 10(-9)) and our results indicate clearly that a gene implicated in idiopathic hemochromatosis (IH) determinism is located in the HLA-A region. The linkage disequilibrium between A3 and B7 was found to be far greater in IH patients than in controls. The authors have therefore hypothesized that this might be due to a selective advantage of this haplotype in IH. The A3, B7, Dw2 HLA haplotype has been shown to exert a protective effect against common insulin dependent diabetes (IDD). Thus the patients were divided into two groups according to the presence or absence of a definite IDD. B7 was found more frequently in IH patients without IDD but the difference is not significant. In this context, the strong linkage disequilibrium between A3 and B7 might be due to the protective effect of the B region of this haplotype against IH secondary IDD.

Seeding of the 10-day mouse embryo thymic rudiment by lymphocyte precursors in vitro.

The thymic rudiment was removed from the mouse embryo at 10 days of gestation, while it was still included in the 3rd branchial arch. When cultured alone, either in vitro or on the chick chorioallantoic membrane (CAM), it failed to develop as a lymphopoietic organ and remained in an epithelial state. If it was associated in transfilter culture with various types of hemopoietic organs from either embryonic or adult mice (e.g. yolk sac, fetal liver, thymus, bone marrow), it became seeded by lymphoid precursor cells and underwent a normal histogenetic process. If the donor and the receptor explants belonged to different strains of mice, the thymus that developed in culture was chimeric: thymic stroma cells (i.e., epithelial and connective cells) were of the receptor explant type, whereas the lymphoid population was of the donor type. Two genetic markers were used to label the thymic cell types, the Thy-1-1-Thy-1-2 system and the isozymes of the glucose phosphate isomerase.

Dysgerminoma with giant cells. A case report with immunoperoxidase.

A 15-year-old female with a positive pregnancy test was found to have a large ovarian tumor. Histology revealed a pure dysgerminoma with unusual giant cells, reminiscent of syncytiotrophoblast. PAP immunoperoxidase to the beta subunit of human chorionic gonadotropin (HCG) was performed on tumor sections. HCG was demonstrated within and confined to the giant cells. All studies for alpha-fetoprotein were negative. The use of immunoperoxidase in the evaluation of tumor markers and in the management of this unique tumor is discussed.

Legionnaires' disease in Johannesburg.

Legionnaires' disease (LD) was first identified as an epidemic pneumonia occurring in the USA in 1976. Sporadic cases have since been identified in many countries. This report describes the first 2 cases in South Africa of LD with pneumonia in patients in whom the diagnosis was established by the finding in early acute and follow-up convalescent sera of a greater than fourfold rise in titre with the immunofluorescent antibody test. The microbiological investigations are discussed in some detail and the clinical, radiological and laboratory features of LD are briefly reviewed.

Evidence of infection by viruses in small British field rodents.

Four populations of small wild British rodents were studied by capture--re-capture methods over a period of three years. Samples of blood were taken from these animals and tested for antibodies to nine viruses. Animals were removed from another 11 sites around the UK, and immunosuppressed. Samples of tissue from these animals were tested for the presence of viruses by passage in laboratory mice and serum samples from some of them were tested for antibody to the nine viruses. Indications were found of the possible influence of epizootic outbreaks of certain diseases on animal populations.

A note on some Enterobacteriaceae from the faeces of small wild British mammals.

Over a four-year period 878 samples of faeces were collected from five species of small wild rodents and two species of shrews at sites in England, Scotland and Wales. A search was made for Salmonella in these samples but no isolations of this genus were made. Several other genera of Enterobacteriaceae were identified.

Freeze dried platelets for HLA alloantibodies absorption.

Antibodies to HLA-A and B are specifically absorbed onto platelets. A method of absorption of these antibodies using freeze dried platelets is described. This method allows long-term storage and standardized absorption procedures. Anti-DR antibodies and cold lymphocytotoxins are recovered in the supernatant.

Epignathus with placental hydrops.

A stillborn male infant with massive epignathus showed placental enlargement, hydramnios, and hydrops. These changes are thought to be due to cardiac decompensation from circulation of the large vascular tumor mass. Y bodies present in the tumor suggest a mitotic origin for epignathi from host cells.

Leiomyomatosis peritonealis disseminata with endometrium.

A case of leiomyomatosis peritonealis disseminata (LPD) is presented with the unique finding of occasional endometrial glands and stroma within smooth muscle nodules. The possible origins of this tissue are discussed in relation to the histogenesis and prognosis of LPD.

Blink reflex: review of methodology and its application to patients with stroke syndromes.

The blink reflex may be elicited in the orbicularis oculi following electrical stimulus to the periorbital regions. It consists of a direct early component (R1) and a late component (R2) which is both direct and consensual. The amplitudes of R1 and R2 are reduced and the latencies normal in patients with lesions affecting the parietal lobe of the cerebral hemisphere. The blink reflex may well include tactile contributions from the parietal lobe. The latency of R1 may be prolonged by injuries of the pons and that of R2 by lateral medullary lesions. Blink reflexes may help to distinguish bulbar from pseudobulbar lesions and to estimate the response to therapy in patients with communication disorders.

Gastrointestinal mucormycosis.

Mucormycosis is an acutely malignant fungal disease causing local thrombosis and infarction. Gastrointestinal infection most commonly occurs in diabetics or in the immunologically compromised. This case demonstrates the radiological appearances of the gastrointestinal lesion and prolonged survival with amphotericin-B therapy.

The acardiac anomaly new case reports and current status.

Two cases of holoacardius are presented. Both had a normal 46,XX female karyotype, identical to that of their respective cotwin. Data are presented supporting the concept that the placental vascular anastomoses are the primary agents in the formation of an acardiac. Cytogenetics, epidemiology and other theories of pathogenesis are discussed.
