Studies on mode of detection of leprosy in China during the years 1981-1998.

Along with the nationwide economic reform initiated in the early 1980s and the rapid decrease of leprosy endemic after the implementation of multi-drug therapy (MDT), the leprosy programme changed from 'vertical' to 'horizontal'. An evolution in the mode of detection of leprosy cases has consequently taken place. Based on the nationwide registration of newly detected cases, the profile of patients at detection has been studied. The proportions of cases corrected significantly with calendar years in detection by dermatological clinics, contact checks, 'clue survey' and mass survey, showing a significant increase in percentage of cases detected through dermatological clinics and contact checks, and decreases through 'clue survey' and mass survey. Detection of cases through dermatological clinics and voluntary reporting have become the main modes of case-finding during 1997-1998, accounting for 37.3% and 28.6%, respectively, where contact check accounts for only 9.1%. In areas with good dermatological services, a significantly higher proportion (75.9%) of cases was detected through dermatological clinics, where voluntary reporting and 'clue survey' were the main modes of detection in endemic areas. As regards confirmation of diagnosis, the great majority of cases were confirmed by leprosy units, even though they were detected in various other situations. Only 6.5% of leprosy cases were detected and subsequently confirmed by doctors in dermatologic clinics. The present modes of detection and their relation to demographical, epidemiological, clinical factors and health services are discussed. This study emphasizes the cardinal importance of the dermatological clinics in the detection of leprosy cases in China at the present time and hence the need to strengthen the training of doctors in these clinics, while continuously encourage their involvement in leprosy control.

[A preliminary study of loss of heterozygosity on chromosome 14 in glioblastoma].

OBJECTIVE: In order to locate the deletion areas probably harboring tumor suppressor genes on chromosome 14 and provide clues for discovering novel tumor suppressor genes. METHODS: Fourteen loci on chromosome 14 were examined to detect loss of heterozygosity(LOH) in 20 cases of glioblastoma(GBM) by PCR based microsatellite polymorphism analyses, in which fluorescence-labeled primers and Perkin Elmer 377 DNA Sequencer were applied. RESULTS: 50% informative cases of GBM displayed LOH on chromosome 14; 38.2% of informative loci showed LOH in our series, in which the most frequent LOH was observed at locus D14S65(57.1%) on 14q31-32.3 and in the chromosomal region from locus D14S63 (46.7%) to D14S74(47.1%) on 14q21-24.1. 32% of informative cases displayed LOH on 14p and 50% on 14q. No microsatellite instability was observed. CONCLUSION: Loss of genetic material on chromosome 14q may play an important role in molecular genetic pathogenesis of GBM. The chromosomal regions at D14S65 on 14q31-32.3 and from D14S63 to D14S74 on 14q21-24.1 may harbor novel tumor suppressor genes associated with GBM.

[Intervention of Fuzhenghuayu decoction on autocrine activation of hepatic stellate cells].

OBJECTIVE: To investigate the intervention of Fuzhenghuayu decoction on autocrine activation of hepatic stellate cell. METHODS: The drug serum containing Fuzhenghuayu decoction was collected from the normal rats, and cultured with activated hepatic stellate cells (HSC) in vitro. The conditional medium from the drug serum treated HSC was collected, and added to primarily cultured quiescent HSC. The primary cell proliferation was assayed by MTT colorometric test, and the contents of type I collagen and vasular endothelial growth factor (VEGF) in the medium were assayed with ELISA. RESULTS: The coditional medium from activated HSC could stimulate the proliferation and type I collagen secretion in the quiescent HSC. The drug serum inhibited this stimulating action and VEGF secretion from activated HSC. CONCLUSIONS: Fuzhenghuayu decoction has inhibitory action on the autocrine-activating pathway of HSC. The mechanism may be associated with inhibiting the secretion of VEGF by activated HSC.

[An off site petroleum-contaminated soil bioremediation technology: soil compositing in windrow].

With off-site bioremediation technology, a soil contaminated by crude oil from Liaohe Oil Field was treated on a 20 x 10 m prepared bed. 8 composting windrow units were set, each measured 8 m in length, 2 m in width, and 0.35 m in height. The results showed that when the pollutant petroleum hydrocarbon(TPH) was within the range of 4.16-7.72 g.100 g-1 soil, the total degradation rate of TPH reached 45.19%-56.74% after 53 days operation, which indicated that a technological basis would be provided for the bioremediation of oil-contaminated soil.

Determination of the dominant language hemisphere by functional MRI in patients with temporal lobe epilepsy.

OBJECTIVE: To determine the dominant language hemisphere in patients with intractable temporal lobe epilepsy before surgical treatment by using functional MRI (fMRI). METHODS: Twelve patients underwent fMRI. Among them, 4 patients also underwent the Wada test. Anterior temporal lobectomy was performed on patients with epileptic focus on the non-dominant hemisphere, and selective amygdalo-hippocampectomy on patients with epileptic focus on the dominant hemisphere. RESULTS: Ten patients had left dominance, and 2 right dominance. The fMRI results were concordant with Wada test results in the 4 patients who accepted the Wada test. Seven patients underwent anterior temporal lobectomy, and 5 underwent selective amygdalo-hippocampectomy. There was no post-operative dysphasia. CONCLUSION: fMRI is an alternative method of noninvasive functional mapping for language dominance in patients with temporal lobe epilepsy.

Diagnosis and surgical treatment of temporal lobe epilepsy.

OBJECTIVE: To discuss the diagnosis and surgical treatment of temporal lobe epilepsy. METHODS: From 1996 to 1998, 36 patients presenting with medically intractable temporal lobe epilepsy were identified by EEG and MR, including volumetric MR, for hippocampal formation. Among them, 16 patients underwent anterior temporal lobectomy, while 20 patients accepted selective amygdalohippocampectomy. The results of surgical operation and follow-up are analyzed. RESULTS: With respect to seizure outcome, all patients benefited from surgery. The surgical results were satisfactory in 24 cases, notable in 11 cases, and good in 1 case, and there were no complications. CONCLUSION: MR is beneficial for the localization of epileptic foci. Surgery is an effective method for the treatment of temporal lobe epilepsy; selective amygdalohippocampectomy is the first choice for medial temporal lobe epilepsy. The transzygmatic-temporal lobe base approach presented in this study improved the safety of the surgery.

Effects of extrinsic positive end-expiratory pressure on work of breathing in patients with chronic obstructive pulmonary disease.

OBJECTIVE: To investigate the effects of extrinsic positive end-expiratory pressure (PEEPe) on work of breathing in patients with chronic obstructive pulmonary disease (COPD) and their corresponding mechanism. METHODS: Ten ventilated patients with COPD were included in the study. A Bicore CP-100 pulmonary monitor (Bicore Monitoring System, USA) was used for monitoring respiratory mechanics. First, dynamic intrinsic positive end-expiratory pressure (PEEPi, dyn) was measured when PEEPe was zero, which was called PEEPi, dynz. Then the PEEPe was set randomly at 0%, 40%, 60%, 80% and 100% of PEEPi, dynz respectively. Pulmonary mechanics and other parameters (heart rate, blood pressure and blood gas analysis) were measured 30 minutes after the level of PEEPe was changed. RESULTS: Work of breathing patient (WOBp), pressure time product, difference of esophageal pressure and PEEPi, dyn decreased significantly when PEEPe was applied, and continued decreasing as PEEPe was increased. Work of breathing ventilator increased significantly when PEEPe was increased to 80% and 100% of PEEPi, dynz. Significantly positive linear correlation was found between the changes in WOBp and in PEEPi, dyn. CONCLUSIONS: WOBp decreases gradually as PEEPe is increased. WOBp decreases by narrowing the difference between the alveolus pressure and the central airway pressure at the end of expiration when PEEPe is applied.

Effects of extrinsic positive end-expiratory pressure on cardiopulmonary function in patients with chronic obstructive pulmonary disease.

OBJECTIVE: To choose one optimal extrinsic positive end-expiratory pressure (PEEPe) for ventilated patients with chronic obstructive pulmonary disease (COPD) and to compare two methods for choosing the optimal level of PEEPe. METHODS: Ten ventilated patients with COPD were included in the study. First, static intrinsic positive end-expiratory pressure (PEEPi, st) was measured when PEEPe was zero, and the PEEPi, st was called PEEPi, stz. PEEPe at 0%, 40%, 50%, 60%, 70%, 80%, 90% and 100% of PEEPi, stz, respectively, were applied randomly. Respiratory mechanics, hemodynamics, and oxygen dynamics were recorded 30 minutes after the level of PEEPe was changed. RESULTS: When PEEPe was not higher than 80% of PEEPi, stz, no measurement changed significantly. When PEEPe was increased to 90% and 100% of PEEPi, stz, PEEPi, st, peak inspiratory pressure, plateau pressure, pulmonary capillary wedge pressure and central venous pressure increased significantly, P < 0.01. Cardiac output and left ventricular work index decreased significantly, P < 0.01. Oxygen delivery decreased significantly, P < 0.05. When PEEPe was increased to 100% of PEEPi, stz, the right ventricular work index decreased significantly, P < 0.05. CONCLUSION: Eighty percent of PEEPi, stz was the upper limit of PEEPe. The results of the two methods used to set the level of PEEPe were identical.

[The effects of extrinsic positive end-expiratory pressure on work of breathing in patients with chronic obstructive pulmonary disease].

OBJECTIVE: To study the effect of extrinsic positive end-expiratory pressure(PEEPe) on work of breathing in chronic obstructive pulmonary disease (COPD) patients and its mechanism. METHODS: 10 ventilated patients [4 males and 6 females with mean age of (70.1 +/- 5.5) years] with exacerbation of COPD were admitted into the study. All the patients' cardiopulmonary functions were stable, and they could receive pressure support ventilation (PSV). Patients with sever heart dysfunction, hepatic dysfunction, renal dysfunction or fever were ruled out. Each patient had an esophagus balloon pressure sensor inserted, and the esophagus balloon was properly positioned using "occlusion test". Flow sensor was placed at the end of tracheal tube opening. Then esophagus balloon pressure sensor and flow sensor were connected with Bicore CP-100 pulmonary monitor. Dynamic intrinsic positive end-expiratory pressure (PEEPi, dyn), difference of esophagus pressure (dPes), work of breathing patient (WOBp), work of breathing ventilator (WOBv), pressure time product (PTP), tidal volume (Vt), respiratory rate (RR), minute ventilation (MV), peak inspiratory flow rate (PIFR), peak expiratory flow rate (PEFR), expiratory airway resistance (Rawe), fraction of inspiration (Ti/Ttot) and so on were measured. PEEPi,dyn was measured by esophagus balloon technique. Electrocardiogram and noninvasive blood pressure modes were added to HP M1165A monitor. Heart rate (HR) and blood pressure (BP) were also measured. Arterial blood was analyzed to measure pH, partial pressure of oxygen in arterial blood (PaO(2)), partial pressure of carbon dioxide in arterial blood (PaCO(2)) and saturation of arterial blood oxygen (SaO(2)). At first, PEEPi, dyn was measured when PEEPe was zero, which was called PEEPi, dynz and was set as baseline. Then PEEPe was set randomly, which was 0%, 40%, 60%, 80% and 100% of PEEPi, dynz respectively. Other parameters remained stable. All the indexes were measured 30 minutes after the level of PEEPe was changed. RESULTS: WOBp decreased as PEEPe increased, from (1.08 +/- 0.49) J/L of PEEPe-0% of PEEPi,dynz to (0.57 +/- 0.32) J/L of PEEPe-100% of PEEPi, dynz, and significant difference was found between each level of PEEPe and the baseline (P < 0.01). But we didn't find significant difference between PEEPe-80% of PEEPi,dynz and PEEPe-100% of PEEPi, dynz. WOBv increased significantly as PEEPe increased, from (1.50 +/- 0.47) J/L of PEEPe-0% of PEEPi, dynz to (1.82 +/- 0.56) J/L of PEEPe-80% of PEEPi, dynz and (1.85 +/- 0.48) J/L of PEEPe-100% of PEEPi,dynz(P < 0.01). The total WOB (WOBp + WOBv) remained steady. PTP trended to decrease, from (153.6 +/- 76.8) cm H(2)O x s x m(-1) of PEEPe-0% of PEEPi, dynz to (86.9 +/- 45.9) cm H(2)O x s x m(-1) of PEEPe-100% of PEEPi, dynz; the difference of PTP between PEEPe-40% of PEEPi,dynz and PEEPe-0% of PEEPi, dynz was significant(P < 0.05), the differences between other levels of PEEPe and PEEPe-0% of PEEPi, dynz were very significant (P < 0.01), but no significant difference was found between PEEPe-80% of PEEPi, dynz and PEEPe-100% of PEEPi, dynz. PEEPi,dyn also decreased significantly as PEEPe increased, from (7.70 +/- 1.42) cm H(2)O of PEEPe-0% of PEEPi, dynz to (2.10 +/- 0.57) cm H(2)O of PEEPe-100% of PEEPi, dynz, very significant differences were found between each level of PEEPe and the baseline (P < 0.01). Significant positive linear correlation was found between DeltaWOBp and DeltaPEEPi, dyn were(r = 0.609, P < 0.01, n = 10). dPes decreased as PEEPe increased, from (15.20 +/- 4.16) cm H(2)O of PEEPe-0% of PEEPi, dynz to (8.20 +/- 4.13) cm H(2)O of PEEPe-100% of PEEPi, dynz, and very significant differences were found between each level of PEEPe and the baseline (P < 0.01). But the difference between PEEPe-80% of PEEPi, dynz and PEEPe-100% of PEEPi, dynz wasn't significant, even dPes of PEEPe-100% of PEEPi, dynz was higher than that of PEEPe-80% of PEEPi, dynz. Vt didn't change markedly. RR, MV and f/Vt increased slightly. Both PIFR and PEFR didn't change markedly. At the same time, HR, BP and blood gas analysis were investigated, but no significant change was found. CONCLUSION: PEEPe can reduce WOBp significantly and improve the coordination between the patient and ventilator. It is shown that decrement of pressure difference between alveolar and central airway leads to decrement of WOBp.

[Investigation of the polymorphism of HLA-DQCAR microsatellites in the healthy and pulmonary tuberculosis in Hunan Hans].

OBJECTIVE: To observe HLA-DQCAR microsatellite distribution and the association of this gene with pulmonary tuberculosis in Hunan Hans. METHODS: The polymorphism of HLA-DQCAR microsatellite was analyzed by electrophoresis on a 8% denaturing polyacrylamide gel. The length of its alleles was determined by sequencing. RESULTS: Ten alleles, DQCAR101, 103, 105, 107, 111, 113, 115, 117, 119 and 121, were found in 68 Hunan healthy Hans, but the frequencies of them were different. In addition to the above-mentioned alleles, DQCAR 99 and 109, were also found in 79 pulmonary tuberculosis patients in Hunan Hans. This results as well showed that DQCAR115 allele was significantly associated with pulmonary tuberculosis in Hunan Hans (RR = 2.12, Pc < 0.05). CONCLUSIONS: The frequency of DQCAR alleles is of great polymorphism. DQCAR115 is significantly associated with pulmonary tuberculosis in Hunan Hans.

[The effects of nifedipine on proliferation and DNA synthesis of gingival fibroblasts].

OBJECTIVE: To investigate the effects of nifedipine, a calcium channel blocker, on cellular proliferation in gingival fibroblasts in vitro. METHODS: Gingival fibroblasts were cultured and incubated with various concentrations of nifedipine (1,200.0, 360.0, 108.0 and 32.4 micrograms/L) for up to 7 days. H-TdR incorporation assay in vitro taken was used to evaluate the amount of DNA synthesis. RESULTS: Cell numbers were significantly different among these groups on sixth and seventh day, especially higher in 1,200.0 micrograms/L and 360.0 micrograms/L groups. The incorporation percentage in high concentration groups were significantly higher than those in others. CONCLUSION: Nifedipine can influent proliferation of gingival fibroblasts in high concentration.

[Study on the application of quantitative analysis of cell-wall amino acids in actinomycetes classification].

This paper has collected 70 recognized type strains of actinomycete from home and abroad Microbiological culture collection centers. Adopting thin-layer chromatography and thin-layer chromatogram scanner methods to quantitatively analyze the composition of cell-wall amino acids in actinomycete. It has made comparison between quantitative results and qualitative ones. After a thorough discussion, a revision is made on the standard of dividing cell-wall types in actinomycete chemical classification, which has provided some original suggestion in hope of improving the research of actinomycete chemical classification.

[Immunocytochemical localization of fertilin beta on human sperm].

Fertilin is a kind of sperm plasma membrane protein that mimics snake venom protein. It belongs to the ADAMs family of surface proteins that contain a disintegrin and a metalloprotease domain. Fertilin functions in the sperm-egg binding process by connecting the sperm to the egg plasma membrane via a binding site in the disintegrin domain of fertilin beta (HF93). Its localization on the sperm is in the change. In this study, the monoclonal antibody against human fertilin beta was prepared and used to analyze the localization of fertilin beta on capacitated and acrosome-reacted sperm by immunofluorescence and immunoelectron microscopy techniques. The results were as follows: (1) fertilin beta became restricted to the anterior head during the course of capacitation. (2) During the course of acrosome reaction, the expression and localization of fertilin beta changed immensely on the anterior head and restricted to the lateral of posterior head at last. The restrictions of fertilin beta to the anterior head of capacitated sperm of human beings indicated that fertilin beta may be involved in the binding the sperm to the epithelial cells of the oviduct; the restrictions of fertilin beta to the posterior head domain of acrosome-reacted sperm implied its function in sperm-egg binding and fusion.

[Effects of gliclazide on blood rheology of diabetes Wistar rat].

OBJECTIVE: To assess the effects of Gliclazide on blood rheology of alloxan-induced diabetes Wistar rats. METHODS: Four weeks after the establishment of the animal models, Gliclazide was given to the rats by gastrogarage for 8 weeks. A comparison was made on the hemorrheologic parameters between the control group and the study group. Also a comparative analysis of the study group befor and after the administration of Gliclazide was performed. RESULTS: All of the parameters related to the blood rheology of alloxan-induced diabetes Wistar rats showed significance differences in the controlled comparison and the before-and-after comparison (P < 0.05). CONCLUSION: Gliclazide can obviously improve the blood rheology of diabetic Wistar rats.

Arabidopsis transcription factors: genome-wide comparative analysis among eukaryotes.

The completion of the Arabidopsis thaliana genome sequence allows a comparative analysis of transcriptional regulators across the three eukaryotic kingdoms. Arabidopsis dedicates over 5% of its genome to code for more than 1500 transcription factors, about 45% of which are from families specific to plants. Arabidopsis transcription factors that belong to families common to all eukaryotes do not share significant similarity with those of the other kingdoms beyond the conserved DNA binding domains, many of which have been arranged in combinations specific to each lineage. The genome-wide comparison reveals the evolutionary generation of diversity in the regulation of transcription.

Isolation and characterization of the notch ligand delta4.

Notch signaling plays a critical role in a variety of developmental programs. In vertebrates, the complexity of the process is underscored by the existence of multiple Notch receptors and multiple ligands, each of which displays a distinct expression profile. Furthermore, the ligands can be subdivided into two families, the Serrate/Jagged family and the Delta family. Here we present the isolation of a novel Notch ligand, Delta4. Expression analyses indicate that mouse Delta4 is highly expressed in the eye and lung during embryogenesis and in the heart, lung, liver, and kidney of the adult. Functionally, Delta4 is indistinguishable from Jagged1 in its abilities to inhibit myogenesis and to stimulate transcription through Notch1 and the DNA binding protein CSL.

Angiogenesis is induced in a rabbit model of hindlimb ischemia by naked DNA encoding an HIF-1alpha/VP16 hybrid transcription factor.

BACKGROUND: Hypoxia-inducible factor-1 (HIF-1) is a heterodimeric transcription factor that regulates expression of genes involved in O(2) homeostasis, including vascular endothelial growth factor (VEGF), a potent stimulator of angiogenesis. We sought to exploit this native adaptive response to hypoxia as a treatment for chronic ischemia. METHODS AND RESULTS: A hybrid protein consisting of DNA-binding and dimerization domains from the HIF-1alpha subunit and the transactivation domain from herpes simplex virus VP16 protein was constructed to create a strong, constitutive transcriptional activator. After transfection into HeLa, C6, and Hep3B cells, this chimeric transcription factor was shown to activate expression of the endogenous VEGF gene, as well as several other HIF-1 target genes in vitro. The bioactivity of HIF-1alpha/VP16 hybrid gene transfer in vivo was examined in a rabbit model of hindlimb ischemia. Administration of HIF-1alpha/VP16 was associated with significant improvements in calf blood pressure ratio, angiographic score, resting and maximal regional blood flow, and capillary density (all P:<0.01). CONCLUSIONS: The HIF-1alpha/VP16 hybrid transcription factor is able to promote significant improvement in perfusion of an ischemic limb. These results confirm the feasibility of a novel approach for therapeutic angiogenesis in which neovascularization may be achieved indirectly by use of a transcriptional regulatory strategy.

A single residue contributes to the difference between Kir4.1 and Kir1.1 channels in pH sensitivity, rectification and single channel conductance.

Kir1.1 and Kir4.1 channels may be involved in the maintenance of pH and K+ homeostasis in renal epithelial cells and CO2 chemoreception in brainstem neurons. To understand the molecular determinants for their characteristic differences, the structure-function relationship was studied using site-directed mutagenesis. According to previous studies, Glu158 in Kir4.1 is likely to be the major rectification controller. This was confirmed in both Kir1.1 and Kir4.1. Mutation of Gly210, the second potential rectification controller, to glutamate did not show any additional effect on the inward rectification. More interestingly, we found that Glu158 in Kir4.1 was also an important residue contributing to single channel conductance and pH sensitivity. The E158N Kir4.1 mutant had a unitary conductance of 35 pS and a midpoint pH for channel inhibition (pKa) value of 6.72, both of which were almost identical to those of the wild-type (WT) Kir1.1. Flickering channel activity was clearly seen in the E158N mutant at positive membrane potentials, which is typical in the WT Kir1.1 but absent in the WT Kir4.1. Reverse mutation in Kir1.1 (N171E) reduced the unitary conductance to 27 pS (23 pS in WT Kir4.1). However, the pH sensitivity of this mutant did not show a marked difference from the WT Kir1.1. Therefore, it is possible that a residue(s) in addition to Asn171 is also involved. Thus we studied several other residues in both M2 and H5 regions. We found that joint mutations of Val140 and Asn171 to residues seen in Kir4.1 greatly reduced the pH sensitivity (pKa 6. 08). The V140T mutation in Kir1.1 led to a unitary conductance of approximately 70 pS, and the G210E mutation in Kir4.1 caused a decrease in pH sensitivity of 0.4 pH units. These results indicate that the pore-forming sequences are targets for modulations of multiple channel-biophysical properties and demonstrate a site contributing to rectification, unitary conductance and proton sensitivity in these Kir channels.
