Respiration related neurons in the region of the nucleus tractus solitarius of the rabbit.

Systematic mapping of respiration-related units (RRUs) in the region of the nucleus tractus solitarius (NTS) was performed on rabbits anesthetized with urethane. Of 523 RRUs 295 (56%) were inspiratory (I), 130 (25%) expiratory (E) and 98 (19%) phase-spanning (PS). Such a large number of E and PS RRUs in NTS have not been reported in the cat. Six out of 16 I RRUs 6 of 13 E RRUs, and 1 of 3 PS RRUs had projections to contralateral cervical spinal cord. The 'no I inflation' test was conducted on 76 I and 38 E RRUs, of which 29 I RRUs were identified as I alpha and 31 as I beta. Thirteen E RRUs were inhibited and 6 facilitated by preceding lung inflation. Almost all types of respiratory neurons so far reported from different nuclei of cat's brainstem have been located in the NTS region of the rabbit.

The role of the human caudate nucleus in acupuncture analgesia.

The role of the caudate nucleus in acupuncture analgesia was studied in 17 patients receiving caudate stimulation through chronic implanted electrodes to relieve intractable pain caused by late malignancy. Electrical stimulation of the head of the caudate nucleus provided relief from intractable pain in all 17 patients. The pain and pain tolerance threshold were elevated, the alteration in skin galvanic activity, respiratory movement and fingertip plethysmography elicited by a given painful stimulus were depressed. The analgesia required a period of induction and persisted for some time after cessation of stimulation; it showed no obvious segmental topography. These characteristics are similar to those observed in acupuncture analgesia. The effect of caudate stimulation was similar to that of electric needling in depressing the late component of the somatosensory evoked potentials (SEPs) recorded over the scalp and from the centrum medianum of the thalamus. Evoked potentials could be recorded from the caudate nucleus when an acupuncture point was electrically stimulated. The evoked potential record was a complex wave of positive and negative components, the peak latencies of main components being 100-180 msec for positive wave and 148-332 msec for negative wave components. The present study supports the supposition that the caudate nucleus may play a role in acupuncture analgesia. The possibility that the effect of caudate stimulation is brought on by inhibiting the activity of the medial thalamus is discussed.

Rod and rod mediated function in patients with beta-thalassemia major.

An electroretinographic (ERG) study was undertaken to test the hypothesis that scotopic retinal function is altered in transfused thalassemics on chronic Deferoxamine (DFO). ERG a- and b-wave responses and dark adapted visual thresholds were obtained from 11 patients with beta-thalassemia major, ages 7 to 38 (median 17) years. A quantitative model of the activation of phototransduction was fitted to the a-waves to estimate the gain of the transduction processes and the saturated amplitude of the rod photoresponse. From b-wave stimulus/response functions. the saturated b-wave amplitude and an index of b-wave sensitivity (log sigma ) were calculated. The patients' data were compared to those of normal subjects. The relations of the ERG parameters to age. average ferritin level, and duration of transfusion without DFO as well as other clinical parameters were examined. Longitudinal measures of b-wave responses and dark adapted visual thresholds. available for nine of the patients, were examined for significant change over time. For all patients both the gain and saturated amplitude of the rod response are normal. In two patients log sigma is below the 99% prediction interval for normal. One has low scotopic visual sensitivity. The duration of transfusion therapy unprotected by DFO chelation therapy was correlated with log a. These results suggest iron accumulation rather than DFO toxicity underlies scotopic dysfunction in older thalassemics. some of whom may have had extended periods of transfusion without the protection of chelation. Thus, monitoring of retinal function is recommended in such patients.

Long-term follow-up of a phase II trial of oral altretamine for consolidation of clinical complete remission in women with stage III epithelial ovarian cancer in the Southwest Oncology Group.

OBJECTIVE: This report provides follow-up progression-free survival (PFS) and median survival data for women who achieved clinical complete remission (cCR) from stage III ovarian cancer after first-line therapy and were treated with altretamine consolidation therapy. METHODS: Patients who enrolled in the SWOG 9326 study from September 1993 to July 1997 were required to have documented cCR from stage III ovarian cancer following front-line platinum-based therapy. Treatment consisted of 6 months of oral altretamine at 260 mg/m(2)/day for 14 consecutive days of a 28-day cycle. RESULTS: Ninety-seven of 112 enrolled patients were evaluable for efficacy. This report presents median 6.2-year follow-up, dating from study registration. Median PFS was 28 (95% CI: 19-43) months. Median PFS for patients with optimal disease was 45 (95% CI: 27-48) months and for patients with suboptimal disease was 17 (95% CI: 12-26) months. Twenty-six of 61 (43%) patients with optimally debulked lesions and 5 of 36 (14%) patients with suboptimally debulked lesions remained disease free. Median survival of patients with optimally debulked disease has not been reached; median survival of patients with suboptimally debulked disease was 39 (95% CI: 19-51) months. No treatment-related adverse events were reported during the follow-up period. CONCLUSIONS: Consolidation therapy with oral altretamine was generally well tolerated and associated with prolonged progression-free and overall survival in the Phase II setting.

Prediction of elimination of leprosy in leprosy endemic areas of China.

A study was carried out based upon the data from the National System for Leprosy Surveillance and using appropriate mathematical models. The results showed that of 337 counties where the national goal of basic eradication of leprosy had not been reached and in 40 counties where the WHO goal of leprosy elimination had not been achieved in 1996, the detection rates in calendar years followed exponential models with significant goodness-of-fit. In the 67 counties with downward trends of detection rates, the national goal can be met in terms of detection rate in 6% of counties before the year 2000 or 34.4% before the year 2010, or, in terms of prevalence rate in 31.3% before the year 2010. In the 11 counties with downward trends of the detection rates, the WHO target can be met in eight to ten counties within this century when the duration of disease was determined with the WHO definition. If the MB proportion among new cases increased by 10%, the target would be met one year later. However, at the same MB proportion, the change of fixed treatment schedules from PB six months and MB two years to PB nine months and MB three years will cause achievement of the goal to be postponed by two to ten years.

Development and characterization of a prepubertal rat Sertoli cell line, 93RS2.

Sertoli cells in the seminiferous epithelium provide both structural and nutritional support to germ cells during spermatogenesis. Primary Sertoli cells in culture are an effective tool for the in vitro study of Sertoli cell function; however, primary cultures are inherently variable, time consuming to prepare, expensive, and wasteful of animals. We therefore developed a Sertoli cell line, called 93RS2, by immortalizing primary Sertoli cells derived from prepubertal rats with SV40 tsA255. This cell line proliferates at the permissive temperature (32 degrees C) and has enhanced expression of a differentiated Sertoli cell phenotype at the nonpermissive temperature (40-41 degrees C). Cytogenetic analysis demonstrated that 93RS2 has 42 chromosomes per cell, the same as a normal rat. mRNA analysis showed that this cell line, when cultured at a nonpermissive temperature, exhibited increased expression of transferrin in the presence of testosterone and enhanced expression of sulfated glycoprotein-2. A tumorigenicity assay showed that 93RS2 cells were temperature-dependent for growth in soft agar and were capable of forming tumors in nude mice. In conclusion, this rat 93RS2 cell line should be useful for the study of Sertoli cell function.
