[The treatment of aorto-enteric fistulas. Remark apropos of 2 case reports].

Aorto-enteric fistulas: about two cases. In this paper, on the occurrence of two new cases records, we resume the current data of the publications about aorto-enteric fistulas. We emphasize the symptomatology inasmuch the diagnostic approaches of primary and secondary aorto-enteric fistulas. The different ways of the operative treatment are discussed. The conclusion is extra-anatomic bypass associated with graft excision is currently the best therapeutic choice in case of secondary fistulas. About primary fistulas, the treatment is still controversial immediately by extra-anatomic bypass or arterial graft in situ and closure of the digestive fistula.

[Long-term results in ileo-anal reservoirs in the treatment of ulcero-hemorrhagic rectocolitis].

The authors report long term results of 53 ileo-anal anastomosis for ulcerative colitis. They used a "J" pouch technique with anastomosis to the anal canal after mucosectomy of the lower rectum. An upstream ileostomy should be created and left in place for two to three months. Early morbidity primarily involves pelvic or parietal infectious problems, while late complications comprise pelvic fistulae, obstructive events, and episodes of pouch inflammation. Functional results improve over the first year and remain stable thereafter. The major long term problem remains that of pouch inflammation its treatment, and the understanding of its pathophysiology.

Hepatic resections after chronotherapy: surgical aspects.

Hepatic resection has always been recommended for the treatment of metastatic disease in selected patients. Surgeons have had modified their approach and timing of surgery since the introduction of chemotherapy. The authors report their experiences since the introduction of chronochemotherapy. From 1990 up to 1994, 9 hepatic resections were performed for metastatic colorectal carcinoma. During the same period, 11 arterial accesses were implanted for diffused hepatic disease. Of the 9 resected patients, 5 are still alive with a mean follow-up of 17.7 months with no signs of recurrent disease.
