Reconstruction of the anterior cruciate ligament. A new positioning and fixation technique.

A new technique for ACL reconstruction is presented. The middle third of the patellar tendon is anchored to the inner surface of the lateral femoral condyle under direct visual control. The transplant is fixed tibially beneath a bone block removed from the ventral tibial head. Thirty-three patients were evaluated preoperatively and 27 months (range, 24 to 30 months) postoperatively. The mean preoperative function score (Lysholm scoring scale) was 72 +/- 14 points; the mean postoperative score was 84 +/- 13 points. The Lachman test was positive for all patients preoperatively, but the test results in the postoperative follow-up examination were negative--1+ in 31 knees and 2+ in 2 knees. All 33 knees elicited a definite "hard" end point. The postoperative KT-1000 measurement for anterior tibial displacement showed no significant difference between the operated versus the unoperated knee. Before their knee injury all patients had been engaged in sports. At the follow-up examination, 13 patients had made a full return to their former sports activities, 13 were restricted in their sports activities, and 7 patients no longer participated in sports.

[A case with complicated disorder of the occipito-cervical junction].

Only very few complete aplasias of the posterior atlas arch have been described. Aplasia combines with assimilation of the anterior atlas arch and a basilar impression is an even rarer variant of this developmental anomaly in the occipitocervical region. The clinical symptoms in the case described were limited to persistent nuchal pain and severely limited function of the cervical spine.

[Meniscus replacement using incongruent transplants--an experimental study].

The effect of incongruous medial meniscal grafts on the articular surface was studied. We compared the cartilage degeneration in 4 Groups of 25 sheep knees. The histological changes were graded according to the score of Mankin, the morphological alterations by the score of Jackson. In Group 1 no surgery was performed. In group 2, the meniscus was totally detached from its base at the capsule and refixed without changes in the congruity or isometry. This group provided the basic data. In group 3, the contralateral medial meniscus was used as a transplant by reversing it. The reattachment was done according to isometric conditions. With this technique only the congruity of the tibial and femoral surface was modified. In group 4, the medial meniscus was completely removed. The results of group 1 (Mankin grade 0.58; Jackson grade 0.00) and group 2 (grade 0.50 and 0.47) showed no difference. The highest degree of degenerative changes occurred in the meniscectomized knees (group 4, grade 4.47 and 1.73), however considerable changes were also found in the knees with an incongruous meniscal graft (group 3, grade 3.37 and 1.27). The results suggest that incongruous grafts will lead to degeneration of the articular surface but still have a chondroprotective effect.
