Correlating imaging and histopathology of an odontogenic keratocyst in the nevoid basal cell carcinoma syndrome.

We present the correlative diagnostic imaging and histological findings in a case of multiple odontogenic keratocysts (OKC) associated with the Nevoid Basal Cell Carcinoma Syndrome (NBCC). Whilst CT displayed aspects of bone morphology not visible on the plain films, contrast-enhanced MR provided the essential macropathological detail, including focal wall-enhancement and iso-intense intraluminal soft-tissue masses which correlated with the histological findings of focal inflammatory ulceration of the cyst lining, orthokeratosis and cell debris. The features identified by these combined imaging techniques are helpful in distinguishing the OKC from other cysts or neoplastic lesions and can therefore alter surgical management.

Mandibular infected buccal cyst and paradental cyst: the same or separate entities?

The WHO classification of odontogenic cysts includes the radicular (residual) and paradental (inflammatory collateral; mandibular infected buccal) cysts as inflammatory odontogenic cysts. Because the paradental group of inflammatory cysts may clinically cause diagnostic and therapeutic problems when they are associated with first and second molar teeth it was decided to review the literature and present an additional case. The aetiology and histological features of the inflammatory collateral cyst, the paradental cyst and the mandibular infected buccal cyst are identical and the differences that exist in their clinical and radiological presentation can be related to the different teeth that are involved and the difference in the ages at which these teeth erupt. These cysts represent the same entity and their treatment is dependent on the tooth involved. With the aid of magnetic resonance imaging (MRI) the distinction between a paradental cyst on the buccal aspect of a molar tooth and a periodontitis can be made.

Pneumosinus dilatans of the ethmoid sinus--incidental finding on a cephalometric radiograph.

A case of pneumosinus dilatans of the posterior ethmoidal region is presented. The condition known as pneumosinus dilatans is discussed, as are accessory ethmoidal air cells with respect to their most common localization and radiographic appearance. The diagnostic imaging features of this condition are described and a recommendation of views to adequately delineate this disorder are given. The importance of an adequate and thorough review of the cephalometric radiograph for pathologic conditions is stressed.

Ewing's tumor of the jaw.

Previous reports have focused on the fact that laminar periosteal new bone is a common radiologic sign of Ewing's sarcoma of the jaws. Critical analysis of the literature does not lead to this conclusion. A case is reported and radiologic appearances described. A review of the literature shows striking clinical and radiologic similarities to osteogenic sarcoma of the jaws. A differential radiologic interpretation of Ewing's sarcoma of the jaws is offered.
