Glycopyrrolate as a premedicant: comparison with atropine.

A double-blind comparison of glycopyrrolate with atropine as the anticholinergic component of premedication was made in 200 patients undergoing minor or intermediate surgical procedures. Glycopyrrolate was associated with a smaller increase in heart rate, but there was no difference between the drugs in respect of cardiac arrhythmia, change in arterial pressure, control of secretions in the upper respiratory tract, frequency of nausea and vomiting after operation or subjective well-being of the patients.

An ultrastructural study of the placenta in materno-fetal rhesus incompatibility.

An electronoptical study has been made of eleven placentae from cases of materno-fetal rhesus incompatibility. The characteristic findings are focal, but sometimes quite extensive, syncytial necrosis, retention of normal pinocytotic and secretory activity in the non-necrotic syncytiotrophoblast, cytotrophoblastic hyperplasia, thickening of the trophoblastic basement membrane, immature-type endothelial cells in the fetal villous vessels and thickening or lamination of the capillary basement membranes. The pathogenesis of many of these chantes is not clear but there is no evidence that they are immunologically mediated. It is suggested that the syncytial necrosis may be due to narrowing of the intervillous space as a result of increased villous size, that the cytotrophoblastic hyperplasia is a response to the syncytial damage and is responsible for the changes in the trophoblastic basement membrane and that the fetal capillary changes are indicative of endothelial cell damage due, possibly, to fetal anaemia. Despite the damage suffered by the placenta in materno-fetal rhesus incompatibility there is little evidence of impaired functional efficiency.

The identification of glycoproteins associated with elastic-tissue microfibrils.

Cell cultures derived from foetal bovine ligamentum nuchae accumulate extracellular fibrils morphologically identical with elastic-tissue microfibrils. Two glycoproteins synthesized by the ligament cells are closely related to the matrix microfibrils as assessed by immunological and chemical extraction techniques.

Ultrastructure of the placenta in prolonged pregnancy.

An electron-microscopic study has shown that in placentae from prolonged pregnancies there is hyperplasia of the villous cytotrophoblastic cells, degeneration of occasional isolated cytotrophoblastic cells, a paucity of syncytial microvilli in some areas and microvillous proliferation in others, abnormalities of microvillous form, focal syncytial necrosis, dilatation of syncytial endoplasmic reticulum, decreased synctial pinocytic activity, a reduced size and number of syncytial mitochondira, thickening of the trophoblastic basement membrane and contraction, blebbing and vacuolation of endothelial cells of the stromal foetal villous capillaries. Many of these morphological changes indicate that there is, in prolonged pregnancy, a decline in trophoblastic functional activity but it is suggested that this is due, not to placental senescence, but to a relatively mild, sustained uteroplacental ischaemia. Although there is morphological evidence to suggest that compensatory mechanisms come into play to limit the effects on the placenta of uteroplacental ischaemia the changes in the foetal capillaries, which are of unknown pathogenesis, will result in decreased foetal perfusion of the placenta and thus restrict still further adequate materno-foetal transfer.

Evaluation of the anticholinergic actions of glycopyrronium bromide.

1. Glycopyrronium was evaluated by intramuscular, intravenous and oral routes in six adult volunteers for its efficacy as an antisialogogue as also for its action on other aspects of cholinergic activity. 2. It was found to be an effective antisialogogue of long duration of action by all three routes. When given orally the effects were delayed in onset and persisted for too long. Intramuscular and intravenous routes were useful. The intramuscular absorption of the drug is rapid and consistent. 3. Sweat gland activity was affected in a similar fashion but less consistently and other parameters were mostly unaffected. 4. Glycopyrronium 0.2 mg intramuscularly was found to be the optimal dose. Larger doses produced subjective discomfort out of proportion to a further reduction in salivary secretion. 5. Intravenous administration causes no changes in cardiovascular stability.

A possible role for placental lysosomes in the formation of villous syncytiotrophoblast.

An ultrastructural and ultrahistochemical study of first trimester human placentae confirms previous reports that the cytotrophoblastic cells show a spectrum of differentiation, that dissolution of the limiting membrane of the cytotrophoblastic cells occurs and that fragments of free membrane can be found in the syncytiotrophoblast. There is an aggregation of primary lysosomes in the region of approximation of the cytotrophoblast to the syncytiotrophoblast, free lysosomal enzymes are found in the space between the two trophoblastic components, secondary lysosomes have been noted in the vicinity of fragmenting cytotrophoblastic cell membrane and the incorporation of a segment of free membrane into a vesicular structure has been noted. It is suggested that placental lysosomes mediate the dissolution of the cytotrophoblastic cell membranes that is a necessary prerequisite for their full differentiation into syncytiotrophoblast and it is further suggested that one of the principal roles of placental lysosomes is in the structural refashioning of the organ that occurs during the first trimester.

Effects of training on the visual recognition of consonants.

Visual recognition of consonants was studied in 31 hearing-impaired adults before and after 14 hours of concentrated, individualized, spechreading training. Confusions were analyzed via a hierarchical clustering technique to derive categories of visual contrast among the consonants. Pretraining and posttraining results were compared to reveal the effects of the training program. Training caused an increase in the number of visemes consistently recognized and an increase in the percentage of within-viseme responses. Analysis of the responses made revealed that most changes in consonant recognition occurred during the first few hours of training.

Evaluation of doxapram for arousal from general anaesthesia in outpatients.

In a double-blind study of 100 outpatients receiving methohexitone, nitrous oxide and halothane, the administration of doxapram 80 mg i.v. was associated with a significantly more rapid recovery from anaesthesia, the effect being more obvious in women than men. Doxapram produced a reduction of 3-4 min in the time of recovery to a safe level of consciousness. The effect was probably a result of the more rapid elimination of halothane caused by stimulation of respiration. In the patients who received doxapram the incidence of minor complications during recovery was reduced and there was some evidence of an improvement in the quality of recovery.

Evidence for two nucleotide sequence orientations within the terminal repetition of adeno-associated virus DNA.

Duplex adeno-associated virus (AAV) DNA, produced by annealing plus and minus virion single strands, has been digested with several bacterial restriction endonucleases. These studies reveal the existence of alternate secondary structures at the termini of duplex AAV DNA. Analysis of the sites of endo R-Hpa II cleavage, the products of complete endo R-Hpa II digestion, and the multiple terminal secondary structures leads to the conclusion that there are two possible nucleotide sequences at each end of AAV DNA. A model that attributes the terminal nucleotide sequence heterogeneity to two possible orientations of the first 120 nucleotides at each end of the DNA is proposed; in one case the sequence is 1 to 120; in the other case the sequence is inverted. An origin of the inversion is suggested based on previously described intermediates in AAV DNA replication.

Preliminary evaluation of a new psychotropic drug, lenperone, in the treatment of acute schizophrenia.

An open trial of a new psychotropic drug, lenperone, was carried out in 10 patients with acute schizophrenia. The duration of treatment was 28 days and the maximum dose administered was 60 mg per day. A pronounced antipsychotic effect was seen in half of the patients, although all 10 patients showed a pharmacological response to the drug. The incidence of side-effects was low. Haematological, biochemical and electrocardiographic tests failed to demonstrate any abnormality due to treatment.

Syncytial knots and intervillous bridges in the human placenta: an ultrastructural study.

An electron microscopic study has shown that the syncytial knots of the villi of the human placenta contain aggregated nuclei which exhibit marked degenerative changes; within the cytoplasm there is an abundance of cytoplasmic filaments and many stacks of annulate lamellae. It is suggested that syncytial knots are a sequestration phenomenon in which senescent nuclear material is aggregated and removed from metabolically active areas of the syncytiotrophoblast. Intervillous bridges appear to be formed chiefly by fusion of syncytial knots from adjacent villi, and it seems reasonable that the effete material in a syncytial knot should be used for this purpose. The intervillous bridges hava a fine structure which suggests that they have a mechanical function, and this lends support to the theory that they form an internal strut system within the placenta.

Role of doxapram in reducing pulmonary complications after major surgery.

In a double-blind study patients given a single dose of doxapram 1-5-2-0 mg/kg combined with morphine postoperatively had a significantly lower incidence of reflex postoperative cough and expectoration of purulent sputum than patients given morphine alone. They also had significantly higher arterial oxygen tensions five days postoperatively. Smaller differences were found with naloxone combined with morphine.
