In the imaging of tumors using radiolabeled monoclonal antibodies, the use of PET gives increased sensitivity over conventional gamma camera imaging techniques. Copper-64, a positron-emitting radionuclide, has been labeled to 1A3, an anticolorectal carcinoma monoclonal antibody, and its fragments 1A3-F(ab')2 utilizing the bifunctional chelate Br-benzyl-TETA. The 64Cu-labeled intact 1A3 and 1A3-F(ab')2 have been evaluated as potential imaging agents for PET. Biodistribution studies of 64Cu-benzyl-TETA-1A3 and 64Cu-benzyl-TETA-1A3-F(ab')2 in tumor-bearing hamsters were compared with those of 111In-Br phi HBED-1A3, 111In-Br phi HBED-1A3-F(ab')2 and 125I-labeled intact 1A3 and 1A3-F(ab')2. Tumor uptake of 64Cu-labeled intact 1A3 and fragments in the hamster model was superior to both 111In- and 125I-labeled intact 1A3 and fragments. Human dosimetry data for 64Cu- and 123I-labeled 1A3 and 1A3-F(ab')2 were calculated from biodistribution data in rats. High kidney uptake of 64Cu-benzyl-TETA-1A3-F(ab')2 precludes clinical study at this time; however, the data shows that 64Cu-benzyl-TETA-1A3 would be suitable for positron tomography imaging of colorectal cancer in patients.
In an effort to develop radiopharmaceuticals useful for the diagnostic imaging of steroid receptor-positive breast tumors, we have radiolabeled an analog of the antiprogestin RU486 (mifepristone), modified to incorporate an N2S2 chelate system in the 11 beta-position, with 99Tc, 99mTc, and 186Re. For the 99Tc-labeled analogs (3), a syn pair and two individual antidiastereomers (linker methylene versus metal-oxo, relative to the N2S2 plane) were isolated. In competitive radiometric binding assays, the syn pair (3syn1,2) had affinity for the progesterone receptor that was 25% that of (promegestone) R5020 (or 161% that of progesterone), and the individual anti-diastereomers had affinities of 47% (3anti1) and 7% (3anti2) that of R5020 (or 303% and 45% that of progesterone). The specific-to-nonspecific binding ratio of the 99mTc (4) and 186Re (5) 11 beta-linked syn systems are 75/25 and 54/46, respectively. In vivo, conjugates 4 and 5 showed progesterone receptor-mediated uptake in rat uterus, but also high uptake in non-target tissues, presumably because of the high lipophilicity of the metal complexes. Modified systems may be useful in vivo as receptor-directed agents for diagnostic imaging or treatment of steroid receptor-positive tumors.
A retrospective review of methotrexate (MTX) treatment assessed the clinical course in 124 rheumatoid arthritis (RA) patients. After 5 years, 39 (31%) patients continued MTX with clinical benefit. Although patients continuing MTX after 5 years were younger (45 +/- 13 v 54 +/- 12 yrs, P less than .001) and had a shorter disease duration of RA (9.3 +/- 8.1 v 14 +/- 11 yrs, P less than .05) than patients who discontinue the drug, these differences were not considered clinically significant. MTX was discontinued in 20 patients for a lack of clinical benefit, in 21 patients for non-drug-related reasons, and in 44 patients for suspected adverse drug reactions. The adverse drug reactions requiring permanent discontinuation of MTX were nausea, stomatitis, hair loss, rash, pulmonary reactions, elevated liver enzymes, hematologic abnormalities, and hepatic fibrosis. At least one adverse drug reaction was reported by 115 (93%) patients receiving MTX, but the majority did not require permanent drug discontinuation. Although the prevalence of adverse reactions increased with longer duration of therapy, no differences existed in the type of reactions reported over 5 years of treatment. There were no risk factors identified that were clearly associated with the development of toxicity. Long-term therapy was primarily limited by adverse reactions rather than loss of efficacy.
Though consumers generally favor hospital advertising, it has not emerged as a major factor in hospital choice research. A survey examined the effectiveness of hospital advertising and its strength as a factor in hospital choice. The majority of the respondents approved of hospital advertising and showed knowledge of hospital specialties. However, hospital choice was influenced primarily by physician recommendation and location, rather than advertising.

Reiter's syndrome is classically described as the triad of urethritis, conjunctivitis, and arthritis, It has many manifestations and has rarely been reported to occur in association with amyloidosis. Four cases of systemic amyloidosis have previously been reported. This case describes a patient who developed progressive renal amyloidosis after a 17-year history of severe Reiter's syndrome. Immunofluorescent staining of the renal biopsy was strongly positive for AA protein, the type of protein found in secondary amyloidosis. This is the first case in which amyloidosis has been proven to be secondary to Reiter's syndrome and not merely the coincidental occurrence of two rare diseases.
Ss judged "ratios of distances" and "differences of distances" between pairs of U.S. cities. Results fit the theory that Ss used two comparison processes as instructed. A ratio scale of distances between cities was constructed from the 2 rank orders. From this scale, an interval scale of the city locations on an east-west continuum was derived. This scale agrees with the subtractive model fit to "ratios" and "differences" of easterliness and westerliness, and it also agrees with multidimensional scaling of judged distances between cities. These findings are consistent with the theory that Ss use subtraction when instructed to judge either "ratios" or "differences," but that they can use both ratio and difference operations when the stimuli (in this case, distances) constitute a ratio scale on the subjective continuum.

An irregular proteinaceous film populated by sessile macrophages and large giant cells was found on the surface of a posterior chamber lens implant. This had to be removed from a position with its haptics in the capsular bag 8 months after surgery because of increased uveitis. The implant had been placed at the time of extracapsular cataract extraction in a patient with pre-existing active bilateral chronic iridocyclitis of a non-granulomatous type. An anterior chamber lens implant placed after intracapsular cataract extraction in the other eye of the same patient was tolerated, and the same type of implant was used to replace the removed posterior chamber lens implant.
Localized intracochlear injections of the fluorescent retrograde label diamidino yellow were used to investigate the organization of efferent projections from the brainstem to different turns of the cochlea, in the guinea pig. It was found that the location of small neurones within the lateral superior olive ipsilateral to the injection varied in a systematic manner when injections proceeded from base to apex of the cochlea. In addition, a cruder form of cochleotopic organization was present in that most of the large medial system efferent neurones were labelled only after injection into the 3 most basal turns of the cochlea. The decline of medial system efferent innervation proceeding from base to apex was most striking for the contralateral efferent neurones. The details of base to apex innervation density were different for the different nuclei of origin of the medial system, implying the existence of complex subsystems within the medial neurone population.

It is a strange paradox that now that the treatment of venereal diseases is comparatively painless, effective and easily available, there has been a considerable upsurge in the incidence thereof. A global picture of the success of preventive and curative measures is difficult to obtain since comprehensive statistics are not available. The single most important reason for this is that these diseases are not notifiable, whether treated in the clinics of local authorities or hospital environs or by private practitioners, making the detection of accurate epidemiological trends extremely difficult. Since patients often desire anonymity, there has been an increasing drift towards treatment at the last two sources mentioned above. This article studies the only available recorded information, and hence summarizes the effectiveness of the Cape Town City Council and Cape Divisional Council health departments in combating sexually transmitted diseases in the Western Cape.
The wide spectrum of associated disorders, the previous lack of understanding of pathogenesis, and the older, arbitrary terminology in relation to ultimate prognosis have led to confusion in the evaluation and management of Raynaud's syndrome. A unified concept of pathogenesis, in which vasospasm and arterial occlusive disease are the fundamental lesions, is presented herein. The associated disorders found in our 219 patients with continuous follow-up are listed and related to the underlying pathogenetic mechanisms. Our approach to diagnosis led to the discovery of an associated condition in 71 percent of our patients. The evaluation can be done in a cost-effective manner. The emphasis of treatment should be on conservative medical management, with avoidance of cold and tobacco and judicious use of pharmacologic agents remaining the cornerstones of therapy. Cervicothoracic sympathectomy has no role in the treatment of Raynaud's syndrome.
The safety and patient acceptance of two preparations of modified (reduced and alkylated) immune globulin for intravenous use were evaluated; one preparation was formulated as a 5% solution in 10% maltose (IGIV-maltose), the other did not contain maltose (IGIV). In this double-blind trial each of 29 immunodeficient patients received three consecutive monthly infusions (100 or 150 mg/kg immune globulin) of one preparation before being crossed over to the other. Only 3 of 29 patients had adverse reactions when on IGIV-maltose, compared with 22 who had side-effects during infusions of IGIV (p < 0.001). Adverse reactions were recorded during 3 of 87 IGIV-maltose infusions and during 51 infusions with the maltose-free IGIV (p < 0.001). 27 patients expressed preference for IGIV-maltose. IGIV-maltose seems safe and will permit rapid infusion of large doses of immune globulin, thus improving the management of patients with antibody deficiency diseases.
